        병원급 의료기관 의료부대사업의 경제적 파급효과

        This study is conducted to estimate economic ripple effects of subsidiary services of hospitals. Using the Input/Output Analysis, this study analyzes production inducement effect, added value inducement effect, and labor inducement effect. Also, it assesses potential economic effects of the subsidiary services of which the scope is expanded as the government`s proposed in 2014. Data regarding hospital subsidiary services and economic effects are obtained from the hospitals` financial statements on the National Tax Services and the Bank of Korea. The major results of this study are summarized as follows; subsidiary service profits of hospitals are 466 billion won and rent profits of hospitals are 152 billion won. Of these, the rate of subsidiary service profits in tertiary hospitals is about 66% of total subsidiary service profits. Producement inducement effect of subsidiary services of hospitals is higher than that of total industry, service industry and medical service industry. Added value inducement effect of subsidiary services of hospitals is higher than that of total industry, manufacturing industry, service industry and medical service industry. Job position inducement effect of subsidiary service in hospitals is higher than that of total industry, service industry and medical service industry. Also, employment inducement effect of subsidiary service in hospitals is higher than that of total industry and medical service industry. The results may suggest that subsidiary services revenue in medical institutions contribute to improving operating profits. Facing with intense market competition and pressures to control health care costs, hospitals may need to determine whether subsidiary services help boost their profitability and improve customer satisfaction.

        토우슬과 회우슬의 에탄올 추출물의 성분 및 생리활성 비교

        본 연구에서는 우슬을 이용한 고부가가치 식품개발 연구의 일환으로, 우슬의 기원식물별 성분과 생리활성의 차이를 확인하기 위해 국내산 토우슬과 중국산 회우슬의 ethanol 추출물 및 이의 순차적 유기용매 분획물을 조제하여 각각의 유용성분을 분석하고, 이들의 in-vitro 항균활성, 항산화 및 항당뇨 활성을 비교하였다. 국내 대한약전에서 토우슬 및 회우슬 2종 모두를 우슬로 규정하고 있으며, 구분없이 사용되고 있는 실정이나, 분석 결과 토우슬은 회우슬물보다 지용성 물질을 많이 포함하며, 회우슬의 경우 상대적으로 수용성 당류를 많이 포함하고 있었다. 또한 토우슬 및 회우슬 모두 다양한 종류의 폴리페놀들을 함유하고 있으나, 회우슬이 토우슬보다 상대적으로 많은 총플라보노이드 및 총폴리페놀 함량을 나타내었다. 항균활성 및 α-glucosidase 활성 평가 결과 토우슬의 분획물이 회우슬의 분획물보다 활성이 우수하였으며, 항산화능(DPPH 음이온 소거능, ABTS 양이온 소거능, nitrite 소거능 및 환원력)의 경우 토우슬보다 회우슬이 더욱 우수함을 확인하였다. 본 연구결과는, 국내에서 동일 약재로 사용되고 있는 토우슬과 회우슬은 추출물의 경우 성분 및 효능에서 매우 유사하나, 분획물의 경우 성분 및 활성의 차이가 나타남을 의미하며, 특히 우슬 추출물의 분획물을 이용한 기능성식품 및 천연물 신약 개발시에는 국내산 토우슬과 중국산 회우슬을 구분하여 사용하여야 함을 제시하고 있다. In the course of a study for the development of functional foods utilizing Woosul (Achyranthis radix), the components and various biological activities of Korean Woosul (AJN: Achyranthes japonica Nakai) and Chinese Woosul (ABB: Achyranthes bidentata Blume) were compared. Woosul in Korea, including AJN and ABB, are regulated and part of the Korean Pharmacopoeia. From AJN and ABB, ethanol extracts and their subsequent organic solvent fractions were prepared and their in-vitro antimicrobial, antioxidant and anti-diabetes activities were evaluated. Although AJN and ABB have no clear distinction in terms of usage in Korea, our results suggest that AJN has higher quantities of lipid-soluble components and lower amounts of water-soluble sugars than does ABB. ABB also appears to possess greater amounts of flavonoid and polyphenol substances than AJN. Analyses of biological activities showed that the fractions of AJN were more active as antibacterial agents, and possessed more pronounced α-glucosidase inhibitory activities than those of the ABB fractions. However, the antioxidant activities of the ABB fractions, as determined by DPPH anion-, ABTS cation-, nitrite-scavenging activity and reducing power, were higher than those of the AJN fractions. Our results suggest that the components and bioactivity of the extracts and subsequent fractions of AJN and ABB are different. Therefore, usage of either AJN or ABB should be carefully considered, as regards their individual properties, when the active fractions of Woosul are employed in the development of functional foods or novel plant-derived medicines.

        일개 대학병원 외래환자의 예약부도(No -Show) 관련요인

        Objective: This study examines factors associated with no-show in an academic medical center in Korea. Methods: Electronic data regarding medical appointment during one academic year (2013.03~2014.02) were derived from a major academic medical center in Seoul. Medical appointment was divided into two groups (i.e., consultation vs. test, treatment, and surgery). Comparisons between patients who did show up and those who failed to show up (no-show) were made. A multiple logistic regression was run to estimate factors that are significantly associated with no-show. Results: The results show that overall no-show rate was 5.0%, but the rate of the appointment for test, treatment, and surgery (8.0%) is higher than that of consultation (4.7%). In general, patients aged less than 18 and carrying Medical Aid had higher odds of no-show. Afternoon appointments and Saturday appointments also had higher odds of no-show. In addition, those who had appointments with the Department of Ophthalmology and Department of Otolaryngology and those who made appointments with instructors or residents had higher odds of no-show. Also, slightly different results were found for the two different groups of medical appointment. Conclusion: The results of this study can be used for setting priority for no-show management based on identifying significant no-show predictors. It is important for hospitals to develop more tailored no-show management approaches for consultation, test, treatment, and surgery, respectively, given that the results are somewhat different by the medical appointment type. In addition, it is recommended that future research incorporate qualitative information about the reasons of no-show.

        정신건강의학과 외래 환자들의 도착지연 관련요인 분석: 일개 대학병원 사례

        Objectives: The purpose of this study is to examine factors related to the delayed arrival of appointment time for outpatients of a psychiatric department in an academic medical center in Korea. Methods: This study investigated relationships of delayed arrival with demographic, appointment related, and practice related factors, by analyzing data related to outpatient appointments for about two years from November 1 in 2013 to October 30 in 2014 at a university hospital in Seoul. Results: Among 95,470 final research subjects, 80,871 outpatients arrival in their appointment time, 14,559 outpatients were late, thus, a rate of delayed arrival is 15.29%. As the results of multiple logistic regression analysis, the odds ratios of delayed arrival become greater as they are younger, male , living in the residence, In-cheon and Gyeong-gi area, and their health insurance types are industrial accident compensation insurance and car insurance. Also, the odds ratios of delayed arrival become greater as the appointment time is from 8:30 to 11:00, they appoint in December, January, and February, and temperature gap is over -2 degrees Celsius between the reserved day and average temperature. Similarly, the odds ratio of delayed arrival is higher as return patients, obsessive-compulsive disorder, and practice by doctor B. Conclusions: This study found that various factor influence delayed arrival of appointment time. Medical institutions should find various methods such as focusing continuously on patients who tend to late their arrival time, developing specific appointment system regulating the factors of delayed arrival, and developing guidelines of the patients who tend to late their arrival time.

        중국 현지 의료소비자의 특성 및 의료기관 선택 연구

        Purposes: This study identifies local Chinese consumers' standard for selecting medical care provider and their standard for choosing medical staff, as well as their expectations and concerns regarding Korean medical care providers. Methodology: A survey was conducted in China, to identify Chinese medical care consumers' standards for selecting a medical provider and the factors that influence their use of general hospitals. A total of 1,500 people across three cities, between the ages 18 and 60 participated in the survey. Moreover, a multiple logistic regression analysis was used to analyze the factors that affect Chinese medical care consumers' use of general hospitals. Findings: A total of 75.5 percent respondents chose general hospitals as their most frequently-used medical provider. Those who have health insurance, visit general hospitals as outpatients or are hospitalized more frequently than those who do not have a health insurance. Furthermore, those who have private insurance visit general hospitals as outpatients or are hospitalized more frequently than those who are not signed up for private insurance. Major standards for selecting a hospital included: the doctor's skills, word-of-mouth regarding the hospital, and distance to the hospital from the respondents' home. Standards for choosing medical personnel included word-of-mouth regarding the medical team, recommendations from family members or acquaintances, and medical team's notoriety. Friends and neighbors, family members, television and other media outlets were the channels for acquiring information on a hospital. It was found that Chinese people mostly visit the cardiovascular department of Korean hospitals for treatment. For using Korean hospitals in China, the majority of respondents answered that they were concerned about the cost. Practical Implications: Backed by highly skilled medical experts and cutting-edge technology, Korean medical care providers are attempting to enter China's medical care market. To succeed in China's medical care market, it is vital to conduct a clear and precise analysis.

        복지용구 전시체험관 경험을 통한 인식 변화: A 전시체험관을 중심으로

        연구목적 본 연구는 복지용구 전시체험관에서의 경험을 통해 복지용구에 대한 인식개선 효과를 확인함으로써 복지용구 체험의 필요성을 제시하고, 양질의 복지용구 서비스를 제공하기 위한 개선방안을 모색하고자 한다. 연구방법 2022년 10월부터 2023년 3월까지 전시체험관을 방문한 20세 이상 관람객을 대상으로 복지용구 체험과 사용법 등의 교육을 받은 뒤 관련 인식이 어떻게 변화하였는지 설문조사하였다. 전체 방문객 중 설문조사 참여 의사를 밝힌 305명 중 불성실한 답변 39명을 제외한 266명을 대상으로 본인 또는 가족의 복지용구 사용 여부에 따라 그룹을 구분하였다. 구조화된 설문지를 사용하여 복지용구 이해도와 필요성, 노인의 신체기능, 장기요양보험제도, 전시체험관의 만족도 및 필요성, 복지용구 이용자 대상의 복지용구 만족도 등의 내용을 설문지에 포함하였다. 통계분석은 STATA 17과 SAS Enterprise 8.2를 사용하여 기술통계와 t-test, CHI-SQUARE, 일반화선형모형을 활용한 다중회귀분석을 사용하였다. 연구결과 전반적으로 복지용구의 이해도와 필요성, 노인장기요양보험제도, 노인의 신체기능에서 전시체험관 방문 전후 인식 개선의 효과를 보였으며(p<0.001), 복지용구 이용여부에 따른 그룹별 비교 또한 방문 전후 인식 개선의 효과를 보였다(p<0.001). 복지용구 이용자를 대상으로 전시체험관에서 체험하고 만족한 복지용구는 전동침대, 성인용 보행기, 수동휠체어 순으로 상대적으로 고가의 복지용구 체험 만족도가 높은 것으로 나타났다. 결론 전시체험관의 경험이 방문객의 복지용구에 대한 인식 개선 효과를 가져온 것을 확인하였다. 이를 통하여 노인 비율이 높은 지역을 위주로 전시체험관을 확대하여 복지용구 이용자와 보호자에게 다양한 복지용구 체험과 올바른 복지용구 사용법 교육을 하는 것이 필요하다. 또한 일반 국민을 대상으로 복지용구와 노인과 관련된 인식 개선을 위한 교육 및 홍보 프로그램을 마련해야 할 것이다. Objective : This study investigated to improve the perception about using assistive devices, the physical function of the elderly, and long-term care insurance through an aging-friendly experience exhibition. Methods : From October 2022 to March 2023, a survey was conducted to investigate how visitors’ perception changed after attending an exhibition program—specifically, after experiencing and learning about assistive devices. Among the 358 visitors, 305 expressed an interest in participating in the survey and were categorized according to whether they or their family members used assistive devices. Using a structured questionnaire, we designed the survey to assess their understanding of the physical function of the elderly, long-term care insurance, and assistive devices; their satisfaction and opinions about the necessity of the experience exhibition; and their satisfaction regarding the products targeting assistive device users or their families. Descriptive statistics, t-test, chi-square test, and multiple linear regression analyses were performed using the STATA 17 program and SAS enterprise 8.2. Results : Overall, the experience exhibition improved visitors’ understanding of long-term care insurance, the physical function of the elderly, and the necessity of assistive devices (p<.0001). Comparisons by groups, determined according to whether assistive devices were used, also showed an overall cognitive improvement following the visit (p<.0001). The assistive device user group showed a relatively high satisfaction of using expensive devices in the order of electric beds, adult walkers, and manual wheelchairs. Conclusion : The results confirmed the effect of the exhibition on improving visitors’ perception about assistive device usage. Based on these results, it is necessary to organize such exhibitions mainly in areas with a high older population so that assistive device users or their families can experience various products and become more educated about the correct use of such devices.
