      • Recent developments in liquid-phase synthesis and applications of nanomagnesia

        Recent developments in the synthesis of nanomagnesia of controlled sizes and shapes that are suitable for various applications are reviewed. Two main methods, based on liquid-phase synthesis, i.e., chemical methods and bio-based methods, are used to synthesize nanomagnesia. Conventionally, nanomagnesia was synthesized by chemical methods such as coprecipitation, sol-gel, combustion method, and so on using different chemical agents and stabilizers which later on become responsible for several biological risks because of the toxicity of used chemicals. Bio-based protocols are growing as another environmental friend method for the synthesis of various nanostructures especially nanomagnesia using biomass, plant extracts, alga, and fungi as a source of precursor material. The ideal method should offer better control of textural properties of nanostructures and decrease the necessity for purification of the synthesized nanoproducts, which sequentially removes the use of large amounts of chemicals and organic solvents and manipulation of products that are unsafe to the environment. Finally, the broad applicability of nanomagnesia in diverse areas is presented. Employment of nanomagnesia reported in several laboratory and industrial fields are valued from the standpoint of the significance of these issues for technological requests, as described in the literature. Nanomagnesia has various applications such as antimicrobial performance, removing pollutants, batteries application, and catalysis.

        Site effect microzonation of Babol, Iran

        Extensive researches on distribution of earthquake induced damages in different regions have shown that geological and geotechnical conditions of the local soils significantly influence behavior of alluvial areas under seismic loading. In this article, the site of Babol city which is formed up of saturated fine alluvial soils is considered as a case study. In order to reduce the uncertainties associated with earthquake resistant design of structures in this area (Babol city), the required design parameters have been evaluated with consideration of site's dynamic effects. The utilized methodology combines experimental ground ambient noise analysis, expressed in terms of horizontal to vertical (H/V) spectral ratio, with numerical one-dimensional response analysis of soil columns using DEEPSOIL software. The H/V spectral analysis was performed at 60 points, experimentally, for the region in order to estimate both the fundamental period and its corresponding amplification for the ground vibration. The investigation resulted in amplification ratios that were greater than one in all areas. A good agreement between the proposed ranges of natural periods and alluvial amplification ratios obtained through the analytical model and the experimental microtremor studies verifies the analytical model to provide a good engineering reflection of the subterraneous alluviums.

        Frequency of Vertebral Endplate Modic Changes in Patients with Unstable Lumbar Spine and Its Effect on Surgical Outcome

        Study Design: Prospective cohort study. Purpose: In this study, we investigated the frequency of vertebral endplate Modic changes (MCs) and their effects on surgical outcomes in patients with unstable lumbar spines. Overview of Literature: Signal changes in endplates have been classified into three types by Modic. The prognostic role of MCs has been investigated in various spinal disorders. Methods: A series of 70 patients with clinical and radiographic unstable lumbar spine were included in the study. Endplate signal intensity was determined according to Modic classification. All patients underwent instrumented posterolateral fusion. Functional evaluation was made using the visual analog scale (VAS) and Oswestry disability index (ODI). Results: Eighteen patients (26%) had normal endplate intensity, 31 patients (44%) had MC type I, 20 patients (28%) had MC type II, and one patient (1.4%) had MC type III. Pain level VAS and ODI decreased significantly from the preoperative evaluation to the sixmonth and one-year postoperative evaluations. The surgical outcome (VAS and ODI) was not significantly different between the various types of MC. Conclusions: Posterolateral fusion is an effective treatment in patients with unstable lumbar spines. MC do not have a significant effect on the surgical outcome of these patients.

        Pial Arteriovenous Fistula with Giant Varices: Report of Two Cases with Good Surgical Outcome

        Pial arteriovenous fistulas (pAVF) are rare vascular lesions consisting ofone or more arterial connections to a single venous channel without anyintervening nidus of vessels or capillaries. Case 1 : A 65-year-old woman with a complaint of headache and lefthand paresthesia was referred to us. Magnetic resonance imagingshowed a large saccular lesion with signal void in the posterior part ofthe right sylvian fissure and catheter angiography showed a giant venousaneurysm fed by one branch of the middle cerebral artery (MCA) anddraining into the vein of Trolard. Case 2 : A 12-year-old boy was transferred to our hospital with a historyof sudden loss of consciousness and hemiplegia. Brain computed tomographyrevealed a massive hemorrhagic mass in the right hemisphere andcerebral angiography showed a pAVF with a large aneurysmal varix,which was fed by multiple branches of the right MCA and draining intothe superior sagittal sinus. Both patients underwent craniotomy and after ligation of vascular connections,aneurysmal varices were removed completely. Surgical resectioncan be a safe method for treatment of pAVFs, particularly in those withlarge varices.

        The Association of Dietary Total Antioxidant Capacity with Inflammatory Biomarkers and Anthropometric Indices in Patients Who Candidate for Coronary Artery Bypass Graft Surgery: a Cross-sectional Study

        Antioxidant compounds can attenuate inflammation and delay degenerative processes especially in the cardiovascular system. This study aimed to determine the relationship between dietary total antioxidant capacity (DTAC) and serum biomarkers in patients undergoing coronary artery bypass graft surgery. In this cross-sectional study, 146 patients who had referred to Imam Ali Hospital in Kermanshah were recruited and demographic, anthropometric, physical activity and dietary data were collected. Fasting blood glucose (FBG), serum levels of lipid profile, inflammatory markers (interleukin [IL]-17, intercellular and vascular cell adhesion molecules [ICAM, VCAM]), and total antioxidant capacity (TAC) were also measured. A regression model adjusted for confounding variables presented that the coefficients of ICAM and VCAM (ng/dL) in the third tertile of DTAC were lower than those in the first tertile (β = −417.2, 95% confidence interval [CI] = −509.9 to −324.5, p < 0.001; β = −293.2, 95% CI = −334.3, −252.1, p < 0.001, respectively). The β of serum TAC (ng/dL) in the third tertile was 0.86 (95% CI = 0.77, 0.95, p < 0.001) higher than that in the first tertile. DTAC had no statistically significant relationship with lipid profile, FBG, and IL-17 levels, and anthropometric indices. In this study DTAC was associated with increased serum TAC and decreased cell adhesion molecules. Therefore, diet antioxidants may be beneficial in attenuating of inflammation in coronary artery diseases.

        Effects of Chromium Picolinate Supplementation on Cardiometabolic Biomarkers in Patients with Type 2 Diabetes Mellitus: a Randomized Clinical Trial

        Type 2 diabetes mellitus (T2DM) is a serious public health problem accompanies with several complications. This study was conducted to evaluate the effects of chromium picolinate (CrPic) supplementation on the glycemic status and lipid profile in patients with T2DM. The patients with T2DM (n = 52) were randomly allocated into 2 groups. One group received 400 μg CrPic per day and the other group took placebo; the intervention duration was 8 weeks. Anthropometric indices and metabolic factors were measured at the beginning, and at end of the study. The patients were recommended not to change their normal diet, life style and medication. No significant changes were observed for weight, body mass index, and fasting blood glucose (FBG) in both groups; while intra-groups changes in homeostatic model assessment for insulin resistance (HOMA-IR) value was significant (p < 0.05). Results of analysis of covariance showed that there were significance differences between groups in total cholesterol, low density lipoprotein cholesterol and HOMA-IR at the end of the intervention adjusting for baseline levels (p = 0.035, 0.030 and < 0.001, respectively). In this study, oral supplementation with 400 μg CrPic for eight weeks did not alter FBG concentration as well as anthropometric parameters in individuals with T2DM. However, the modest beneficial effects of chromium supplementation on insulin resistance as indicated by HOMA-IR and lipid profile were found.

        Dietary Diversity Score and Its Related Factors among Employees of Kermanshah University of Medical Sciences

        Dietary diversity score (DDS) is known as an indicator of food quality. Dietary diversity can promote health status. The aim of this study was determined DDS and its related factors in Kermanshah University of Medical Sciences (KUMS) employees. This cross-sectional study was conducted on 190 employees of KUMS in 2015. According to the population of KUMS centers which were selected randomly (Paramedical, Public Health faculties, Imam Reza Hospital and province health center), subjects were selected by convenience sampling method. Food frequency questionnaire (FFQ) was used to calculate DDS. Foods were divided into 5 main groups: grains, vegetables, fruits, meat, and dairy products. The main groups had 23 subgroups. Total DDS divided to 4 quartiles: less than 3.0, 3.0-5.5, 5.6-8.5, and more than 8.5. Anthropometric parameters including: weight, height, waist circumference (WC), and hip circumference were measured. Data were analyzed by Kolmogorov-Smirnov test, χ<sup>2</sup> test, and analysis of variance (ANOVA) test with SPSS 20 software (IBM Corp., Chicago, IL, USA). The mean ± standard deviation of DDS and body mass index (BMI) were 5.68 ± 1.73 and 25.1 ± 3.42 kg/㎡, respectively. The average of the waist-to-hip ratio (WHR) in men and women was 0.92 ± 0.04 and 0.86 ± 0.06, respectively. There was statistically significant difference between DDS and self-reported economic status (p < 0.022). No significant difference was observed between DDS and BMI or WC. However, significant negative correlation was observed between DDS and WHR in men (p < 0.019). This study showed that DDS had a negative correlation with the WHR. Therefore, dietary diversity may improve health status by effect on fat distribution in body.

        Anthropometric Indices from Primary to High School in the West of Iran: Epidemiologic Trends

        Examining the trend of anthropometric indices in children and adolescents in each region can be highly beneficial in providing effective strategies to improve the status of their growth. This study was conducted with the aim of determining the trend of anthropometric indices in students from primary to high school in the west of Iran. Data were collected using a demographic questionnaire and health certificates for anthropometric information of primary school students, and current anthropometric measurements with scale for high school students. Then, all of the data were analyzed in AnthroPlus and SPSS software. Of the 731 students, 350 were female and 381 were male. Mean height Z-score to age showed a significant reduction from primary to high school (from +0.0386 to -0.27416), and mean body mass index Z-score to age showed a significant increase (from -0.3916 to +0.1826). Prevalence of stunting and overweight/obesity in high school was 1.4 and 2.5 times higher than primary school, respectively, but the prevalence of wasting reduced by 2.3 fold. Girls were more affected by the increased trend of obesity and stunting compared to boys. Nutritional transition was evident in students. Healthcare policy-makers should design and implement a comprehensive health strategy to deal with this situation, especially in girls.

        Probiotic Yogurt Fortified with Vitamin D Can Improve Glycemic Status in Non-Alcoholic Fatty Liver Disease Patients: a Randomized Clinical Trial

        Non-alcoholic fatty liver disease (NAFLD) is a significant public health problem globally and the most notable chronic liver disease in Asian countries. Various dietary supplements have been assessed as potential methods to alleviate the metabolic damages related to NAFLD, but the results of these works have been equivocal. This study aimed to evaluate the effects of probiotic yogurt fortified with vitamin D (Pro-YFD) on glycemic and anthropometric indices in patients with NAFLD. One hundred and four NAFLD patients of both sexes were randomly allocated to 2 groups: group A (Pro-YFD) and group B (unfortified yogurt). The intervention period was 3 months. Fasting blood samples were obtained for measuring fasting blood sugar (FBS) and insulin level. Food intake was measured using a validated food frequency questionnaire. Body composition was estimated by bio-impedance. Eighty-eight patients completed the study. The mean serum level of 25(OH)D<sub>3</sub> was elevated significantly (p < 0.001), while insulin level decreased significantly (p < 0.003) in group A at the end of the study. FBS levels showed no significant differences between the groups at the end of the trial. Also, there were no significant changes in diet caloric intake, physical activity, or anthropometric indices in the 2 groups during the interventions. Pro-YFD in the diets of patients with NAFLD may attenuate insulin resistance and improve serum level of 25(OH)D<sub>3</sub>.

        The Association Between Dietary Acidity and Clinical Symptoms in Patients With Rheumatoid Arthritis

        This study aimed to investigate the relationship between dietary acidity load and clinical symptoms in the patients with rheumatoid arthritis (RA). This case-control study examined 55 patients with RA and 215 healthy individuals in a Ravansar non-communicable diseases (RaNCDs) cohort study, Iran. Participants’ food intakes were assessed using a validated food frequency questionnaire. The dietary acidity was calculated using potential renal acid load (PRAL), net endogenous acid production (NEAP), and dietary acid load (DAL) scores. The patients with RA were identified based on the self-reporting, medications history, and the approval of the cohort center physician following patients’ examination. The odds ratio (OR) of joint stiffness in fully adjusted model was greater in the upper median of dietary acidity than in the lower median (PRAL: odds ratio [OR], 1.18; 95% confidence interval [CI], 0.59-2.36), but there was no statistically significant difference. The OR of joint pain in the upper median of dietary acidity was less than in the lower median in fully adjusted model (PRAL: OR, 0.70; 95% CI, 0.46-1.29), but the difference was not statistically significant. After adjusting potential confounders, people in the upper median of dietary acidity had a higher OR of developing RA than those in the lower median (PRAL: OR, 1.39; 95% CI, 0.70-2.76); however, it was not statistically significant. There was not any statistically significant relationship among dietary acidity and the odds of joint pain, joint stiffness, and developing RA.
