Case Report : Retroperitoneal Fibrosis in a Patient with Rheumatoid Arthritis

A 54-year-old male diagnosed with rheumatoid arthritis (RA) was effectively treated with methotrexate and adalimumab. He was admitted with fatigue and right lower back pain which had persisted for 1 month. An enhanced abdominal computed tomography scan showed an ill-defined mass with soft tissue attenuation surrounding the right common iliac artery involving the right middle portion of the ureter. Laparoscopic ureterolysis and biopsy were performed. Microscopic evaluation confirmed the presence of fibroblastic proliferation, with a pleomorphic inflammatory cell infiltrate consisting predominantly of lymphocytes, macrophages, and vascular endothelial cells, without granuloma or neoplastic cells. Therefore, our diagnosis was retroperitoneal fibrosis (RPF)-associated RA. Clinicians should consider the possibility of RPF in patients with RA who experience lower back pain, abdominal pain, or dysuria, and order suitable imaging studies. (J Rheum Dis 2015;22:391-394)

저자들은 1996년 9월부터 11월 사이에 가톨릭대학교 성가병원 신경외과 중환자실에서 다약제 내성을 보이는 F. adoratum 4 예가 다발성으로 분리되었음을 인지하였다. Flavobacterium adoratum 은 그람 음성 간균으로 이 균에 의한 임상적으로 유의한 감염증례는 전세계적으로 보고된 예가 매우 드물며 국내에서도 현재까지 보고된 예가 없었기 때문에, 이에 저자들은 국내 최초로 보고하는 바이다. 저자들은 역학적 조사를 실시함과 동시에 분자역학적 형별로서 infrequent restriction site polymerase chain reaction (IRS-PCR)을 사용하여 분자수준에서 이 균주들의 일치성 여부를 분석하였다. IRS-PCR 결과 이들 네 균주 모두 동일한 균주임을 증명할 수 있었다. 역학적 조사상 감염원을 규명하지는 못했으나 형별 성적을 토대로 볼 때, 한 균주가 여러 환자들에게 다발적으로 전파되었음을 추정할 수 있었다. F. adoratum에 의한 감염질환은 발생 빈도가 드물지만 다약제 내성의 경향을 보이는 균주라는 점에서 앞으로 주요한 병원 감염균 중 하나로서 주목해야 할 대상이라고 생각한다.
A novel pathogenic role of the ER chaperone GRP78/BiP in rheumatoid arthritis

<▼1><P>The ER chaperone GRP78/BiP is crucial for the development of rheumatoid arthritis.</P></▼1><▼2><P>An accumulation of misfolded proteins can trigger a cellular survival response in the endoplasmic reticulum (ER). In this study, we found that ER stress–associated gene signatures were highly expressed in rheumatoid arthritis (RA) synoviums and synovial cells. Proinflammatory cytokines, such as TNF and IL-1β, increased the expression of GRP78/BiP, a representative ER chaperone, in RA synoviocytes. RA synoviocytes expressed higher levels of GRP78 than osteoarthritis (OA) synoviocytes when stimulated by thapsigargin or proinflammatory cytokines. Down-regulation of <I>Grp78</I> transcripts increased the apoptosis of RA synoviocytes while abolishing TNF- or TGF-β–induced synoviocyte proliferation and cyclin D1 up-regulation. Conversely, overexpression of the <I>Grp78</I> gene prevented synoviocyte apoptosis. Moreover, <I>Grp78</I> small interfering RNA inhibited VEGF<SUB>165</SUB>-induced angiogenesis in vitro and also significantly impeded synoviocyte proliferation and angiogenesis in Matrigel implants engrafted into immunodeficient mice. Additionally, repeated intraarticular injections of BiP-inducible factor X, a selective GRP78 inducer, increased synoviocyte proliferation and angiogenesis in the joints of mice with experimental OA. In contrast, mice with <I>Grp78</I> haploinsufficiency exhibited the suppression of experimentally induced arthritis and developed a limited degree of synovial proliferation and angiogenesis. In summary, this study shows that the ER chaperone GRP78 is crucial for synoviocyte proliferation and angiogenesis, the pathological hallmark of RA.</P></▼2>
The Side-Chain Effects on Photovoltaic Properties of Copolymers Based on Phenothiazine and Thiophene

<P>In this study, the alternating conductive polymers based on phenothiazine and bithiophene unit, poly[(N-10-dodecyl-phenothiazin-3,7-ylene)-alt-(2,2’-bithiophen-5-yl)] (P1) and poly[(N-10- dodecyl-phenothiazin-3,7-ylene)-alt-(4,4’-didodecyl-2,2’-bithiophen-5-yl)] (P2), were synthesized by Suzuki coupling reaction in the presence of palladium catalyst. The structures of synthesized compounds were confirmed by 1H NMR. The optical, electrochemical properties of synthesized polymers were investigated by UV-Vis, cyclic voltammetry (CV). The optical band gap of 2.22 eV and 2.50 eV was obtained from absorption onset of UV-Vis spectrum. Then, the two devices using blends of the polymer, as a donor, and PC71BM, as an acceptor, were fabricated by spin-coating. The device with a P1:PC71BM (1:4, w/w) as an active layer exhibited the best performance with an open circuit voltage (VOC) of 0.68 V, short circuit current (JSC) of 3.5 mA/cm2, fill factor (FF) of 31.8 % and power conversion efficiency (PCE) of 0.74 %.</P>

<P>We compared a real time-nucleic acid sequence-based amplification (RTi-NASBA) with conventional NASBA, galactomannan enzyme immunosorbent assay (GM-EIA), and Mycology Study Group of the European Organization for Research and Treatment of Cancer (EORTC/MSG) criteria for the diagnosis of invasive aspergillosis (IA). From May 2004 to May 2005, blood samples (314 in total) were collected twice a week from 78 patients with hematologic diseases during neutropenic fever after chemotherapy or hematopoietic stem cell transplantation. Results were compared with each other on the basis of EORTC/ MSG criteria. The cutoff of conventional NASBA was set to be 3.5; GM 0.5; RTi-NASBA, 20% above the negative control. There were 22 patients with IA (7 probables and 15 possibles) and 56 patients with nonfungal infection. The Kappa statistic for RTi-NASBA versus conventional NASBA was 0.80 (0.66-0.82; <I>p</I><0.001) indicating that there was fairly good accordance between two tests. RTi-NASBA showed sensitivity 0.96, specificity 0.43, positive- and negative-predictive value 0.40 and 0.96, respectively. GM showed good specificity (0.98), while the sensitivity (0.45) was poor. When we use the combination of GM with either of two NASBAs, the sensitivity was improved up to 100%. In conclusion, RTi-NASBA could be a good alternative to the conventional one for the screening of IA.</P>

In order to efficiently fulfill the agreement Minamata convention, Path to the environment from mercury emissions from the production of mercury should be aware of the environment of mercury flow through the integrated management of the entire process. The reason is low detection rate in the paint product is determined by the dilution due to the pigment while being mixed with a large amount of solvent. When compared to the amount of mercury treatment in this study and in previous studies that the amount of mercury put paint(pigment), The mercury emissions from existing literature 18 times higher, In accordance with the regulations, such as RoHS mercury-containing products to the existing paint is considered to be due to decreased As eradicated.

Gabexate mesilate (GM), a synthetic protease inhibitor, has an antiproteinase activity on various types of plasma serine proteases. However, its role on matrix metalloproteinases (MMPs) has not been identified. In this study, we investigated the effect of GM on MMPs and on the invasion and metastasis of human colon cancer cell lines and neoangiogenesis. The activities of MMPs secreted from these cells were significantly reduced by GM but unaffected by the serine protease inhibitor aprotinin. GM directly inhibited purified progelatinase A derived from T98G human glioblastoma cells. In vitro, GM significantly reduced the invasive ability of colon cancer cells but not cellular motility, whereas aprotinin affected neither. Liver metastatic ability and tumorigenic potential in nude mice were remarkably reduced on treatment with GM. Immunohistochemical analysis of GM-treated tumors in mice showed a marked increase in apoptosis and a significant reduction in tumor angiogenesis. Human umbilical vein endothelial cell proliferation, tube formation, and neoangiogenesis in the rabbit cornea and Matrigel implanted in mice were significantly inhibited by GM. These results suggest that GM is a novel inhibitor of MMPs and that it may inhibit the invasion and metastasis of human colon cancer cells by blocking MMPs and neoangiogenesis.

Vimentin is a growth-related gene and often expressed when epithelial cells are stimulated to proliferate by growth factors. In cancer, vimentin expression is associated with a dedifferentiated malignant phenotype, increased motility, invasive ability and poor prognosis. We studied the regulation of vimentin mRNA and multistep invasion processes following treatment of 12-O-tetradecanoylphorbol 13-acetate (TPA) and all-trans-retinoic acid (RA) in Hep 3B hepatocellular carcinoma cells. TPA showed marked induction of vimentin mRNA, while RA decreased the mRNA level. TPA or RA did not affect cell proliferation, cell-matrix prolein adhesion, and matrix metalloproteinases and urokinase plasminogen activator activities. In vitro invasion ability was significantly increased or decreased with TPA or RA treatment, paralleled to the in vitro motile activity, respectively. These findings suggest that TPA and RA could modulate the invasive potential of Hep 3B cells by altering cellular motility related to differential regulation of vimentin mRNA.
