        Szary 증후군

        Szary syndrome is a leukemic variant in the spectrum of cutaneous T cell lymphoma. This uncommon hematologic malignancy presents features of erythroderma, severe pruritus, lymphadenopathies and circulating atypical T lymphocytes with immunohistochemical phenotype of helper T cell. Some other diseases which can eventually progress to erythroderma may cause difficulties in diagnosing the disease. The 59-year-old female patient presented with generalized erythematous follicular papules, patches, plaques with severe scaling. Under the impression of erythroderma secondary to pityriasis rubra pilaris, our initial treatment with oral acitretin improved the skin symptoms for a while, but it became intractable. On the routine peripheral blood examination, moderate leukocytosis composed predominantly of lymphocytes was found. Close examinaton of the lymphocyte on light microscope revealed large atypical lympocytes with hyperconvoluted nucleus. The peripheral blood lymphocytes were CD2+, CD3+, CD5+ cells but there were a few CD4+ or CD8+ cells. Lesional tissue showed the similar feature as blood. This finding was very unusual but further diagnostic approaches were not available. The patient received 2 cycles of chemotherapy (CHOP regimen), but she died of opportunistic infections 3 months after starting the chemotherapy. (Korean J Dermatol 2000;38(5):650~654)

        World Wide Web을 통한 일반인의 탈모에 대한 질의 분석:효율적인 원격진료의 구현을 위한 조건

        N/A Background:The growing popularity of World Wide Web (WWW) contributes to sharing personal information. In the field of medicine, the concerns of telemedicine via WWW are growing as an issue. But in Korea, telemedicine is an unfamiliar field as ever. Objective:We analyzed the queries of the visitors of our home page, alopecia clinic (www. talmo.co.kr). Based on the results of the analysis, we tried to evaluate the possibility of telemedicine in a specific field of dermatology and to establish an efficient basis and way of information sharing. Methods:We collected 295 queries of the web visitors. The queries were listed on a bulletin board system or sent to us via electronic mail. They were collected from June 22, 1998 to April 8, 1999. Results:Most of the queries were from male visitors (M:F=198:44). The visitor's age ranged from 17 to 40. Their connection time and connection weekday was distributed evenly. Most of the queries were concerned at alopecia (loss of hair, 283 queries, 95.9%). They requested the treatment (150), the diagnosis (75), the cause (22) and the associated diseases (19). 33 (44%) out of the 75 queries on the diagnosis could not be diagnosed correctly. Conclusion:The proportion of inspection in the diagnosis of dermatologic diseases is greater than the other medical departments. Thus, the possibility of establishing teledermatology is relatively easier. We analyzed 295 web-based queries and present several problems with our store-and-forward system. Establishing an efficient teledermatology system requires availability of qualified digital images and prepared protocols. Also problems of liability, reimbursement and coordinated national policy should be solved. Despite these problems, in the next generation of information highway, telemedicine shall be a mandatory field. (Korean J Dermatol 2000;38(5):629~634)

        비소각화증의 임상적 고찰

        Background:Arsenics have been used for various medical purposes. These arsenics can cause different kinds of cutaneous and internal malignancies. The arsenic-induced malignancies can be fatal or cause severe morbidity. Objective:For early detection and prevention of the arsenic-induced malignancies, the authors analyzed the clinical characteristics of arsenic-induced skin diseases. Methods:Medical records of 15 patients(10 males, 5 females) with arsenical keratosis(AK) during a period from September 1994 to October 1999 were reviewed. Interview, physical examinations, skin biopsy, and laboratory tests were performed. Results:14 patients used Korean proprietary pills(KPP, Hwan-Yak), which were suspected to contain certain amount of arsenics, as a drug for treatment of certain diseases such as vitiligo(4 patients, 26.7%), laceration, syphilis, etc. The other formulation of medication was Salvarsan injection. Age at KPP or Salvarasan medication was from seven to thirty-five years old. Duration of medication was from one month to four years. Bowen's disease(BD), squamous cell carcinoma(SCC) and basal cell carcinoma(BCC) were detected in ten(66.7%), three(20%) and one(6.7%) patients, respectively. One(6.7%) patient had lung cancer. Sites of predilection were palmoplantar area(100%) for AK, trunk(60%) for BD and lower extremities(45%) for SCC and BCC. Conclusion:Chronic exposure to arsenic can induce the cutaneous and systemic malignancies. To prevent these malignancies, strict managements and supervising programs on certain arsenic-containing drugs(probably KPPs) are required, and regular cutaneous and systemic examinations for the patients with AK should be performed.         소형 무전기를 위한 On-Board 이중대역 안테나 개발

        본 논문에서는 소형 무전기에 적용이 가능하며, RFID/USN 920 MHz 대역(917~923.5 MHz)과 WiFi 2.4GHz 대역(2.4~2.483 5GHz)대역에서 반사손실이 10 dB 이상이며, 전방향 방사특성을 갖는 On-Board 이중대역 안테나를 제안하였다. 안테나 설계의 기본 구조는 Meander 모노폴 안테나이며, 제안조건을 만족하는 안테나 설계를 위해 이중 스터브와 탭를 사용하였다. 이중 스터브와 탭은 스터브에서 공통모드와 차동모드를 형성하여 안테나의 리액턴스를 영향을 주어, 안테나 특성인 대역폭과 반사손실, 이득, 방사특성을 개선하게 된다. 무전기 시스템 크기는 52×77 ㎟이고, 안테나 부분은 52×15 ㎟ 크기로 제한하였으며, 기판은 두께가 0.8 mm이고, 기판은 상대유전율은 4.4인 FR4 기판을 사용하였다. 실험 결과, 이중대역에서 반사손실 10 dB를 만족하였으며, 최대 대역폭과 최대이득은 RFID/USN 대역에서 82 MHz(8.9 %)와 3.17 dB, WiFi 대역에서 360 MHz(15.1 %)와 1.95 dB이며, 방사특성은 전방향 특성의 결과를 얻었다. 본 논문의 결과를 적용함으로써 무전기의 가격경쟁력과 생산성 효율을 향상시킬 수 있다. In this paper, it can be applied to a walkie-talkie, the RFID / USN 920 MHz band(917~923.5 MHz) and WiFi 2.4GHz band(2.4~2.483 5GHz) return loss is 10 dB over the band, on-board dual band with omni-directional radiation characteristics is proposed. The basic structure designed antenna is used meander monopole antenna. It was used as double stubs and tabs for antenna designs that meet the criteria proposed. The double stub and the tab affects the reactance of the antenna to form a common-mode and differential-mode in the stub to improve the antenna characteristics and return loss in the bandwidth, gain and radiation characteristics. The system size of walkie-talkie is 52×77 ㎟, the size of the antenna is limited to 52×15 ㎟, the thickness of FR4 dielectric substrate is 0.8 mm, FR4 dielectric constant 4.4 is used. For experimental results, the return loss is measured more than 10 dB, the maximum gain is measured 1.95 dB, the maximum bandwidth is measured 360 MHz, the radiation characteristic is measured omni-directional. By a walkietalkie terminal design applying the results of the paper, the handset"s price competitiveness and production efficiency can be improved.

        AHP분석에 의한 부동산중개 서비스의 중요도 분석

        1. CONTENTS (1) RESEARCH OBJECTIVES The object of this study identifies the weights of customer satisfaction determinants in real estate brokerage service. And the other purpose of the study is to test the differences of weight between customer group and real estate expert group. These objectives performed using by the RE-SERVQUAL model. (2) RESEARCH METHOD The survey research is employed for the customers and real estate experts. This research is employed the AHP(Analytic Hierarchy Process) for the weight of customer satisfaction determinants in real estate brokerage service. And then the t-test assesses whether the weight means of two groups are statistically different from each other. (3) RESEARCH FINDINGS The weights of real estate brokerage serVlce determinants have been identified. There are the differences of weight between custorrer group and expert group. The order of expert group's weight is following by the interaction(OA18), the outcome quality (OA12) , and the physical environrrent(O.170). The order of customer group's weight is following by the outcome quality(O.619), the interaction(O.294), and the physical environment(O.087). The outcome quality and physical environrrent have statistically differences between two groups. However, the interaction factor didn't. 2. RESULTS We found the weights of RE-SERVQUAL model using by the AHP. These weights will suggest to reduce the difference of view point between customers and experts.

      • MPC5553 DSP 기반의 전기버스 전장시스템용 3㎾급 절연형 DC/DC 컨버터의 설계

        본 논문에서 설계한 3kW급 절연형 DC/DC 컨버터는 전기 버스의 주 동력원인 고전압 배터리로부터 차량의 전장시스템에 저전압 전원을 공급한다. 이 전력변환에는 고전압 배터리측과 저전압 배터리측의 절연이 반드시 필요하기 때문에 고주파 변압기를 사용한 절연형 DC/DC 컨버터가 일반적으로 사용된다. 따라서 ZVS 동작이 가능한 위상천이 방식의 풀-브리지 컨버터를 선정하였다. 본 논문에서 사용된 전력변환장치는 차량시스템에 적용하기 적합한 프리스케일사의 32-Bit DSP인 MPC5553 기반의 제어기를 이용한 실험을 통해 타당성을 검증하였다.

        In automobile, the introduction of electronically commutated motors has been accompanied by a proliferation of electronic devices. With this proliferation of electronic devices, an emphasis has been placed on EMC issues. This paper is proposed to use SRM as a radiator cooling fan in automotive applications. To drive SRM, Energy efficient C-dump converter is applied. It is verified more efficient than other converters through simulation and experiments. And also SRM is valid automotive applications that have strict EMC standards. It is compared SRM with BLDC and DC motor by experiments.
