Nicotinic acid in a dose of 3 grams daily was given to 18 hyperlipemic patients having essential hypertension or obesity for a period of 12 weeks in order to evaluate its effects on serum lipids. Following were the results. 1. The administration of nicotinic acid had to be interrupted within 1 week in 7 out of 18 patients due to the side effects like cutaneous flushing, itching, nausea, vomiting and diarrhea. 2. The side effects of nicotinic acid appeared to be more severe in Koreans than in the cases reported in the foreign literatures. 3. Of all the serum lipids components, β-lipoprotein cholesterol decreased the most after the administration of nicotinic acid, and therefore, the ratio of β/? lipoprotein cholesterol. Serum phospholipids tended to decrease but to the extent of statistical insignificance. 4. Nicotinic acid, despite of its frequent side effects, appears to be an useful anti-cholesterol agent as it selectively decreases β-lipoprotein cholesterol which has the closest relationships with atherosclerosis.

The concentrations of serum lipids values were studied in 137 patients suffering from essential hypertension by means of paper electrophoresis and chemical analysis. The lipids values were discussed according to age and sex, and the values were independently grouped to determine the relation between clinical signs and the observed values. The following results were obtained. 1. The serum lipids values in essential hypertension did not show a statistically significant variation with age and sex. Accordingly, it was suggested that some factors other than age and sex might be related to serum lipids values. 2. It was observed that the serum α-lipoprotein cholesterol values were normal in mild to moderate hypertension for the corresponding age groups of normal controls but showed statistically low values in the group of 251 ㎜Hg, or more of the systolic blood pressure, and were more closely related to the systolic blood pressure than in the diastolic blood pressure. 3. It was observed that the serum β-lipoprotein cholesterol values showed statistically significant high values in the hypertensives than in the corresponding age groups of normal controls, and were closely related to diastolic blood pressure than in systolic blood pressure. 4. It was observed that the serum phospholipids values showed statistically significant high values in the hypertensives than in the corresponding age groups of normal controls and were more closely related to diastolic blood pressure than in the systolic blood pressure. 5. A lesser tendency of the serum lipids values to vary with the severity of the eye ground changes was noted in the hypertensives for whom the mean values were generally higher than in the normal controls. 6. A greater tendency of the serum lipids values to vary with the severity of the albuminuria excreted was noted in the hypertensives for whom the mean values were generally higher than in the normal controls. 7. From the results obtained, it was believed that the high serum lipid values in essential hypertension were not related to their age and sex, but were closely related to their clinical signs such as elevation of blood pressure, especially, of diastolic blood pressure and increased excretion of albuminuria.
