      • Prognostic Factors and Survival in Acute Myeloid Leukemia Cases: a Report from the Northeast of Iran

        Background: Acute myeloid leukemia (AML) is a clonal hematopoietic disorder resulting from genetic alterations in normal hematopoietic stem cells. The aim of this study was to evaluate prognostic factors and survival of AML patients in the Northeast of Iran. Materials and Methods: This retrospective study covered 96 patients with AML referred to Emam Reza Hospital, Mashhad city, Iran, from 2009 to 2015. Age, sex, blood group, type of AML, fever, consumption of amphotericin B, cytogenetic forms and survival were analyzed. Also, WBC, hemoglobin and platelet levels were checked. Mean follow-up was 30.5 months (60.4% mortality). Survival was plotted by GraphPad Prism 5 with Log-rank test. Results: The mean age for all AML patients at diagnosis was 40.4 years (range, 17-77 years). Some 42.7% patients were aged <35 years and 40.6% were male. In all patients, 76% had fever and 50% consumed amphotericin. T(15;17)(q22;q21) had the most prevalence (37.7%) compared to other forms. Out of 92 patients, O+(30.4%) was the most common blood group and AML-M5 (28.3%) the most common subtype. There was a significant difference in survival based on WBC and consumption of amphotericin B (P<0.05). Conclusions: WBC level, fever and consumption of amphotericin B proved to be factors for survival of AML patients. The mean age for patients in Iran is lower than other areas in the World and also survival in this study was higher than in other studies.

      • Frequency of FLT3 (ITD, D835) Gene Mutations in Acute Myelogenous Leukemia: a Report from Northeastern Iran

        Background: FLT3 is mutated in about 1/3 of acute myelogenous leukemia (AML) patients. The aim of the present study was to report the prevalence of FLT3 mutations and comparison with prognostic factors in AML patients in the Northeastern of Iran. Materials and Methods: This cross-sectional study concerned 100 AML cases diagnosed based on bone marrow aspiration and peripheral blood. DNA for every AML patient was extracted and underwent PCR with FLT3-ITD primers. Results: The mean age at diagnosis was 28.5 years (range, 1-66 years), 52 patients (52%) being male. Out of 100 AML patients, 21 (21%) had FLT3 mutation, (17 with FLT3-ITD, 81%, and 4 with FLT3-D825, 19%). Of the 21, 14 (66.7%) had heterozygous mutation. There was no significant difference between age, sex and organomegaly between patients with FLT3 mutation versus FLT3 wild-type. Conclusions: Our frequency of FLT3 is in line with earlier fidnings of approximately 20 to 30% and also the prevalence of FLT3-ITD is more than FLT3-D35 mutation. There was no significant difference between prognostic factors (age and sex) in the patients with FLT3 mutation versus FLT3 wild-type. The prevalence of FLT3 heterozygous mutations is more that homozygous mutations in AML patients.

        Mechanical behavior of steel-concrete composite decks with perfobond shear connectors

        Exodermic deck systems are new composite steel grid deck systems which have been used in various projects during the past decade. One of the eminent features of this system is considerable reduction in the structure weight compared to the ordinary reinforced concrete decks and also reduction in construction time by using precast Exodermic decks. In this study, dynamic properties of the Exodermic deck bridges with alternative perfobond shear connectors are investigated experimentally. In order to evaluate the dynamic properties of the decks, peak picking and Nyquist circle fit methods are employed. Frequencies obtained experimentally are in good agreement with the results of the finite-element solution, and the experimental results show that the first mode is the most effective mode among the obtained modes. The first four modes are the rigid translational motion modes, and the next two modes seem to be rigid rotational motion modes around a horizontal axis. From the 7th mode onwards, modes are flexible. The range of damping ratios is about 0.5%. Furthermore, the static behavior of the Exodermic decks under a static load applied at the center of the decks was investigated. Failure of the decks under positive bending was punching-shear. The bending strength of the decks under negative bending was about 50 percent of their strength under positive bending. In addition, the weight of an Exodermic deck is about 40% of that of an equivalent reinforced concrete slab.

        Health Risk of Potato Farmers Exposed to Overuse of Chemical Pesticides in Iran

        Background: Potato is the main crop of Ardabil Plain (accounting for one-fifth of potato production in Iran). Its health hazard risk to farmers is rising due to the increasing rate of pesticide use. The present study analyzes potato farmers' health hazard risk in the use of chemical pesticides. Methods: The rate of pesticide use by farmers (n = 370) was first compared with the recommended dosage (on pesticide label). Then, a composite index was employed to estimate the health hazard risk of farmers during pesticide use, and the variables accounting for pesticide overuse and nonoveruse were analyzed. Safety behavior was examined in four steps, namely of pesticide purchase and storage, preparation, application, and postapplication. Results: It was found that 74.6 percent of potato farmers used pesticides in higher concentrations than the recommended dosage. The higher average rate of pesticide use versus recommendation (label instruction) was related to Chlorpyrifos and Trifluralin, and the highest average health hazard risk among farmers was related to the use of Chlorpyrifos and Metribuzin. Farmers with a higher risk of health hazard displayed much lower safety behavior than the other farmers at all steps of pesticide use. Conclusion: The most important variables discriminating the health hazard risk of farmers' overuse included health behavior identity, attitude, knowledge and awareness, and cues to action. Therefore, using social media, holding local exhibitions, and engaging local leaders and skilled farmers in the region to improve farmers' attitudes and health behavior identity toward the dangers of chemical pesticides can play a significant role in motivating farmers' display of overuse preventive behaviors.

        Effects of Tanacetum polycephalum on passive avoidance learning and oxidative stress in epileptic model of memory impairment in the male Wistar rats

        In the present study, the medicinal plant Tanacetum polycephalum, was used to evaluate its effect on oxidative stress and memory deficit caused by PTZ in the rats. Twenty-four rats were randomly categorized into four groups: control negative under treatment with PTZ (sub-threshold dose 35 mg kg−1 for 1 month), control positive under treatment with phenobarbital (PB 30 mg kg−1), and two PTZ groups under treatment with T. polycephalum extract (TPE 300, and 600 mg kg−1). Standard passive avoidance test was used to evaluate memory and learning. After behavioral evaluation, the rats were anesthetized and their brains were removed; following the preparation of brain homogenates, oxidative stress parameters were evaluated using specified methods. It was found that step-through latency in passive avoidance test in PTZ group receiving TPE was greater than that of PTZ group. In addition, brain lipid peroxidation (malondialdehyde) and nitric oxide level in TPE-treated group was significantly lower and glutathione activity and superoxide dismutase level were significantly greater in comparison to PTZ group. Our results showed that TPE could prevent memory deficit and ameliorate hippocampal oxidative stress in PTZ-kindled rats.

      • Persian Version of Functional Assessment of Cancer Therapy- Breast (FACT-B) Scale: Confirmatory Factor Analysis and Psychometric Properties

        Background: The Functional Assessment of Cancer Therapy - Breast (FACT-B) scale is widely used to measure health-related quality of life in cancer patients. The aim of the present study is to validate the FACT-B in a sample of Iranian women with breast cancer. Materials and Methods: The sample consisted of 300 women selected through non-random convenient sampling procedure from oncology hospitals and clinics in Kermanshah and Shiraz cities. They were asked to fill in the Persian versions of the FACT-B scale, Hospital Anxiety and Depression Scale, the European Organization for Research and Treatment of Cancer quality of life EORTC QLQ30. Confirmatory factorial analysis of the methods, concurrent validity and discriminant, and Cronbach's alpha for internal consistency were applied. Results: Internal consistency using Cronbach's alpha was 0.63 to 0.93 for the subscales and 0.92 for the total scale. Significant correlations between FACT-B and other measures indicate that this scale had concurrent and discriminant validity. The values of fit indices were satisfactory. Conclusions: The Persian version of the FACT-B scale is valid and reliable and, therefore, the scale can be used in research and clinical settings to assess health-related quality of life in Iranian patients with breast cancer.

        In vitro evaluation of the fracture resistance and microleakage of porcelain laminate veneers bonded to teeth with composite fillings after cyclic loading

        PURPOSE. There is insufficient data regarding the durability of porcelain laminate veneers bonded to existing composite fillings. The aim of the present study was to evaluate the fracture resistance and microleakage of porcelain laminate veneers bonded to teeth with existing composite fillings. MATERIALS AND METHODS. Thirty maxillary central incisors were divided into three groups (for each group, n=10): intact teeth (NP), teeth with class III composite fillings (C3) and teeth with class IV cavities (C4). Porcelain laminate veneers were made using IPS-Empress ceramic and bonded with Panavia F2 resin cement. The microleakage of all of the specimens was tested before and after cyclic loading ($1{\times}10^6$ cycles, 1.2 Hz). The fracture resistance values (N) were measured using a universal testing machine, and the mode of failure was also examined. The statistical analyses were performed using one-way ANOVA and Tukey post hoc tests (${\alpha}=.05$). RESULTS. There was a significant difference in the mean microleakage of group C4 compared with group NT (P=.013). There was no significant difference in the fracture loads among the groups. CONCLUSION. The microleakage and failure loads of porcelain laminate veneers bonded to intact teeth and teeth with standard class III composite fillings were not significantly different.

        Reciprocal indirect interactions between Tetranychus urticae and Aphis gossypii mediated by cucumber plant

        Induced plant responses to herbivory can alter plant quality and influence subsequent interactions with organisms that use that plant as food source. In this study, we conducted several experiments in order to understand whether preference and performance of the cotton aphid, Aphis gossypii (Hem: Aphididae) and the spider mite, Tetranychus urticae (Acari: Tetranychidae) are affected by the previous herbivory of conspecific or heterospecific species on cucumber plants. Longevity, fecundity and pre-imaginal development time were measured as performance criteria. In addition, we explored whether these effects are local or systemic. In the case of performance experiments, the results varied from negative to neutral depending on the performance criteria, no positive effect was observed in studied interactions. Also, depending on performance criteria, the previous herbivory affected the plant systemically, or locally. Results of preference experiments indicated that mites and aphids prefer to settle and produce offspring on control leaf disc to prevent detrimental effects of the previous herbivory. Spider mites showed a stronger preference than aphids in the detection of induced leaf discs, which resulted in more mites being present and laying more egg on control leaf disc in both local and systemic treatments. Performing both preference and performance experiments, not only reveals the effect of herbivores on each other's mediated by the plant but also reveals more information about the sensitivity of herbivores to change in the quality of their host.
