Degeneration of the intervertebral disc of the lower lumbar spine is somewhat more often than other spines due to anatomical characteristics. Several authors reported that there is a difference in the incidence of disc degeneration in the L4∼5 and L5∼S1 disc spaces dependent on position of intercrestal line. In 1982, Kirkaldy- Willis recognized the “level at Risk”, accordance with site of the L5 veretbra and intercrestal line, that is, when the L5 vertebra is sited low in the pelvis, the L4∼5 joint is risk in recurrent rotational strain and when L5 is high, the L5∼S1 joint is at risk to the strain. The authors analized 60 cases of lumbar myelogran showing positive findings, and studied the clinical relevance of the location of intercrestal line in relation to the site of disc degeneration in lower lumbar spine. Obtained following results are; 1. On analysis of clinical symptoms, two cases out of 60 cases were L3~4, 46 cases were L4~5 and 12 cases were L5~S1, and that of myelogram, two cases were L3~4,48 cases were L4~5, 10 cases were L5~S1 2. Among the 60 cases, 14 cases were high sited L5 vertebrae and 46 cases low sited. 3. Low sited L5 vertebrae were 37 cases(80.4%) out of 42 cases which corresponded clinical symptoms and lumbar myelogram.

The management of the femoral shaft fracture in children is quite different from that of in adult because of the differences of the anatomy, biomechanics, and physiology of a child's skeleton. Generally, femoral shaft fractures in children heal easily and satisfactorily with conservative treatment. Among 71 cases of femoral shaft fractures in children who were treated at Soonchunhyang university hospital during period from Jan. 1980 to Dec. 1985, only with 50 cases follow up study was made and analyzed both clinically and radiologically. The following results were obtained; 1. The ratio of male and female was about 1.3:1 and the highest incidence was between the age of 5 and 8 years(50%). 2. The main cause of fracture was traffic accident(82%) and the next most frequent one was fall down. 3. Middle 1/3 of the femoral shaft was most commonly injuried(64%) and transverse type was common. 4. 43 cases were treated with conservative and 7 cases by open reduction with internal fixation. 5. In general, angulation at fracture site was under 15 degree at final examination. 6. Among 50 cases, overriding of the fractured fragments was occurred in 28 cases and the average overriding was 1.2cm in length.
