A clinical observation was made on the pyonephrosis of the in-patient in the Department of Urology, Seoul National University Hospital during the period from January, 1965 to June, 1969 and the results were as follows: 1. Of 1,420 cases hospitalized, 11 cases were pyonephrosis, giving a rate of 0.77%. Age distribution ranged from 22 to66 years with the average of 40 years and the most prevalent was fourth and fifth decade. 2. As to the etiologic factors of the pyonephrosis,4 cases were due to non-specific inflammation, 2 of whom diabetes mellitus, 3 renal tuberculosis, 2 renal or ureter stone, 1 congenital ureteropelvic junction stenosis and 1 postirradiative ureteral obstruction. 3. The most common complaints were pain on the involved site in 10 cases. Palpable mass, fever and chill, and G-1 symptoms were found in 8 cases respectively. Costovertebral angle tenderness, turbid urine and anemia were noted in 6 cases and frequency in 5 cases, hematuria in 4 cases and dysuria, general weakness, headache and or dizziness in 3 cases respectively. 4. Among the 11 cases, 9 showed non-function on intravenous pyelogram, 1 showed delayed visualization with calyceal displacement downward. On retrograde pyelography, 5 cases showed moderate hydronephrosis, 3 cases showed advanced hydronephrosis and in 3 cases retrograde pyelography failed due to ureteral obstruction. 5. Nephrectomy was performed in 9 cases, nephrectomy after nephrostomy in I case, and no operation in 1 case.

A total of 1,498 small mammals (rodents and insectivores), including Apodemus agrarius (n = 1,366), Crocidura lasiura (54), Mus musculus (32), Micronytus fortis (28), Eothenomys regulus (9), Micronys minutes (6), and Cricetulus triton (3), were live-trapped in Gyeonggi-do (Province) (Paju-si, Pocheon-gun, and Yeoncheon-gun) near the demilitarized zone (DMZ) from December 2004 to September 2005. A. agrarius was found to be infected with 3 species of echinostomes (Echinostoma hortense, Echinostoma cinetorchis, and Euparyphium murinum), while C. lasiura was infected with 1 species (Echinochasmus japonicas) of echinostome. Other mammals were free from echinostome infections. Total 16 E. hortense were detected in 7 (0.5%) mice, 9 E. cinetorchis from 5 (0.4%), and 3 E. murinum from 2 (0.1%) out of 1.366 A. agrarius examined. E. japonicus was found only in 1 (1.9%; total 3 specimens) C. lasiura. These results demonstrate that A. agrarius and C. lasiura, inhabiting near the DMZ of Gyeonggi-do serve as the natural definitive hosts for several species of echinostomes, although their infection rates are low. This is the first record of natural infections of A. agrarius with E. cinetorchis and C. lasiura with E. japonicus in the Republic of Korea.

Purpose: Infection with HIV-1 virus has become a critical worldwide public health problem. The oral complications of HIV infection with its progression of Impairment of the host response to combat infection present unique challenges to the periodontists. Material and Methods : Medline research was carried out to find relationship of the progression of HIV infection to the occurrence of oral lesions including the HIV-related periodontal diseases. Results; The linear gingival erythema, necrotizing ulcerative periodontitis, necrotizing ulcerative gingivitis and oral candidiasis are common lesions in HIV-infected individuals. The linear gingival erythema and necrotizing ulcerative periodontitis lesions in HIV-infected subjects were found to have a similar microbiological profile. There are several general considerations in the periodontal management of the HIV-infected patient with or without periodontal disease. The altered immunity and host response in patients with HIV infection may also affect the incidence and severity of other common forms of periodontal disease not associated with HIV infection. Conclusion: Periodontal diseases in HIV-infected individuals present unique challenges in diagnosis, monitoring, treatment and maintenance. Therefore exact HIV staging, geographic location, antiviral and antimicrobial therapies and oral habits should be taken into consideration when treating HIV-infected patients.

Purpose: The aim of this study was to evaluate the patient’s and dentist’s perspective and knowledge on rapid human immunodeficiency virus screening test in dental setting. Material and Methods: In March 2009, 100 patients and 100 dentists at College of Dentistry, Yonsei University were provided an attitude assessment survey. Results: Results were analyzed for acceptance of testing and potential barriers. 94% of patients agreed to take a rapid HIV screening test and 77% of dentists were willing to provide the test to their patients. Also the current knowledge state of patients and dentists on HIV infection was not accurate as expected. Conclusion: Overall, dental clinic patients widely accepted the offer of rapid oral HIV screening. Rapid HIV screening test in the dental setting can be an important option to increase the number of individuals who know their HIV status.

Purpose: There is no consensus regarding the relationship between the width of keratinized mucosa and the health of periimplant tissues, but clinicians prefer to provide enough keratinized mucosa around dental implants for long-term implant maintenance. An apically positioned flap during second stage implant surgery is the chosen method of widening the keratinized zone in simple procedures. However, the routine suture techniques used with this method tend to apply tension over the provisional abutments and decrease pre-existing keratinized mucosa. To overcome this shortcoming, a pre-fabricated implant-retained stent was designed to apply vertical pressure on the labial flap and stabilize it in a bucco-apical direction to create a wide keratinized mucous zone. Methods: During second stage implant surgery, an apically displaced, partial thickness flap with a lingualized incision was retracted. A pre-fabricated stent was clipped over the abutments after connecting to the provisional abutment. Vertical pressure was applied to displace the labial flap. No suture was required and the stent was removed after 10 days. Results: A clinically relevant amount of keratinized mucosa was achieved around the dental implants. Buccally displaced keratinized mucosa was firmly attached to the underlying periosteum. A slight shrinkage of the keratinized zone was noted after the healing period in one patient, but no discomfort during oral hygiene was reported. Clinically healthy gingiva with enough keratinized mucosa was achieved in both patients. Conclusions: The proposed technique is a simple and time-effective technique for preserving and providing keratinized tissue around dental implants.

High temperature fuel cell system, such as molten carbonate fuel cells(MCFC) and solid oxide fuel cells(SOFC), are capable of operating at MW rated power output. The power output change of high temperature fuel cell imposes the thermal and mechanical stresses on the fuel cell stack. To minimize the thermal-mechanical stresses on the stack and increase the systems reliability, we should divide the power plant configuration to several banks. However, the improvement of reliability in fuel cell power plant system causes an increase of the investment cost, for example, replacement costs, labor costs, and so on. For this reason - the balance between investment and reliability improvement - many studies about the appropriate level of investment have been conducted. In this paper, we evaluate the cost for operation and installation, the benefit for electric energy and thermal energy sales, and the system reliability for several cases : these cases relate with the bank configuration.

High temperature fuel cell system, such as molten carbonate fuel cells(MCFC) and solid oxide fuel cells(SOFC), are capable of operating at MW rated power output. The power output change of high temperature fuel cell imposes the thermal and mechanical stresses on the fuel cell stack. To minimize the thermal-mechanical stresses on the stack, increases in the power output of high temperature fuel cell typically must be made at a slow rate. So, the short time interruption of high temperature fuel cell causes considerable generated energy losses. Because of the characteristic of high temperature fuel cell, we analyzed the impact of electrical fault in the fuel cell plant on other fuel cell generators in the same plant site. A various grounding configuration and voltage sag are analyzed. Finally, we presented the solution to minimize the effect of fault on other fuel cell generators.
