        Association Between Length of Stay in the Intensive Care Unit and Sarcopenia Among Hemiplegic Stroke Patients

        Objective To discuss the association between the length of stay at the intensive care unit (ICU) and sarcopenia among hemiplegic stroke patients. Methods This study evaluated 66 hemiplegic stroke patients with history of ICU admission using handgrip strength and bioelectrical impedance analysis to obtain height-adjusted appendicular skeletal muscle mass. The diagnosis of sarcopenia was made according to the muscle mass based on the Asian Working Group for Sarcopenia. The patients were divided into sarcopenic and non-sarcopenic groups. The two groups were statistically analyzed, and the significant factors with differences were studied. A multivariate logistic regression analysis was performed to examine the association between length of stay in the ICU and sarcopenia, after adjusting for potential confounders. Results Among 66 hemiplegic patients with an ICU admission history, 12 patients were diagnosed with sarcopenia. Sarcopenia patients showed lower scores on the Korean version of the Modified Barthel Index and the Korean version of the Mini-Mental State Examination. Additionally, patients with sarcopenia had a longer length of stay in the ICU, and univariate and multivariate analyses confirmed that the ICU length of stay was significantly related to sarcopenia (adjusted odds ratio=1.187; 95% confidence interval, 1.019–1.382; p=0.028). Conclusion The length of stay in the ICU was significantly associated with sarcopenia in hemiplegic stroke patients.

      • Trend Analysis of Nursing Research Published in the Journal of the Korea Convergence Society

        Aim(s): This study aimed to analyzed the pater on nursing in the Journal of the Korea Convergence Society(2010-2020), to identify the research trends and to propose the future direction development of nursing research. Method(s): Of the total of 2,597 papers, 421 papers in the field of nursing were analyzed for research type and methodological characteristics. For frequency analysis of analysis items, SPSS 25.0 (IBM Corp., Armonk, NY, USA) and Netminer 4.3 software were used for visualization of key words. Our researchers analyzed the number of authors, author affiliation, and research purpose to analyze the characteristics of the study type. And for the analysis of key words, visualization analysis was performed using a word cloud. Result(s): As a result of analysis of the research type characteristics of published papers in 421 nursing studies, the number of authors was the highest with 184 studies (43.7%) with two authors. As a result of the analysis of author affiliation, in the case of the 1st author, nursing professors were the most distributed with 311 articles (73.9%) of the total, followed by nursing graduate students with 43 articles (10.2%), nurses with 40 articles (9.5%), and nursing. There were 3 lectures (0.7%) of each lecturer and researcher in the nursing field. As a result of analyzing the purpose of the study, 288 studies (68.4%) correspond to fact-finding research, showing the highest frequency, followed by program development and evaluation with 52 studies (12.4%). As a result of performing word cloud analysis based on the frequency of appearance of the presented keywords, the key word with the highest frequency was health. As a result of analyzing the characteristics of the research methods of 421 published papers, 69.4% of the research designs were research studies, and 63.2% of the studies provided the basis for sampling the number of papers. G-power program accounted for 92.9% of the program used for sampling. The majority of study participants were adults (42.8%), and questionnaires (69.1%) were used the most for data collection. There were 299 studies (71.0%) of all studies that suggested the reliability and validity of the measurement tool. Conclusion(s): Based on this result, the Journal of the Journal of the Korea Convergence Society should cooperated that nursing research can be integrated with other fields, continued to challenge creative research methodologies, developed research themes, and reinforced of screening standards for quality improvement. For this reason, it is suggested a trend analysis research of convergence research with various field researchers and a multi-faceted analysis study on research trends related to nursing.

        간호학생을 위한 궤양성 대장염 환자간호 시나리오 개발 및 효과분석

        본 연구는 간호학생을 대상으로 궤양성 대장염 환자의 간호를 위한 시뮬레이션 시나리오를 개발하고 그 효과를 확인하고자 시행되었다. 연구대상은 G시 소재의 일개 대학 3학년 간호학생 90명으로 자료수집은 2018년 11월 15일부터 12월 10일까지 진행되었으며 SPSS 20.0 프로그램을 이용하여 빈도, 백분율, 표준편차, χ²-test, t-test로 분석하였다. 연구결과 궤양성 대장염 환자간호시 필요한 설사, 영양, 건강증진의 간호과정을 경험할 수 있는 표준화된 시나리오가 개발되었으며, 개발된 시나리오는 궤양성 대장염 환자 간호에 있어 사전조사보다 사후조사에서 간호학생의 자신감, 이론적 지식, 임상수행술기에서 통계적으로 유의하게 상승하였다. 결론적으로 본 연구는 통합적인 교수학습방법의 일환으로 국내최초 궤양성 대장염 환자 간호 시뮬레이션 시나리오라는 점, 시나리오와 간호과정이 접목되었다는 점, 그리고 표준화된 시나리오라는 점에서 기존 연구와 큰 차별성이 있다. 따라서 간호교육에 있어서 이론과 실습을 통합하는 방안을 고민하고, 후속연구에서는 다양한 결과변수 측정, 표준화된 측정도구개발 및 다양한 연구설계를 활용한 추가연구가 필요하다. This study aims to provide a helpful reference for nursing students by developing nursing care scenarios for patients with ulcerative colitis, and to evaluate the developed scenarios’ effectiveness. The study subjects were 90 juniors from the nursing department at a university in G City. Data were collected from November 15 to December 10, 2018, and analyzed using SPSS 20.0 with frequency, percentage, standard deviation, χ²-test, and t-test. The study developed standardized nursing care scenarios for ulcerative colitis, including diarrhea, nutrition, and health improvement. Posttest results of the simulation showed a statistically significant rise in the confidence, theoretical knowledge, and clinical performance skill of nursing students compared to pretest results. In conclusion, this study addresses several gaps in the existing literature as it serves as an integrated teaching-learning method, combines scenarios with the nursing process, and functions as a reference for standardized scenarios, providing the nation’s first nursing care simulation scenarios for patients with ulcerative colitis. Future research can focus on designing ways to integrate theory and practice in nursing education, and measure various outcome variables; developing standardized measuring tools; and utilizing diverse study designs as follow-up.

      • Effects of Microtubule Stabilization by Epothilone B Depend on the Type and Age of Neurons

        <P>Several studies have demonstrated the therapeutic potential of applying microtubule- (MT-) stabilizing agents (MSAs) that cross the blood-brain barrier to promote axon regeneration and prevent axonal dystrophy in rodent models of spinal cord injury and neurodegenerative diseases. Paradoxically, administration of MSAs, which have been widely prescribed to treat malignancies, is well known to cause debilitating peripheral neuropathy and axon degeneration. Despite the growing interest of applying MSAs to treat the injured or degenerating central nervous system (CNS), consequences of MSA exposure to neurons in the central and peripheral nervous system (PNS) have not been thoroughly investigated. Here, we have examined and compared the effects of a brain-penetrant MSA, epothilone B, on cortical and sensory neurons in culture and show that epothilone B exhibits both beneficial and detrimental effects, depending on not only the concentration of drug but also the type and age of a neuron, as seen in clinical settings. Therefore, to exploit MSAs to their full benefit and minimize unwanted side effects, it is important to understand the properties of neuronal MTs and strategies should be devised to deliver minimal effective concentration directly to the site where needed.</P>

      • Text Network Analysis of Research Topics on Virtual Simulation in Nursing

        Aim(s): The purpose of this study is to examine the relationship between keywords on virtual simulation in nursing studies and to develop schematic diagrams of the relationship between these keywords from a macro perspective. It is to identify the trend of the literature on virtual simulation in nursing. Method(s): This was a text-mining study that collected, processes, analyzed text data. For this study, we searched several databases such as PubMed, CINAHL, EMBASE, Web of Science, SCOPUS from 1st~15th, July, 2021. The Inclusion criteria included the following: (a) peer-reviewed, (b) journal article, (c) written in English, (d) including a text ‘virtual’ or ‘virtual reality’, ‘simulation’, ‘education’, ‘learning’, ‘training’ in its each title, abstract and keywords. (e) with nursing students and nurses, (f) to 31, May, 2021. Exclusion criteria included following: (a) not journal articles (dissertations, conference proceedings, Book section et al. b) language besides English, c) not involved virtual simulation, d) participants that did not include nurses and nursing students. Thus, 570 studies were selected in the library databases. Among these data, duplicate (222), no abstract (8), unrelated studies (72) and not articles (63) were excluded. As a result, a total of 213 papers were extracted. The keywords from 213 articles were preprocessed and keyword analysis and topic modeling were performed using NetMiner 4.3 software. For topic modeling, Latent Dirichlet Allocation was applied. Result(s): As for virtual simulation in nursing, keywords with the most frequent appearance and the highest networking degree in centrality were ‘nurse’, ’skill’, ‘care’, ‘knowledge’, ‘performance’, ‘communication’, ‘disaster’, ‘experience’, ‘health’. A total of 2347 keywords were categorized into 10 topics: ‘dementia care competency’, ‘pain assessment’, ‘airway placement management’, ‘operating procedure’, ‘presence & satisfaction’, ‘communication & attitude improvement’, ‘platform world’, ‘disaster response’, ‘game usability’, ‘system for confidence’ Conclusion(s): The identified trends in this study will help nursing educators and researchers to grasp the trends and insight for virtual simulation in nursing and to guide future research directions on virtual simulation in nursing.

        Determining the Most Appropriate Assistive Walking Device Using the Inertial Measurement Unit-Based Gait Analysis System in Disabled Patients

        Objective To evaluate the gait pattern of patients with gait disturbances without consideration of defilades due to assistive devices. This study focuses on gait analysis using the inertial measurement unit (IMU) system, which can also be used to determine the most appropriate assistive device for patients with gait disturbances. Methods Records of 18 disabled patients who visited the Department of Rehabilitation from May 2018 to June 2018 were selected. Patients’ gait patterns were analyzed using the IMU system with different assistive devices to determine the most appropriate device depending on the patient’s condition. Evaluation was performed using two or more devices, and the appropriate device was selected by comparing the 14 parameters of gait evaluation. The device showing measurements nearer or the nearest to the normative value was selected for rehabilitation. Results The result of the gait evaluation in all 18 patients was analyzed using the IMU system. According to the records, the patients were evaluated using various assistive devices without consideration of defilades. Moreover, this gait analysis was effective in determining the most appropriate device for each patient. Increased gait cycle time and swing phase and decreased stance phase were observed in devices requiring significant assistance. Conclusion The IMU-based gait analysis system is beneficial in evaluating gait in clinical fields. Specifically, it is useful in evaluating patients with gait disturbances who require assistive devices. Furthermore, it allows the establishment of an evidence-based decision for the most appropriate assistive walking devices for patients with gait disturbances.
