        Question mark ear deformity: a case study and surgical technique

        Clefts of the ear are rare and typically develop within the earlobe. They were first de scribed by Vincent et al. in association with urogenital anomalies. Subsequently, Cos man reported them in 1970, referring to the condition as “question mark ear,” which led to its naming. This deformity is infrequently reported and is not well-documented in the literature. In this paper, we present the first reported case in the Omani popula tion of bilateral Cosman ear, associated with a mild cranial deformity. We also discuss the social impact of this condition and introduce our corrective technique for moder ate forms of deformity.

      • In Vitro Apoptosis Triggering in the BT-474 Human Breast Cancer Cell Line by Lyophilised Camel's Milk

        Breast cancer is a global health concern and is a major cause of death among women. In Oman, it is the most common cancer in women, with an incidence rate of 15.6 per 100,000 Omani females. Various anticancer remedies have been discovered from natural products in the past and the search is continuing for additional examples. Cytotoxic natural compounds may have a major role in cancer therapy either in potentiating the effect of chemotherapy or reducing its harmful effects. Recently, a few studies have reported advantages of using crude camel milk in treating some forms of cancer. However, no adequate data are available on the lyophilised camel's milk responsibility for triggering apoptosis and oxidative stress associated with human breast cancer. The present study aimed to address the role of the lyophilised camel's milk in inducing proliferation repression of BT-474 and HEp-2 cells compared with the non-cancer HCC1937 BL cell line. Lyophilized camel's milk fundamentally repressed BT-474 cells growth and proliferation through the initiation of either the intrinsic and extrinsic apoptotic pathways as indicated by both caspase-3 mRNA and its action level, and induction of death receptors in BT-474 but not the HEp-2 cell line. In addition, lyophilised camel's milk enhanced the expression of oxidative stress markers, heme-oxygenase-1 and reactive oxygen species production in BT-474 cells. Increase in caspase-3 mRNA levels by the lyophilised camel's milk was completely prevented by the actinomycin D, a transcriptional inhibitor. This suggests that lyophilized camel's milk increased newly synthesized RNA. Interestingly,it significantly (p<0.003) repressed the growth of HEp-2 cells and BT-474 cells after treatment for 72 hours while 24 hours treatment repressed BT-474 cells alone. This finding suggests that the lyophilised camel's milk might instigate apoptosis through initiation of an alternative apoptotic pathway.

        Pituitary apoplexy in an adolescent male with macroprolactinoma presenting as middle cerebral artery territory infarction

        Pituitary apoplexy typically presents with acute headache, vomiting, visual disturbance, and confusion. Herein, we report a rare presentation of ischemic stroke due to pituitary apoplexy. A 16.5-year-old male presented with reduced Glasgow Coma Scale (GCS) score, slurred speech, right-sided hemiparesis, and bitemporal hemianopia. Magnetic resonance imaging of the brain showed a large hemorrhagic sellar/suprasellar mass and an area of cortical T2/FLAIR hyperintensity with corresponding diffusion restriction in the middle cerebral artery territory. Computed tomography (CT) intracranial angiogram showed luminal occlusion of the clinoid and ophthalmic segments of both internal carotid arteries (ICAs, left>right) due to mass pressure effect. Biochemical investigations confirmed hyperprolactinemia and multiple pituitary hormone deficiencies. Stress-dose hydrocortisone was commenced with cabergoline, followed by urgent endoscopic transsphenoidal debulking of the tumor (subsequent histology showing prolactinoma). Postoperative CT angiogram showed improved caliber of ICAs. Intensive neurorehabilitation was implemented and resulted in complete recovery of motor and cognitive deficits. At the last assessment (18.8 years), the patient remained on complete anterior pituitary hormone replacement without cabergoline. Pituitary apoplexy is a medical emergency requiring prompt recognition and treatment and should be suspected in patients presenting with sudden, severe headache; nausea; or visual disturbance and meningism. Ischemic stroke is a rare manifestation of pituitary apoplexy in the pediatric population.

        Scope and challenges of nanoparticle-based mRNA delivery in cancer treatment

        Messenger RNA (mRNA) recently emerged asan appealing alternative to treat and prevent diseases rangingfrom cancer and Alzheimer’s disease to COVID-19 withsignifi cant clinical outputs. The in vitro-transcribed mRNAhas been engineered to mimic the structure of natural mRNAfor vaccination, cancer immunotherapy and protein replacementtherapy. In past decades, signifi cant progress has beennoticed in unveiling the molecular pathways of mRNA,controlling its translatability and stability, and its evolutionarydefense mechanism. However, numerous unsolvedstructural, biological, and technical diffi culties hamper thesuccessful implementation of systemic delivery of mRNAfor safer human consumption. Advances in designing and manufacturing mRNA and selecting innovative deliveryvehicles are mandatory to address the unresolved issuesand achieve the full potential of mRNA drugs. Despite thesubstantial eff orts made to improve the intracellular deliveryof mRNA drugs, challenges associated with diverse applicationsin diff erent routes still exist. This study examines thecurrent progress of mRNA therapeutics and advancementsin designing biomaterials and delivery strategies, the existingtranslational challenges of clinical tractability and theprospects of overcoming any challenges related to mRNA.
