        15세 이하 입원 진료비 본인부담 경감 정책이 평균재원일수 및 입원 진료비 변화에 미친 영향 분석

        Purpose: This study aims to examine the effect of the copayment reduction policy on the health care utilization of patients under age 15 after the policy started in 2017. (이하는 아래 methodoloty로 이동) Methodology: Data on the ALOS, the average admission cost, and the out-of-pocket expenditure for patients under 15 years of age from 2015 to 2019 were obtained from the National Health Insurance database. Policy effects were measured by analyzing three dependent variables before and after policy: the average length of stay (ALOS), the average admission cost, and the out-of-pocket expenditure for patients under 15 years of age. The collected data were analyzed using the SAS package, and the analysis methods used in this study were the mean difference test and linear regression analysis. Findings: The study results reveal that, after the copayment reduction policy in the year 2017, the ALOS and the out-of-pocket expenditure were significantly decreased, but the average admission cost was significantly increased. Practical Implications: These results imply that the policy of copayment reduction for the patients under the age of 15 has contributed to mitigating the patients' financial burden with little concern about growing medical utilization.

        치료 가능한 사망으로 측정한 우리나라 지역 간 건강수준의 격차

        Background: This study aims to figure out the gaps in health status by estimating amenable mortality rate by region, reflecting the characteristics of Korea, and estimating the years of life lost (YLL) per capita by disease. Methods: People who died from amenable diseases between 2008 and 2018 were extracted from the cause of death statistics provided by Statistics Korea. The age-standardized amenable mortality rates were estimated to compare the health status of 229 regions. YLL per capita was calculated to compute the burden of diseases caused by treatable deaths by region. The YLL per capita by region was calculated to identify the burden of disease caused by amenable deaths. Results: First, while the annual amenable mortality rate in Korea is on a steady decline, but there is still a considerable gap between urban and rural areas when comparing the mortality rates of 229 areas. Second, YLL per capita due to the amenable deaths is approximately 14 person-years during the analysis period (2008-2018). Conclusion: Although the health status of Koreans has continuously improved, there is still a gap in health status region by region in terms of amenable mortality rates. Amenable death accounts for a loss of life equivalent to 14 person-years per year. Since the amenable mortality rate is an indicator that can measure the performance of the health care system, efforts at each local area are required to lower it.

      • 호주의 치료재료 관리체계 개혁 동향

        의료기술이 발달하면서 치료재료의 급여 항목도 함께 증가하고 있다. 하지만 한국을 비롯한 세계 각국에서 치료재료에 대한 관리체계가 미흡하여 이로 인한 의료비 지출이 증가하고 있는 상황이다. 호주는 한국과 같이 전 국민 의료보장제도를 운영하고 있으며, 지속적인 보험료 상승으로 최근 치료재료의 급여목록 개편을 통해 재정의 안정성을 확보하고자 하였다. 이 글에서는 호주 정부의 치료재료 관리체계의 특징과 함께 최근 개혁 동향을 살펴봄으로써 한국의 치료재료 관리체계에 대한 시사점을 도출하고자 한다. As medical technology advances, the positive list of prostheses expands. In many countries, including Korea, a lack of management has led to increased health expenditures on prosthesis items. In recent years, Australia, with its national health insurance system like Korea’s, has increased health insurance premiums and amended the list of reimbursable prosthesis items in a bid to ensure financial stability. In this article, we aim to examine the characteristics of prosthesis management in Australia and recent amendments made to its prosthesis list and explore implications for prosthesis management in Korea.
