N/A In order to observe the response of counterregulatory hormones to hypoglycemia in diabetics, plasma glucose, Glucagon, epinephrine and norepinephrine concentrations were measured in an insulin-induced hypoglycemic state. the Blood sample were taken before and 15,30,60; 90 and 120 minutes after intravenous injection of 0.1-0.2 U/kg of purified regular insulin in 16 healthy volunteers, 2 IDDM and 18 NIDDM patients after 12 of hours overnight fasting. The results were as follows: 1. In the normal control, plasma glucose concentration after insulin injection decreased abruptly to reach nadir in 30 minutes (42.6±14.4 mg/dl) while glucagon secretion increased two fold above the baseline in 33 minutes (93.3±32.1 to 204.4±85.5pg/ml, p>0.001). Epinephrine secretion in response to hypoglycemia was abrupt and increased tenfold above the baseline in 30 minutes (from 80.0±22.8 to 876.6±182.7 pg/ml), 2. In IDDM and NIDDM patients, the plasma glucose concentration after insulin injection decreased slowly to reach nadir in 60 mintues while the glucagon response in diabetic groups were depressed, especially in the IDDM group. Also, epinephrine secretion in response to hypoglycemia was severely depressed in the IDDM group but slightly preserved in the NJDDM group. The results of this study indicate that counter- regulatory hormones (glucagon and epinephrine) in diabetics show delayed and depressed responses to hypoglycemia. Norepinephrine does not play a major role in the metabolic response to insulin-induced hypoglycemia in both normal and diabetic people,
