Non-Functional P2X<SUB>7</SUB> Receptor; A New Biomarker for Colorectal Cancer

배경 및 목적 : P2X7는 암세포에 기능을 하지 않는 세포자멸사 ATP 수용체이다. 전립샘, 유방, 피부 및 자궁경부의 상피세포에서 악성을 확인하는데 신뢰할만한 표지자로 여겨졌다. 전립선에서 P2X7 수용체는 악성세포의 꼭대기쪽막에 위치하고 있으며, 주위의 정상샘에서도 비슷하게 위치하고 있다. 대상 및 방법 : 이 연구에서는 대장직장암에서 생화학적 표지자로서 P2X7의 적용을 연구하였다. 내시경적 및 외과적 절제술로 158명의 환자에게서 330개의 검체를 연구하였으며, 조직학적 염색을 통하여 양성 혹은 음성을 판정하였다. 결과 : 대장 신생물에서는 주로 세포질에 염색이 되었으며, 민감도는 대장암 조직에서 100%, 선종에서 82%를 보였으며, 특이도는 100%를 보였다. 듀크 병기와 염색 정도는 상관성이 없었다. 결론 : 악성신생물과 정상세포에서 P2X7 염색이 차이를 보이고 있다. 분자수준의 선종-암종 단계에서 세포질과 핵이 염색되는 것이 연관이 있는지 밝히는 것을 계획하고 있다. P2X7 is an apoptotic APT receptor that is non-functional on cancer cells. It has been shown to be a reliable marker for identification of malignancy in the epithelial cells of cancers including the prostate, breast, skin and uterine cervix. In the prostate, the receptor locates strongly to the apical membrane of malignant cells and also similarly locates in adjacent “normal” glands (a field effect). In this study, we tested the applicability of P2X7 as a biomarker in colorectal carcinogenesis. Methods : We examined 330 endoscopic and surgical specimens from 158 consecutive patients. Cytoplasmic staining judged simply as positive or negative provided the best sensitivity and specificity. Results : Staining in colonic neoplastic tissue was dominantly in the cytoplasm, although there was translocation to the nucleus in some patients. Sensitivity for cancer was 100% and for adenomas 82%. Specificity was 100% (negative in normal biopsies). There was no correlation of staining with Dukes’ staging. Conclusions: There was a clear discrimination between malignant neoplasm and normal cells. Correlation of cytoplasmic and nuclear staining with molecular stages of the adenoma-carcinoma sequence is planned.
