태아의 혈장 Cortisol 및 Cortisone이 분만에 미치는 영향

인간에서 glucocorticosteroids가 분만유발에 기여하는 역할을 규명코져 분만시에 모체 및 신생아의 혈장 cortisol과 cortisone치를 측정하여 그의 변동상황과 상호 관련여부를 살폈다. 1. 모체혈장 cortisol치는 분만에 따른 자궁수축이 전체 혹은 선택적제왕절개분만시에 가장 낮아 417.8 ± 17.2 ng/ml인데 반하여 자궁수축이 가장 강한 자연경질분만시에 600.1 ± 18.6 ng/ml로서 현저한 차이(p$lt;0.5)가 있었으며 자궁수축이 강할수록 높았다. 2. 모체혈장 cortisol치는 자궁수축의 강도에 따른 변동을 보이지 않았다. 3. 태아의 혈장 cortisol치는 모체의 그것보다 크게 낮았으며, 자궁수축의 강도에 따른 현저한 차이(p$lt;0.05)가 있어서 자연경질분만때 148.2 ± 15.9 ng/ml였고 선택적 제왕절개분만때 73.7 ± 9.1 ng/ml이었다. 4. 태아혈장 cortisol치는 자궁수축의 강도에 따라 증가하였으며 모체에 비하여 현저하게 높았다. 따라서, cortisol치는 모체의 것이 태아의 것보다 높았고 아울러 자궁수축의 강도에 따라 양자가 증가하였던 것에 반하여 cortisone치는 모체에서는 변동하지 않았지만, 태아의 cortisol치는 모체의 경우보다 현저하게 높았을 뿐 아니라 자궁수축정도에 따른 증가상을 보여 분만에 모체에서는 cortisol만이, 태아에서는 cortisol과 cortisone이 같이 관여하는 것으로 추정되었다. In order to assess the effect of cortisol and cortisone on the induction of labor and delivery, the plasma concentration of the above two hormones were measured by radiommunoassay at spontaneous and induced vaginal delivery, and elective and emergency cesarean section. The maternal plasma cortisol levels at elective cesarean section, which did not accompany the uterine contraction were 417.8 ± 27.2 ng/ml, and at spontaneous vaginal delivery, which accompanied the most intensive uterine contraction, were 600.1 ± 24.3 ng/ml. However, the cortisone levels in maternal plasma did not change by mode of delivery. The fetal plasma cortisol and cortisone levels demonstrated the significant increase, according to the degree of uterine Contraction. I conclude that the maternal and fetal cortisol, and fetal cortisone may play some role in the induction of labor and delivery.
