원위부 요골 골절의 합병증 중 하나인 장무지 신전건 파열은 지연성으로 발생한다고 알려져 있다. 요골 골절과 동시에 발생하는 급성 장무지 신전건 파열은 매우 드문 경우로, 저자들은 문헌 고찰과 함께 증례 보고 하고자 한다. Late rupture of the extensor pollicis longus (EPL) tendon is an occasional complication following a distal radius fracture. Early rupture of the EPL tendon as a complication of distal radius fracture is rare. We report one case of early rupture of the EPL tendon after distal radius fracture.
유도 선수에서 발생한 수지 근위 지관절의 재발성 전방 회전 탈구 - 1례 보고 -

수지 근위 지관절의 전방 회전 탈구는 매우 드물게 발생한다. 저자들은 여자 유도 선수에서 발생한 수지 근위 지관절의 전방회전 탈구를 경험하였기에 이를 문헌고찰과 함께 보고하고자 한다. 24세 여자 유도 선수로 총 4번의 근위 지관절 전방 탈구가 발생하였다. 수술 시 회전 손상으로 파열된 척측 측부 인대와 파열 없이 늘어져 있는 중앙 건을 확인할 수 있었다. 수술적 치료로 파열된 척측 측부 인대 봉합술과 중앙 건과 측부대 사이를 봉합하여 근위 지관절의 배측부를 강화 시켜주었다. 술 후 운동시에 탈구는 다시 발생하지 않았다. 수지 근위 지관절의 전방 회전 탈구는 매우 드문 손상이지만 다른 탈구처럼 조기 진단과 치료, 그리고 단계적인 재활이 무엇보다 중요하겠다. Volar rotatory dislocation of the proximal interphalangeal joint(PIP) of the finger is rare. We report a female judo player who had volar rotatory dislocation of the PIP joint of the middle finger. She had dislocation of PIP joint total 4 times. At operation, the central tendon was identified as being distension, with the ulnar collateral ligament ruptured. The ruptured ulnar collateral ligament was interposed within the joint. The ruptured ulnar collateral ligament was repaired and extensor expansion was repaired. At last follow-up, she didn't have recurrent dislocation during judo. Accurate diagnosis, early intervention and progressive rehabilitation of this injury are very important as other dislocations.
TA 약침의 안전성 평가 : 세균을 이용한 복귀돌연변이 시험

Objectives : To evaluate the safety of TA pharmacopuncture, bacterial reverse mutation test was performed using histidine requiring Salmonella typhimurium (TA98, TA100, TA1535, and TA1537) and tryptophan requiring Escherichia coli (WP2uvrA (pKM101)) strains in the absence and presence of metabolic activation. Methods : TA pharmacopuncture was prepared by extracting from a herbal mixture (Scutellariae Radix 20 g, Phellodendri Cortex 20 g, Pulsatillae Radix 20 g, Sophorae Tonkinensis Radix et Rhizoma 20 g, Aucklandiae Radix 10 g, Carthami Fructus 30 g) using a mixture solvent of purified water for injection (500 mL) and alcohol (500 mL). In order to determine the dose levels of the main study, the dose range finding study was performed. The high dose of TA pharmacopuncture was selected at 100% and it was sequentially diluted by applying a geometric ratio of 2 to produce lower dose levels of 50.0, 25.0, 12.5, and 6.25%. The treatment volume was 100 μL. As a result, the growth inhibition and precipitation of the test substance were not evident at any dose level in each strain in the absence and presence of metabolic activation. Therefore, the dose levels of the main study were selected as 100, 50.0, 25.0, 12.5, and 6.25%. Also, the negative and positive control groups were set. Results : The mean number of revertant colonies was less than twice when compared to the negative control values at all dose levels of the TA pharmacopuncture in the presence and absence of metabolic activation, showing no dose-related increase. In the positive control group, the number of revertant colonies was markedly increased by more than twice when compared to the negative control group. Conclusions : TA pharmacopuncture did not exhibit any indications of mutagenic potential under the conditions of this study.
동측 역행성 유경 피판술을 이용한 무지의 재건 - 1예 보고 -

Reconstruction of the distal portion of the great toe has always represented a difficult problem to surgeons. There are many methods of flap reconstruction described for foot defects, but none for tip of the great toe. We report a case of reconstruction of the great toe using a homodigital reverse pedicle island flap method. A 25-year-old man had a crushing injury at the distal phalanx of the left great toe. A homodigital reverse pedicle island flap was raised from the dorso-medial aspect of the proximal phalanx of the great toe based on the distal dorsal arcade. The flap covered the exposed distal phalanx and stump was closed without shortening. He made an uncomplicated recovery and when seen 6 months later he had a good cosmetic and functional result. A homodigital reverse pedicle island flap is described for the reconstruction of the distal part of the great toe. It involves only a single stage procedure with minimal donor site morbidity and provides a good cosmetic results.

Objectives : A pharmacokinetic study of HO-pharmacopuncture solution was performed to rats. Blood, urine and stool samples were collected and analyzed in gas chromatogram (GC). Methods : Rats were injected intraperitoneally with HO (Carthami fructus)-pharmacopuncture solution (0.5 ml) and blood samples were collected from tail artery and urine and stool samples were acquired in individual metabolic cages. Results : GC chromatogram analysis of HO-pharmacopuncture solution showed a total of 12 peaks. In serum samples, the largest number of peak was identified on day 1 and six peaks were observed at the same retention time as standard HO until 10 days. In stool and urine samples, peaks at 45 min were observed at the same retention time as standard HO until 10 days. Conclusions : These results may be interpreted as most of components of HO was not accumulated in the serum but excreted as urine and stool.

Objectives : This study was designed to evaluated the potential of TA pharmacopuncture on chromosomal aberrations in Chinese Hamster Lung (CHL/IU) cells. Methods : In order to determine the high dose level of the main study, a dose range finding study was conducted. The high dose was selected at 10.0% of undiluted TA pharmacopuncture and it was sequentially diluted to produce lower dose levels of 5.00, 2.50, 1.25, 0.625 and 0.313% by applying a geometric ratio of 2. The treatment volume was 0.3 mL. As a result, the cytotoxicity and precipitation of the test substance were not evident at any dose level in the short time treatments without and with metabolic activation and the continuous treatment without metabolic activation. Therefore, the dose levels of the main study were selected as 10.0, 5.0, and 2.5%. The treatment volume was 1.3 mL. Also, the negative and positive control groups were set. Results : In main study, the frequency of cells with chromosome aberrations was less than 5% in the short time treatments without and with metabolic activation and the continuous treatment without metabolic activation. Also, there was not statistically significantly different when compared to the negative control group. In the positive control group, the frequency of cells with structural chromosomal aberrations was more than 10% and statistically significantly increased compared to the negative control group. Conclusions : In conclusion, TA pharmacopuncture did not show any indication of potential to induce chromosome aberrations under the conditions of this study.
