A bronchogenic cyst is an uncommon benign congenital anomaly of the primitive ventral forgut which was generally encountered within the mediastinum and detected in pediatric patients. It is rarely detected in adult population as a lateral neck mass. We have recently experienced one case of bronchogenic cyst as a left lateral neck mass in a 49-year-old male. He complaints of a $2{\times}1$ cm sized, soft, non tender, and movable mass on the low lateral neck. The surgical excision of mass was performed and the final histopathologic diagnosis was a bronchogenic cyst. Here, we report this case with the review of literatures.

Vocal fold paralysis is a dangerous complication of thyroid surgery that is often fatal. Therefore, intraoperative neuromonitoring (IONM) has recently been widely adopted and used in thyroid surgery. IONM is thought to be more helpful in identifying the recurrent laryngeal nerve (RLN) and predicting its function in a limited surgical space than conventional thyroid surgery, but there are few reports on this. Therefore, we would like to discuss IONM during robotic or endoscopic thyroidectomy.

Thyroid surgery can cause many complications, but recurrent laryngeal nerve palsy is the most important. Therefore, many efforts have been tried to avoid recurrent laryngeal nerve palsy. Among them, intraoperative neuromonitoring (IONM) is one of the useful tools to find nerves and avoid nerve palsy during surgery. There are various methods, but the method of directly monitoring the movement of the vocal cords using an electromyography tube was introduced first. After that, a method of increasing the signal accuracy by using a pressure sensor and an accelerometer sensor was also introduced. In addition, a needle is directly pierced into the thyroid cartilage or an electrical signal is measured by attaching a sensor to the skin in front of the thyroid cartilage. Various methods of IONM are being tried and studied, and efforts to help find nerves through IONM are continuing. It is expected that overall surgical complications will be reduced by preserving nerves, avoiding changes in voice and swallowing disorders, and reducing the need for tracheostomy due to bilateral vocal cord palsy.

Vocal fold paralysis can occur for various reasons, and vocal fold paralysis treatment is important to improve the quality of life and secure airway according to voice. There are several methods of unilateral and bilateral vocal fold paralysis; voice therapy, injection laryngoplasty, laryngeal framework surgery, reinnervation procedures, tracheostomy, arytenoidectomy, cordotomy, cordectomy, and laterofixation of vocal cord. This review paper aims to discuss the treatment principles of unilateral and bilateral vocal fold paralysis.
술 중 대량 출혈을 동반한 거대 갑상선유두상암종 절제술 1례

A 62-year-old female patient had goiter for twenty years. She visited out-patient clinic with a hoarse voice and intermittent breathing difficulties. About protruding 15cm sized mass located the anterior neck and right vocal cord paralysis was observed. Preoperative CT scan was strongly suspected of thyroid gland cancer and cervical lymph node metastasis. Therefore, fine needle aspiration test was performed and surgical treatment was planned with the histopathologic results (papillary thyroid carcinoma). Surgery was performed with total thyroidectomy, bilateral cervical lymph node dissection, and right selective nodal lymph node dissection (level II-V). During operation right thyroid seemed to be adherent to surrounding tissue and the blood vessels were extremely engorged. There was hypotensive crisis because of intraoperative excessive bleeding. However it was managed by repetitive transfusion. The operation was completed without abnormalities. She underwent 4 times of bleeding control operation due to postoperative bleeding. After complications were improved, we are currently undergoing out-patient follow up without morbidity.
