        Does the Use of Rocuronium-Sugammadex Instead of Succinylcholine in Electroconvulsive Therapy Affect Seizure Duration?

        Objective We compared retrospectively the seizure variables of electroconvulsive therapy (ECT) patients after administration of rocuronium-sugammadex or succinylcholine as a muscle relaxant with propofol anesthesia.Methods The sample comprised 134 ECT patients. The mean age was 33.6±10.48 years. Anesthesia induction was provided with propofol one mg kg-intravenously (IV) followed by succinylcholine 0.5 mg kg<sup>-1</sup> IV (n=68) (Group S) or rocuronium 0.3 mg kg<sup>-1</sup> IV (n=66) (Group R). For patients who were given rocuronium, reversal of the residual neuromuscular block was accomplished with sugammadex (1.5 mg kg<sup>-1</sup> IV). First session seizure variables were compared between the two groups. We also presented the clinical outcome with Clinical Global Impression-Improvement (CGI-I) and overall adverse effects.Results EEG seizure durations in Group R (55.09±36.11 s) and Group S (47.00±26.33 s) were comparable and were not significantly different (p=0.432). The clinical efficacy of ECT measured by CGI-I in both groups was comparable (p=0.075). There were no major complications or death during or after ECT.Conclusion The results of this study show that the use of rocuronium-sugammadex as a neuromuscular blocker instead of succinylcholine during ECT with propofol anesthesia produces similar results in terms of seizure variables and clinical outcomes.

        Assessment of Proportion of Hidden Patients Having Symptoms of Overactive Bladder and Why Has It Been Hidden in Female Outpatients Admitted to Hospital

        Purpose: To determine the proportion of patients with undetected symptoms of overactive bladder by using the overactive bladder-validated 8 (OAB-V8) screening questionnaire and investigate these symptoms were undetected in female patients who were hospitalized. Methods: We invited 2,250 female patients hospitalized in the Aegean region of Turkey to answer a self-administered questionnaire. The questionnaire included questions on evidence of lower urinary tract symptoms (OAB-V8), relevant medical history, and demographic data. Patients with a total OAB-V8 score≥8 were defined as having OAB symptoms. Results: The proportion of patients with OAB symptoms in this study was 40.6%. Nearly 57% of the patients with OAB symptoms had not been previously admitted to any hospital for lower urinary tract symptoms (LUTS). The two most common reasons why women with OAB symptoms did not admit themselves to a hospital because of LUTS were as follows: “I did not think I had a disease” and “The symptoms did not bother me,” with a response rate of 74.7%. The mean OAB-V8 scores of the patients with these two responses were significantly lower than those of the other patients (P<0.001). Conclusions: This is the first study to demonstrate a significant proportion of women with undetected OAB symptoms. The main reasons the women did not admit themselves to a hospital were their unawareness of the disease and because the LUTS were not bothersome. Public awareness programs on this disease may resolve this problem.

        Identification of prestress force in a prestressed Timoshenko beam

        This paper presents a new identification approach to prestress force. Firstly, a bridge deck is modeled as a prestressed Timoshenko beam. The time domain responses of the beam under sinusoidal excitation are studied based on modal superposition. The prestress force is then identified in the time domain by a system identification approach incorporating with the regularization of the solution. The orthogonal polynomial function is used to improve the noise effect and obtain the derivatives of modal responses of the bridge. Good identification results are obtained from only the first few measured modal data under both sinusoidal and impulsive excitations. It is shown that the proposed method is insensitive to the magnitude of force to be identified and can be successfully applied to indirectly identify the prestress force as well as other physical parameters, such as the flexural rigidity and shearing rigidity of a beam even under noisy environment.

        State-space formulation for simultaneous identification of both damage and input force from response sensitivity

        A new method for both local damage(s) identification and input excitation force identification of beam structures is presented using the dynamic response sensitivity-based finite element model updating method. The state-space approach is used to calculate both the structural dynamic responses and the responses sensitivities with respect to structural physical parameters such as elemental flexural rigidity and with respect to the force parameters as well. The sensitivities of displacement and acceleration responses with respect to structural physical parameters are calculated in time domain and compared to those by using Newmark method in the forward analysis. In the inverse analysis, both the input excitation force and the local damage are identified from only several acceleration measurements. Local damages and the input excitation force are identified in a gradient-based model updating method based on dynamic response sensitivity. Both computation simulations and the laboratory work illustrate the effectiveness and robustness of the proposed method.

        Structural damage identification using incomplete static displacement measurement

        A local damage identification method using measured structural static displacement is proposed in this study. Based on the residual force vector deduced from the static equilibrium equation, residual strain energy (RSE) is introduced, which can localize the damage in the element level. In the case of all the nodal displacements are used, the RSE can localize the true location of damage, while incomplete displacement measurements are used, some suspicious damaged elements can be found. A model updating method based on static displacement response sensitivity analysis is further utilized for accurate identification of damage location and extent. The proposed method is verified by two numerical examples. The results indicate that the proposed method is efficient for damage identification. The advantage of the proposed method is that only limited static displacement measurements are needed in the identification, thus it is easy for engineering application.

        Structural damage detection based on residual force vector and imperialist competitive algorithm

        This paper develops a two-stage method for structural damage identification by using modal data. First, the Residual Force Vector (RFV) is introduced to detect any potentially damaged elements of structures. Second, data of the frequency domain are used to build up the objective function, and then the Imperialist Competitive Algorithm (ICA) is utilized to estimate damaged extents. ICA is a heuristic algorithm with simple structure, which is easy to be implemented and it is effective to deal with high-dimension nonlinear optimization problem. The advantages of this present method are: (1) Calculation complexity can be decreased greatly after eliminating many intact elements in the first step. (2) Robustness, ICA ensures the robustness of the proposed method. Various damaged cases and different structures are investigated in numerical simulations. From these results, anyone can point out that the present algorithm is effective and robust for structural damage identification and is also better than many other heuristic algorithms.

        Investigation of the Insulin-like Growth Factor System in Breast Muscle during Embryonic and Postnatal Development in Langshan and Arbor Acres Chickens Subjected to Different Feeding Regimens

        Nutrient availability may control muscle growth directly and indirectly through its influence on regulatory factors. We analyzed the effects of nutrient availability on the breast muscle insulin-like growth factor system. Real time RT-PCR was used to quantify the level of transcription in breast muscle from Langshan (LS) layer and Arbor Acres (AA) broiler chickens subjected to different feeding regimens during embryonic and postnatal development. The AA chickens were fed AA diet (AA, control group) while the LS chickens were either fed LS diet (LL) or AA diet (LA). According to our results, insulin-like growth factor (IGF)-II (embryonic day 16 (E16) - postnatal day 42 (P42)), IGF-I receptor (IGF-IR, E18-P42), and IGF binding protein (IGFBP)-2 (E18-P42), -5 (E16-P14), -7 (E12-P0), and -3 (E12-P0) were positively correlated with IGF-I, while IGFBP-3 (P0-P28) was negatively correlated with IGF-I. In comparison, IGF-IR (E18-P42), IGFBP-2 (E18-P42), IGFBP-5 (E14-P0), and IGFBP-3 (E16-P0) were positively correlated with IGF-II, while IGF-IR (E10-E16) and IGFBP-3 (P0-P28) were negatively correlated with IGF-II. Moreover, IGFBP-2 (E16-P42), -7 (E10-E16), and -3 (E10-E16) were positively correlated with IGF-IR, while IGFBP-3 (P0-P28) was negatively correlated with IGF-IR. Finally, IGFBP-7 (E12-P0) was positively correlated with IGFBP-3, while IGFBP-2 (P0-P28) and -7 (P0-P42) were negatively correlated with IGFBP-3. Overall, the AA chickens exhibited higher levels of IGF-I, IGF-IR, and IGFBP-2 mRNA expression than the LL chickens, while the opposite was true for IGFBP-7. No strain differences in IGF-I, IGF-IR, and IGFBP-7 mRNA expression were detected between LA and AA chickens; however, a strain difference was observed for IGFBP-2. LA chickens exhibited higher levels of IGFBP-2 than LL chickens, while the opposite was true for IGFBP-7. Our data show the first evidence that certain genes may be correlated during specific developmental periods and that strain differences in the expression of those genes in LS and AA chickens are due to differential responses to the same diet.
