N/A Studies on sensitivity and Synergism of Antifungal Agents against Candida albicans Hae Chull Nah, M.D., Seung Chul Lee, M.D., Inn Ki Chun, M.D., Young Pio Kim, M.D. Department of Dermatology, Chonnam University Medical School Kwangju, Korea At the present, various antifungal agents are available, but in clinical settings they are mostly applied as single agents. There are very few reports in the Iiterature on the effectiveness of the combined regimen. The autors tested the sensivities of 17 strains of Candida albicans to 8 popular antifungal agents. Fourteen strains were isolated in our clininc from the cases of mucocutaneous candidiasis and confirmed by API 20 Candida Kit and germ tube test. Three standard strains, Candida albicans 44858, Candida albicans 44859, and Candida albicans 28576 were also used. We also studied the efficacy of ketoconazole with 7 other antifungal agents. The results of minimum inhibitory concentration(MIC) studies were as follwed : ketoconazole, 4-128 mcg/㎖ ; itraconazole, 8-64 mcg/㎖ ; econazole, 16-64 mcg/㎖ ; miconazole, 8-32 mcg/㎖ ; clotrimazole, 8-64 ; 5-fluocytosine, 16-128 mcg/㎖ ; amphotericin B, 0.5-8 mcg/㎖ ; nystatin, 2-8 mcg/㎖. Synergistic effects with ketoconazole were observed in 100% of 5-fluocytosine, followed by clotrimazole(85.7%), econazole(64.3%), amphotericin B(58.8%), miconazole(21.4%), nystatin(14.3%), and itraconazole(6.7%), in order. Additive effects with ketoconazole were observed in 60% of itraconazole, followed by nystatin(50%), econazole(21.4%), miconazole(14.3%), clotrimazole(7.1%), amphotericin B(5.9%), and 5-fluocytosine(0%), in order. Antagonistic effects with ketoconazole were observed in 28.6% of nystatin, followed by itraconazole(26.7%), miconazole(14.3%), amphotericin B(11.8%), and econazole(7.1%), in order 5-flucocytosine and clotrimazole did not show antagonistic effects with ketoconazole. These results suggest that combination of synergistic agents may be of value in combating with Candida albicans infection and also in reducing the incidence of adverse reactions. Also, the fact that various synergistic effects can be obtained by combination of antifungal agents with different or similar mode of action suggests to us the necessity of accurate and standardized in vitro senstivity tests in the in the treament of candidiasis.

일측성 모낭 각화증

Unilateral keratosis follicularis is considered a localized variant of Darier's disease and should be included in the differential diagnosis of diseases with zosteriform keratotic eruptions. It is characterized by the unilateral linear lesions in multiple locations, increased irritation by sweating, lack of solar aggravation, and negative family history. Histopathologic findings reveal acantholytic dyskeratosis similar to a generalized Darier's disease. We report a case of unilateral keratosis follicularis in a 68-year-old man, who presented with unilateral flesh-colored keratotic papules on the left axilla and inguinal area. He was effectively treated with oral 13-cis-retinoid acid (isotretinoin). (Kor J Dermatol 1996;34(1): 127-131)

피부병변이 선행된 급성 림프구성 백혈병

The leukemias are a heterogeneous group of neoplasms arising from the malignant transformation of hematopoietic cells. The cutaneous lesions associated with leukemia may be divided into the specific (leukemic infiltration) and nonspecific (leukemids). These may occur separately or in combination with each other. A 28 year old man admitted to the Department of Dermatology in December 1989, for diffuse infiltrated skin lesions of five months duration. In July 1989, he had experienced hyperpigmented diffuse infiltration on the face, which gradually spread to involve the neck and trunk. In November 1989, he felt cervical, submandibular, and inguinal lymph node enlargement. Skin biopsy specimen showed diffuse dermal and subcutaneous infiltration of atypical lymphocytes. Bone marrow biopsy and peripheral blood smear specimen showed changes of acute lymphoblastic leukemia(L). In addition, we studied infiltrated cell.; by immunoperoxidase staining with a pannel of monoclonal antibodies. The results revealed that the atypical cells expressed mainly helper/inducer T cell marker. (Kor J Dermatol 29(l):114 119, 1991)

임신중 악화된 유두유륜 과각화증

We report a case of hyperkeratosis of the nipple and areola occuing in 33 year old woman. She did not have a history of endocriruipathy, horrnonal therapy and other ceatoses, so this case seems to be the nevoid form by Levy-Franckel classification. Interestingly, however the lesion which had developed before marriage suddenly aggr avated during pregnancy. This may be a unique feature which has not been reported elsewhere. Histapathologic findings are compatible with breviously reported eases. There was no response to topical steroid and retinoic acid treatrnent. (Kor J Dermatol 1994; 32(6): 1111-1113)

We report a case of sebaceous carcinoma arising from nevus sebaceus of Jadassohn ocurring in a 68-year-old male, who showed a pedunculated cauliflower like tumor mass on verrucoid linear plaque in the fronto-parietal scalp. Histopathologic findings revealed mult,iple irregular tumor lobules composed mainly of foamy cytoplasmic atypical cells suggesting undifferrntiated sebaceous cells. Good result was obtained with wide local excision and primary closure without recurrence for 9 months after surgiral excision. (Kor J Dermato 29(1):104 108, 1991)

최근 10년간 악성흑색종의 임상 및 병리조직학적 연구

A total of 39 malignant melanoma cases diagnosed at Chonnam National University Hospital for past 10 years from 1980 to April, 1990 were included for the clinicopathologic study. Among the 29 primary cutaneous melanomas, 18 cases(62.1 % ) occurred on the hands and the feet, especially on the heel. The average age of the patients was 54.3 years at the time of diagnosis and the male to female ratio was 1 to 1,4. Acral lentiginous melanoma was the most common (h2.1% ) histopathologic type and the invasion level c)f 22 cases(78.4% ) was over 1.50mm by Rreslows thickness. (Kor J Dermatol 29(2):193 202)

A Remark on Non-Linear Phonology

종래의 음운론에서는 음운단위들이-음운이든 변별 자질이든-單線的으로 연결되어 있다고 가정해왔다. The Sound Pattern of English(1968)에서도 이러한 單線的 음운조직은 가장 기본적인 가정이었으며, 기실 60년대의 생성음운론은 이 가정에 집착한데서 일어난 여러 문제들을 해결하는데 전력을 기울였다고도 할 수 있다. 본 논문에서는 단선음운론에서는 해결할 수 없는 문제들이 복선음운론에서는 자연스럽게 해결이 됨을 예시하여 복선음운론의 특성과 장점을 自立超分節音韻論, 音節의 기능, 그리고 韻律音韻論의 세 분야로 나누어 살펴보았다. The existing phonology considered phonological units-phonemes or distinctive features-to be concatenated in a linear fashion. In The Sound Pattern of English(1968) we have also found the linear phonological system as one of the most basic assumptions. In fact, it may be safely said that phonologists have done their best to solve various problems which arose from their sticking to the assumption. In this paper, I have showed that many problems which were not solved neatly by linear phonology can be naturally handled in the framework of non-linear phonology. And I have presented characteristics and merits of non-linear phonology, exemplifying each component of it which is divided into three parts: autosegmental phonology, function of syllables and metrical phonology.

A Study on Tensification

영어의 유성파열음은 한국어에서 된소리가 되는 현상은 주지의 사실이다. 이는 어느 두 언어가 같은 소리를 가지고 있으나 음운체계는 서로 다를 수 있다는 한 증거이기도 하다. 파열음을 구분짓는 자질이 무엇인가에 대한 주장은 여러갈래이나 크게 voicing feature와 tensing feature로 대별해 볼 수 있다. 본 논문에서는 이 두 자질을 개관하고 성대의 열림의 정도가 영어의 유성파열음이 한국에서 된소리로 인식되게 하는 요인임을 밝혀보았다. It is a well-known fact that the voiced plosives in English are realized as tense plosives in Korean. It shows that two languages have the same sounds in them, but may have different phonological systems. Phoneticians and linguists have different views of plosives and the primary differentiating features. But the main stream can be divided into two: one is of voicing feature, the other is of tensing feature. In this paper, I have studied the two features and clarified that the tensification of English voiced plosives is due to the degrees of opening of the vocal folds.
