알코올중독자의 삶에 대한 현상학적 연구 : 포토보이스를 활용하여 = Phenomenological Study of Alcoholics' Life-By Utilizing Photovoice

The purpose of this study is two-fold: (1) to understand the nature of alcoholics' life experiences (2) to explore the meaning of the experiences. The study was performed by examining three questions: firstly, what are life experiences of alcoholics' life? secondly, what types of phenomena they show from their experiences? thirdly, what implications do the experiences reveal? I utilized analytical phenomenology by Max Van Manen and made use of photovoice to help better state the stories of the research participants as well as paying attention to alcoholics' stories in the process of collecting data.
The participants of the study are among male alcoholics who were diagnosed as alcoholism and hospitalized, especially nine people who are hospitalized now or maintained abstinence in the midst of recovery after discharging from hospital. Sampling was accomplished for people who fit the standards and they were selected by basic sampling method. Intensive in-depth interview was carried out for data collection for participants from over thirty minutes to two hours and thirty minutes from July 1st to September 27th and interviews were done at least three times to six times. Photos and description of photos and group activities were used for data collection methods along with in-depth interviews through six sessions of photovoice workshop. I attempted to enhance understandings of self-reflection through data collection and appreciation in literature and paintings and to provide insight to the theme by using literature and photovoice photos in analysis.
As a result, I drew out 22 elucidated themes on life experiences of alcoholics and thus examined final 8 essential themes. So the essential themes that I discovered from life experiences of alcoholics are as follows. <To put under the wheels of suffering : passing down alcoholism, living under the circumstance of childhood pain and hard environment> < To fall into a swamp called alcohol, beginning to drink, falling in, character- more distorted, alcohol-led life, selfish and corrupted life>, <To be labeled as an alcoholic: contracted a disease called alcoholism : difficulties caused by alcohol>, <My reality, my life : hospitalized at a mental hospital, prolonged a life vs imprisoned, isolated from family and community - living as an outcast>, <To struggle to live : difficulty to maintain abstinence, living in cold, hard reality, occupation: two-edged sword>, <endless bondage of pain and suffering : relapse- everything falling apart, anxiety and fear due to frequent relapse, alcoholism and death>, <Acceptance- totally different life: me as an alcoholic - acceptance of me and my life, spiritual change, restoration through AA, feeling of things restored>, <To walk the way in honesty and humility : The key to a new life- alcoholism, my life that I would like to live from now on- soaring>
Practical implication of social welfare based on these analysis is as follows.
First, drinking problem of adolescence in life cycle should not be understood as a transitional phenomenon but should be continuously controlled and examined from long term perspective including beginning to end of adolescence. Second, going through hospital treatment as an alcoholic has a great meaning in their life and it is necessary to have systematic, integrated and standardized professional treatment on alcoholism. Third, it is essential to have strong social support, substantial hospital treatment for continuous care in hospital treatment for alcoholism. Fourth, more active involvement of family treatment is required. Fifth, the change of paradigm is essential in involvement of mental health professionals in alcoholism problem. Sixth, it is significant to understand the importance of spirituality of alcoholics' restoration in the area of mental health. Practical methods and programs are urgent for spiritual change and maturity. Finally, it is requested for mental health professionals to have conversion of understanding in terms of relapse of alcoholics in the process of abstinence.
Political implication of this study is as follows. First, it is necessary to have preventative projects nationwide to enhance right understanding of drinking culture and alcoholism, which leads to reduction of domestic and national expense due to alcoholism and of personal difficultly of alcoholics themselves. Second, national efforts are essential to establish standards for alcoholics' hospital treatment. Third, political support and standardized treatment are vital to constitute systematic network of community resources to maintain abstinence for restoration after hospitalization.
The significance of the study through the results are as follows. First, this study is to explore overall life experience of alcoholic patients. Second, I utilized photovoice in performing qualitative research on the life of alcoholics. Third, participants had a chance to reflect on their lives through the process of this study and above all, they experienced self introspection and spiritual maturity while preparing photovoice. Photovoice played an indispensable role in strengthening participants' ability and became very valuable practical method in defending policy in terms of social welfare practice. In addition, photovoice became a starting point of a new possibility of practical method.
The significance I discovered through research result and discussion was very important but now I would like to make a few suggestions for a better study.
First, this study focused on male alcoholic's life experience so there is a certain degree of limitation to reflect experience of every alcoholics. It would be meaningful to compare and contrast male alcoholics' experiences with their female counterparts through qualitative research on female alcoholics' life. Also this study was restricted in a region so it would be desirable to research more broadly towards a variety of alcoholics. Second, photovoice was utilized as a method of collecting data in this study. While participants took part in the study through photovoice for a short period, they went through self introspection and accomplished spiritual maturity. Therefore what changes they accomplished practically through photovoice and for example, various quantitative study will be done integratively on participants' spiritual change, change of motives, change of methods, etc. actual use of photovoice will be supported in social welfare practice. Third, the role of photovoice is that the participants' stories were conveyed to others honestly and effectively so it helps participants get empathy. It can be an effective tool for policy by means of photos. Therefore the photos that participants took on the life of alcoholics can be utilized in an organization and in public or to support policy. Furthermore they can be used for social welfare practice in a more active level. photo exhibition by using photovoice could be meaningful resource in connection with other resources in the future.

국문 초록 (Abstract)

본 연구의 목적은 알코올중독자의 삶에 대한 경험의 본질을 이해하고 그 경험이 의미하는 바가 무엇인지를 탐구하는 것이다. 이러한 연구목적을 달성하기 위해 첫째, 알코올중독자의 삶의 경험은 어떠한가?, 둘째, 알코올중독의 삶의 경험에서 드러나는 현상은 무엇인가?, 셋째, 알코올중독자의 삶의 경험에서 드러나는 현상의 의미는 무엇인가? 라는 연구 질문을 가지고 연구를 수행하였다. 연구를 수행하는데 있어서 Max Van Manen의 해석학적 현상학 연구방법을 통하여 규명하되 자료를 수집하는 과정에서 당사자들의 이야기에 귀 기울이고 연구참여자들이 자신의 이야기를 좀 더 잘 진술하도록 돕기 위해 포토보이스를 활용하였다.
본 연구의 연구참여자는 알코올중독으로 진단을 받고 입원치료를 경험한 남성알코올중독자로 입원 중에 있거나 퇴원 후 단주를 유지하며 회복 중에 있는 사람으로 9명이 본 연구에 참여하였다. 표집방법은 기준표본추출방법으로 위의 기준에 부합되는 사람을 선정하였다. 자료수집은 2013년 7월 1일부터 9월 27일까지 연구참여자 한명을 기준으로 30분 이상 2시간 30분 이하의 집중적인 심층면담을 최소 3차례에서 최대 6차례까지 진행하여 자료를 수집하였다. 또 6회기의 포토보이스워크숍에서의 사진과 사진에 대한 설명, 집단 활동 내용을 심층면담 자료와 더불어 주된 자료수집 방법으로 활용하였다. 그리고 문학과 회화에서의 자료수집과 감상을 통해 자기성찰에 관한 연구자의 이해를 높이고자 하였고, 분석에서도 문학작품과 포토보이스 사진을 활용함으로써 주제에 대한 통찰을 제공하고자 하였다.
본 연구의 결과 알코올중독자의 삶의 경험에 관한 22개의 규명된 주제들을 도출하였고 이를 통해 최종 8개의 본질적 주제가 정해져 이를 토대로 고찰하였다. 알코올중독자의 삶에 대한 경험에서 발견한 본질적 주제는 다음과 같다. : <고통의 수레바퀴에 놓여짐 : 알코올중독의 대물림, 어린 시절의 상처와 어렵고 힘든 환경에서 살아옴>, <술이라는 늪에 빠져 듦 : 마시기 시작함-빠져듦, 성격-더 왜곡되어 감, 술이앞서서 살아옴-이기적이고 타락한 삶>, <알코올중독자라는 꼬리표를 달게 됨 : 알코올중독이라는 이상한 질병에 걸림, 술로 인해 생긴 어려움>, <내가 처한 나의 현실 : 정신병원에의 입원-생명의 연장 vs 구속됨, 가족, 가족과 사회로부터 소외됨-이방인 되어 살아가기>, <어떻게든 살기 위해 버티기 : 단주를 유지해 가는 과정에서의 어려움, 냉혹한 현실을 살아감, 직업 : 양날의 칼>, <끝없이 돌아가는 고통의 굴레 : 재발-모든 것이 무너져 내림, 잣은 재발로 인한 불안감과 두려움, 알코올중독과 죽음>, <받아들임-완전히 다른 삶의 길을 걸어감 : 알코올중독자인 나-내 삶을 받아들임, 영적인 변화, AA를 통한 회복, 많은 것이 회복되어 감을 느낌>, <그 길을 정직하고 겸손하게 걸어가기 : 새로운 삶의 열쇠-알코올중독, 앞으로 살아가고 싶은 나의 삶-비상>
이러한 분석결과를 바탕으로 한 사회복지 실천적 함의는 다음과 같다.
첫째, 생애주기 상에서 청소년기의 음주문제는 과독기적인 현상으로 이해하는 것만이 아니라 청소년기 초기에서 후기까지 아우르는 장기적인 관점에서 지속적으로 관리하고 점검해야 한다는 것이다. 둘째, 알코올중독자의 삶에서 입원치료는 이들의 삶에 있어 중요한 의미이므로 체계적이고 통합적인 표준화된 알코올중독에 대한 전문적인 입원치료가 이루어지는 것이 필요하다는 것이다. 셋째, 입원치료에 있어서 사회적 지지체계의 강화와 치료의 연속성을 고려한 실질적인 입원 치료서비스의 확립이 필요하다는 것이다. 넷째, 가족치료에의 좀 더 적극적인 개입이 필요하다는 것이다. 다섯째, 알코올중독 문제에 개입하는 정신보건전문가들의 개입에 대한 패러다임의 변화를 필요로 한다는 것이다. 여섯째, 정신보건 전문영역에서 알코올중독자의 회복에 영성의 중요성을 이해하고 영성의 변화와 성숙을 위하여 이를 촉진할 수 있는 실천적 방법과 프로그램의 실시가 필요하다. 마지막으로 알코올중독 문제에 개입하는 정신보건영역의 전문가들의 단주과정에서의 재발에 대한 인식의 전환이 필요하다는 것이다.
이에 따른 정책적 함의는 다음과 같다. 첫째, 알코올중독자의 발생으로 인한 가정적, 국가적 지출과 당사자 개인의 심리적 어려움 등을 줄이고 알코올중독에 대한 사회적 이해 증진을 위해서 음주 문화 및 알코올중독에 대한 올바른 인식 증진을 위한 국가적 차원의 예방적 사업이 필요하다는 것이다. 둘째, 알코올중독의 입원치료에 대한 표준을 마련하기 위한 노력을 국가가 할 필요가 있다는 것이다. 셋째, 입원 치료 후 회복 유지를 위해 지역사회 자원에 대한 체계적인 망의 구성과 활용이 원할 할 수 있도록 하는 정책적인 지원과 치료의 표준화가 이루어 져야 한다는 것이다.
본 연구의 결과를 통해 찾을 수 있는 의의는 다음과 같다. 첫째, 본 연구는 알코올중독환자들의 전반적인 삶의 경험을 탐색한 연구였다는 것이다. 둘째, 알코올중독자의 삶에 대한 질적연구를 수행함에 있어 포토보이스를 활용하였다는 것이다. 셋째, 본 연구를 진행해 나가면서 대부분의 연구참여자들은 자신의 삶을 다시금 되돌아보았고 무엇보다 포토보이스를 준비하는 과정에서 자기통찰과 영성의 성숙을 경험하였다는 것이다. 무엇보다 향후 포토보이스가 사회복지실천에서 당사자들의 역량을 강화하고 강점기반실천을 해 나가고 정책옹호를 하는데 있어서 활용할 수 있는 매우 가치 있는 실천방법으로써 본 연구가 그 가능성을 열어주는 시발점이 되었다는 것에 큰 의의가 있다고 본다.
이상의 연구결과와 논의를 통해서 발견한 의의는 매우 중요한 것이지만 보다 나은 연구를 위해 다음과 같은 제언을 하고자 한다.
첫째, 본 연구는 남성알코올중독자의 삶의 경험에 관한 연구로 모든 알코올중독자에 대한 경험을 반영하는데 한계가 존재한다. 이에 여성알코올중독자의 삶에 관한 질적 연구를 통해 남성알코올중독자의 삶의 경험과 비교연구를 해 보는 것도 의미가 있을 것으로 생각된다. 또 본 연구는 지역적으로 한정되어 이루어진 연구로 좀 더 다양하고 많은 알코올중독자를 대상으로 한 연구가 이루어지기를 희망한다. 둘째, 포토보이스는 이 연구에서 자료를 수집하는 하나의 방법으로 시행되었다. 그럼에도 불구하고 포토보이스를 통하여 연구에 참여한 사람들은 짧은 시간 연구에 참여 했음에도 그 과정에서 자기통찰과 영성의 성숙을 경험하였다. 이에 포토보이스를 통하여 이들이 실제적으로 어떠한 변화들을 이룩할 수 있었는지, 이를 테면 포토보이스에의 참여자들의 영성의 변화나 변화 동기에 대한, 혹은 병식의 변화 등에 대한 다양한 양적 연구가 통합적으로 이루어 질 수 있을 것으로 생각되며 이는 사회복지실천에 있어서 포토보이스의 활용에 좀 더 타당한 근거를 마련해 줄 수 있을 것으로 생각된다. 셋째, 포토보이스는 당사자들의 삶에 대한 이야기들을 타인에게 진실 되고 효과적으로 전달하는 것을 통해 당사자들이 좀 더 공감 받을 수 있도록 돕는 기능을 한다. 그리고 사진이라는 매체를 통하여 정책옹호를 위한 효과적인 수단이 되기도 한다. 이에 알코올중독자의 삶에 대해 당사자들이 찍은 사진은 기관 내에서 혹은 대중적으로 혹은 정책옹호를 하는 과정에서 실제로 활용되어 질 수 있고 이는 좀 더 적극적인 차원의 사회복지 실천을 해 나가는데 도움이 될 것으로 생각된다. 이에 향후 포토보이스를 활용한 사진전 등을 지역의 타자원들과 연계하여 시행해 보는 것도 의미가 있을 것으로 사료된다.
