        Evolution of Diastolic Dysfunction in Patients with Coronary Slow Flow Phenomenon and Acute Non-ST Segment Elevation Myocardial Infarction

        BACKGROUND: Diastolic function has been reported to be impaired in many patients with coronary slow flow phenomenon (CSFP). CSFP has broad spectrum of clinical presentations, including non-ST elevation myocardial infarction (NSTEMI). We sought to study the short-term evolution of diastolic function in CSFP patients presenting with NSTEMI. METHODS: This study included 92 patients with CSFP and acute NSTEMI. Conventional echocardiography Doppler imaging and tissue Doppler echocardiography imaging were used to evaluate diastolic function during index NSTEMI and after 3 months. RESULTS: Mean age of study patients was 45.7 ± 6.8 years. The prevalence of diastolic dysfunction (DD) at baseline was 69 patients (75%) and 28 patients (30.4%) at 3 months, p < 0.001. Various diastolic function indices showed significant improvement from baseline to 3 months follow-up. E/Em was 17.32 ± 3.41 at baseline compared to 12.41 ± 5.58 at 3 months, p = 0.039. Septal e′ velocity was 5.67 ± 4.56 cm/s at baseline compared to 7.78 ± 3.22 cm/s at 3 months, p = 0.023. Medications used were not significantly different between those with improved versus unimproved DD. CONCLUSIONS: Diastolic function seems to improve over short-term follow-up in patients with CSFP presenting with NSTEMI. This could reflect a transient worsening during acute NSTEMI.

      • Success Factors of Highway Construction Projects in Egypt: AHP Approach

        While the idea of the existence of a set of key factors for success in planning, designing and construction of projects is not new, it remains one of the most important issues in the field of construction management. This is due to project success factors are inputs to project management practice which can lead directly or indirectly to project success. This study identifies the success factors which can be used for improving the highway projects performance in Egypt during the project pre-construction and construction phases. Through a detailed literature review 35 success factors are identified. Furthermore, the AHP is adopted to prioritize the studied factors through the application of questionnaire survey. The study establishes that implementing an effective quality control and assurance systems, allow sufficient time for feasibility studies, design, drawing and tender preparation, effective cost control system and frequency of project budget updates, preparing adequate designs and drawings and determining sufficient time to implement the desired scope of work are the most important success factors for enhancing the highway projects performance in Egypt.

        Does oral ciprofloxacin affect the structure of thoracic aorta in adult and senile male albino rats? A clue to fluoroquinolones-induced risk of aortic dissection

        In this study, the effect of oral ciprofloxacin on the structure of the thoracic aorta in rats was investigated. Twenty four male albino rats were divided into 4 groups (6 rats/group): group I (adult control), group II (adult rats treated with ciprof loxacin), group III (senile control), and group IV (senile rats treated with ciprof loxacin). Rats in groups II and IV received ciprofloxacin via oral gavage in a daily dose of 3.5 mg/kg/d for 14 days, while control rats received equivalent amount of distilled water used to dissolve the drug. After 2 weeks, all rats were sacrificed, thoracic aortae were dissected, and half of the specimens were processed for paraffin sections and examined by light microscopy. The other half of the specimens were prepared for scanning electron microscopy. Sections from rats treated with ciprofloxacin showed evident damaging effect on aortic wall particularly in (group IV). Aortic intima showed, focal desquamation of the lining epithelium. Tunica media exhibited loss of the normal concentric arrangement and degeneration of the smooth muscle cells. Immune staining for alpha smooth muscle actin showed muscle damage. Interestingly, some sections in (group IV) showed out-pouch (aneurysm like) of the aortic wall. There was dense collagen fibers deposition. Scanning electron microscopic observations of (group IV) revealed uneven intima, adherent blood cells and fibrin filaments to damaged intima, and out-pouch formation. It was concluded that oral ciprofloxacin caused deleterious structural changes in the thoracic aortic wall of rats explaining clinical observations of fluoroquinolones induced risk of aortic dissection and aneurysm.

        Investigation of jack-up leg extension for deep water operations

        Since the first jack-up was built, jackups have become the most popular type of mobile offshore drilling unit (MODU) for offshore exploration and development purposes in shallow water. The most pivotal component of the jack-up unit is the leg, which can directly affect the global performance of the unit. In this paper, an investigation into extending the length of the jack up leg is carried out in order to study the enhancement of the rig capability to drill in deeper water approaching the range of the Semisubmersible Drilling Unit (SSDU) (300-1000ft). A study of the performance of a deep-water jack-up unit is performed with different leg lengths. Typical leg scantling dimensions and identical external loads are assigned, and then a detailed Finite Element Analysis (FEA) model is created in order to simulate the jack-up leg unit’s structural behavior. A Multi-point Constraint (MPC) element together with the spring element is used to deal with the boundary conditions. Finally, a comparative analysis for five leg lengths is carried out to illustrate their performance, including the ultimate static strength, and weight.

      • Appropriate Timing of Surgery after Neoadjuvant Chemo-Radiation Therapy for Locally Advanced Rectal Cancer

        Background: Surgery is the corner stone for the management of rectal cancer. The purpose of this study was to demonstrate the optimal time of surgical resection after the completion of neoadjuvant chemo-radiotherapy (CRT) in treatment of locally advanced rectal cancer. Materials and Methods: This study compared 2 groups of patients with locally advanced rectal cancer, treated with neoadjuvant CRT followed by surgical resection either 6-8 weeks or 9-14 weeks after the completion of chemo-radiotherapy. The impact of delaying surgery was tested in comparison to early surgical resection after completion of chemo-radiotherapy. Results: The total significant response rate that could result in functional preservation was estimated to be 3.85% in group I and 15.4% in group II. Some 9.62% of our patients had residual malignant cells at one cm surgical margin. All those patients with positive margins at one cm were in group I (19.23%). There was less operative time in group II, but the difference between both groups was statistically insignificant (P=0.845). The difference between both groups regarding operative blood loss and intra operative blood transfusion was significantly less in group II (P=0.044). There was no statistically significant difference between both groups regarding the intra operative complications (P=0.609). The current study showed significantly less post-operative hospital stay period, and less post-operative wound infection in group II (P=0.012 and 0.017). The current study showed more tumor regression and necrosis in group II with a highly significant main effect of time F=61.7 (P<0.001). Pathological TN stage indicated better pathological tumor response in group II (P=0.04). The current study showed recurrence free survival for all cases at 18 months of 84.2%. In group I, survival rate at the same duration was 73.8%, however none of group II cases had local recurrence (censored) (P=0.031). Disease free survival (DFS) during the same duration (18 months) was 69.4 % for patients in group I and 82.3% for group II (P=0.429). Conclusions: Surgical resection delay up to 9-14 weeks after chemo-radiation was associated with better outcome and better recurrence free survival.

      • Resistin and Insulin Resistance: A Link Between Inflammation and Hepatocarcinogenesis

        Background: Hepatocellular carcinoma (HCC) is the sixth most common cancer and the third leading cause of cancer related death overall. The role of insulin resistance in the development of HCC associated with chronic HCV infection has not been established. Resistin is a polypeptide hormone belonging to the adipokine family which could contribute to tumorigenesis and angiogenesis. Our aim was to study serum resistin and insulin resistance as risk factors for HCC in HCV cirrhotic patients. Materials and Methods: This prospective case controlled study included 100 patients with HCV related liver cirrhosis and HCC, 100 patients with HCV related liver cirrhosis without HCC and 50 apparently healthy participants as controls. For all subjects, liver profile, serologic markers for viral hepatitis, lipid profile, alpha-fetoprotein level (AFP), homeostasis model assessment (HOMA) were examined along with resistin. Results: HCC patients had higher mean values of HOMA-IR and resistin than cirrhotic patients and the control subjects (p<0.01). HOMA and resistin were considered independent risk factors in development of HCC, those patients with resistin > 12 ng/ml and HOMA > 4 being 1.6 times more likely to have HCC. Conclusions: HOMA and serum resistin allow for early identification of patients with cirrhosiswho are at substantially increased risk of HCC. Recommendation: HOMA and serum resistin could represent novel markers to identify HCV cirrhotic patients at greater risk of development of HCC.

        Applying the Ferrocement Concept in Construction of Concrete Beams Incorporating Reinforced Mortar Permanent Forms

        This paper presents the results of an investigation aimed at developing reinforced concrete beams consisting of precast permanent U-shaped reinforced mortar forms filled with different types of core materials to be used as a viable alternative to the conventional reinforced concrete beam. To accomplish this objective, an experimental program was conducted and theoretical model was adopted. The experimental program comprised casting and testing of thirty beams of total dimensions 300 9 150 9 2,000 mm consisting of permanent precast U-shaped reinforced mortar forms of thickness 25 mm filled with the core material. Three additional typical reinforced concrete beams of the same total dimensions were also cast to serve as contro specimens. Two types of single-layer and double-layers steel meshes were used to reinforce the permanent U-shaped forms; namely welded wire mesh and X8 expanded steel mesh. Three types of core materials were investigated: conventional concrete, autoclaved aerated lightweight concrete brick, and recycled concrete. Two types of shear connections between the precast permanent reinforced mortar form and the core material were investigated namely; adhesive bonding layer between the two surfaces, and mechanical shear connectors. The test specimens were tested as simple beams under three-point loadings on a span of 1,800 mm. The behavior of the beams incorporating the permanent forms was compared to that of the control beams. The experimental results showed that better crack resistance, high serviceability and ultimate loads, and good energy absorption could be achieved by using the proposed beams which verifies the validity of using the proposed system. The theoretical results compared well with the experimental ones.
