Comparison of Th1 and Th17 inflammatory cytokines between plaque and guttate psoriasis

Background: The phenotypic heterogeneity of psoriasis is suspected to involve different cytokine profiles, yet not elucidated. Objectives: To investigate the tissue mRNA levels of Th1 and Th17 cytokines in patients with plaque psoriasis, guttate psoriasis, and healthy control. Methods: A total of 15 patients with psoriasis (10 plaque type and 5 guttate type) and 5 healthy controls were enrolled. Tissue samples were obtained and the cytokines were analyzed by real-time reverse transcriptase polymerase chain reaction and immunohistochemical staining. Results: The comparison between psoriatic and control tissue samples revealed that the tissue mRNA levels of IL-12, IFN-γ, IL-23, IL-17A, IL-22 and the protein levels of IL-12, IL-2, IFN-γ, IL-17A, IL-22 were significantly increased in psoriatic skin. The comparison between plaque and guttate psoriasis showed that the tissue mRNA levels of IL-23 and IL-17A and cytokine ratio of tissue mRNA levels of IL-23/IL-2 and IL-17A/IL-2 were significantly increased in plaque psoriasis compared to guttate psoriasis. Conclusion: We showed that the expression of Th1 and Th17 cytokines were increased in psoriatic skin compared to control. Among them, the tissue mRNA levels of IL-23 and IL-17A were significantly increased in plaque psoriasis compared to guttate psoriasis, indicating that the imbalance between Th1/Th17 cytokines might play a role in the phenotypic expression of psoriasis.
