N/A We reviewed 6 cases of bile peritonitis without biliary perforation among 25 cases of bile peritonitis during the period from Jan. 1971, to Sept. 1982, and following results are obtained. 1) The most frequent cause of bile peritonitis was perforated cholecystitis and bile peritonitis without biliary perforation was found in 6 cases.2) The possible mechanism of bile peritonitis without biliary perforation was microperforation or filtration of bile through permeable gallbladder in cholecystitis. 3)Cholecystectomy, T-tube choledochostomy, or choledochoduodenostomy were performed in six patients. Five patients were cured and one patient was died The patients without visible perforation had longer duration of illness, more leukocytes, and less complication and mortality than those with visible perforation. In conclusion, we have to keep in mind that bile peritonitis can occur without gross perforation of biliary tree, although it is not so common. The clinical course of bile peritonitis without biliary perforation seems to be somewhat milder than bile peritonitis with biliary perforation. The same surgical procedures can be used for both types of bile peritonitis.
