Metastases to orbit occur rarely but are the most common form of malignant orbital tumors. Usually the orbital metastases occur in disseminated disease. Very rarely orbital metastases can be the presenting feature of systemic malignancy. We report here a case with orbital metastases as the presenting feature of lung malignancy and describe the utility of FDG PET/CT as one-stop-shop imaging modality in this setting.

We present a case of papillary thyroid cancer (post-thyroidectomy status) on regular treatment with suppressive Levothyroxinetherapy. On follow-up at 6 months after radioactive iodine ablation for remnant thyroid tissue, her thyroglobulin, and antithyroglobulinlevels were 0.06 ng/ml and 670 IU/ml, respectively. Low-dose whole-body I-131 scan was negative. To lookfor the cause of isolated increased anti-thyroglobulin level, a whole-body 18F-FDG PET/CT was done which revealed multipleFDG-avid lytic skeletal lesions suggestive of metastases. For confirmation of diagnosis, 18F-FDG PET/CT-guided metabolicbiopsy was done, which revealed Langerhans’ cell histiocytosis on histopathological examination.

The rapidly evolving clinical utility of prostate-specific membrane antigen (PSMA) PET/CT as an imaging modality for prostate cancer (PCa) has brought to the forefront a multitude of non-prostatic diseases also exhibiting PSMA radioligand uptake. We report a case of a 71-year-old male with PCa who underwent PSMA PET/CT for recurrence evaluation. The scan demonstrated [68 Ga]Ga-PSMA-11 uptake in the distal ileum. Retrospective enquiry revealed that the patient was a known case of Crohn’s disease. This case highlights an uncommon finding of PSMA radioligand uptake in the distal small bowel (non-prostatic benign pathological uptake) in a patient with known Crohn’s disease, which may be misinterpreted in the evaluation of PCa.

Testicular torsion is a common differential diagnosis of acute scrotal pain along with acute epididymo-orchitis, which may lead totesticular non-viability. Doppler ultrasound and testicular scintigraphy are two routinely used modalities for the assessment oftesticular viability. However, in some cases, these investigations may prove inadequate in differentiating between the two entitieswith widely differing management. Here, we present a case of a 52-year-old male with questionable viability of testis, who wasinvestigated initially using testicular scintigraphy and was further subjected to a regional 18F-FDG PET/CT scan, in view ofinconclusive findings with the conventional modalities.

Hodgkin’s lymphoma most commonly presents as painless lymphadenopathy and systemic B symptoms like fever, nightsweats, and weight loss. But rarely, it can present with unusual paraneoplastic manifestations. In the present case, we reporta 30-year-old woman who presented with sub-acute onset pan-cerebellar symptoms. The serum neuronal antibody panel wasnegative. She underwent F-18 FDG PET/CT to evaluate the neoplastic etiology as the underlying cause, which demonstratedboth the primary etiology and the paraneoplastic changes in the brain.
