저출산고령화시대에 있어서의 사회보장제도의 현황과 과제 ~ 일본사회보장제도의 변천이 시사하는 교훈 ~

본 연구는 소자고령화가 진행되고 있는 일본의 사회보장제도의 현황과 과제를 검토하고 한국사회에 시사하는 교훈을 정리해서 사회보장제도와 정책에 대한 제안과 방향제시를 목적으로 하고 있다. 한국과 일본은 앞으로도 계속해서 인구가 감소하고 단신세대의 비율이 증가하는 등 공통점이 많다. 특히, 켄죠(2009)가 제시한 복지국가3유형중에서 한국과 일본은 “가족의존형”으로 분류되며 일본사회가 경험해온 시행착오는 가까운 장래에 한국사회와 경험할 가능성이 크다. 본 논문은 일본사회보장제도의 변천과정을 개관하고 최근의 경제상황과 국채발행 및 채무구조, 사회보장급부의 현황을 고찰함과 동시에 일본의 연금제도와 건강보험 및 개호보험의 현황과 과제등에 대한 검토를 통해서 한국사회보장제도에 대한 정책제안과 방향성을 제시하고 있다. In this paper, we will give an overview of the changes in Japan's social security system and understand the current state of Japan's economic growth, debt, and social security benefits. Then, after considering the introduction process, system, and issues of the Japanese pension, medical, and long-term care insurance system, the further declining birthrate and aging population and future issues will be described. also gave five specific lessons that the transition of Japan's social security system. Japan’s system can also be studied in order to improve the direction of South Korea's social security system. Comparing the future forecasts of Japan and South Korea, there are many things in common between Japan and South Korea, such as a declining population and an increase in the proportion of single-person households. The three types of family-dependent (Japanese type) of the welfare state shown by Kenjo (2009) are considered to be classified not only in Japan but also in South Korea. The changes in socio-economic conditions that Japanese society has experienced after the war are expected to occur in South Korea in the near future.

한국노인장기요양보험제도의 과제와 방향성 제시 : 일본노인사회보장제도의 변천과정에 대한 심층분석을 통해서

한국의 노인장기요양보험제도는 2008년에 도입되어 일본에 비해 고령화가 낮은 빠른 시기에 도입되었는데, 이는 고령화가 빠르게 진전되고 의료보험 재정적자로 인해 관심을 가지게 된 것으로 알려져 있다. 양국 모두 사회보험방식에 따른 개호 보험제도를 도입하였는데, 한정된 자원 안에서 공평성을 가지고 어떻게 효율적이며 효과적으로 운용해 나갈지를 과제로 들 수 있다. 따라서 양국의 사회보장제도에 대해 고찰하고 자원확보에 대한 노력, 개호보험사업계획의 책정, 케어매니져 창설, 개호인재의 질향상, 4가지를 테마로 일본의 개호보험이 한국 노인장기요양보험에 주는 시사점을 살펴보았다. In Korea, the national mandatory long-term care insurance (LTCI) for old people was established in 2008. The introduction was reportedly due to rapid aging and fiscal deficit of medical insurance program in Korea. The ratio of Korea’s old adults population at the point was lower than that of Japan when it introduced its LTCI. Korea and Japan’s LTCI are both covered by social insurance; therefore, the challenge lies in how to manage the insurance system effectively and efficiently with limited resources and make sure everyone gets a fair share. We review the social security reviews the social security systems of the two countries and implications of Japan’s LTCI for Korea’s LTCI, under four topics: effort to secure resources, designing a long-term care insurance scheme, the birth of care managers, and nurturing skilled LTCI workforce.
