        Sarcopenia and Post-Operative Morbidity and Mortality in Patients with Gastric Cancer

        Purpose: Surgical resection for gastric adenocarcinoma is associated with significant post-operative morbidity and mortality. The aim of this study was to assess the prognostic significance of sarcopenia in patients undergoing resection for gastric adenocarcinoma with respect to post-operative morbidity and survival. Materials and Methods: A retrospective analysis was conducted on a cohort of consecutive patients who underwent surgical resection for gastric adenocarcinoma between 2008 and 2014. Patient demographics, radiological parameters, and pathological data were collected. OsiriX software (Pixmeo) was used to measure skeletal muscle area, which was normalized for height to calculate skeletal muscle index. Results: A total of 56 patients (41 male, 15 female; mean age, 68.4 ± 11.9 years) met the inclusion criteria. Of these, 36% (20 of 56) of the patients were sarcopenic pre-operatively. Both sarcopenic and non-sarcopenic patient groups were equally matched with the exception of weight and body mass index (P=0.036 and 0.001, respectively). Sarcopenia was associated with a decreased overall survival (log-rank P=0.003) and was an adverse prognostic predictor of overall survival in multivariate analysis (hazard ratio, 10.915; P=0.001). Sarcopenia was a predictor of serious in-hospital complications in multivariate analysis (odds ratio, 3.508; P=0.042). Conclusions: In patients undergoing curative resection for gastric cancer, there was a statistically significant association between sarcopenia and both decreased overall survival and serious post-operative complications. The measurement and reporting of skeletal muscle index on pre-operative computed tomography should be considered.

        Prognosis-Informed Wind Farm Operation and Maintenance for Concurrent Economic and Environmental Benefits

        Advances in high-performance sensing and signal processing technology enable the development of failure-prognosis tools for wind turbines to detect, diagnose, and predict the systemwide effects of failure events. Although prognostics can provide valuable information for proactive actions in preventing system failures, the benefits have not been fully utilized for the operation and maintenance decision-making of wind turbines. This paper presents a generic failure prognosis informed decision-making tool for wind farm operation and maintenance while considering the predictive failure information of an individual turbine and its uncertainty. In the presented approach, the probabilistic damage growth model is used to characterize individual wind turbine performance degradation and failure prognostics, whereas the economic loss measured by monetary values and environmental performance measured by unified carbon credits are considered in the decision-making process. Based on customized wind farm information input,the developed decision-making methodology can be used to identify optimum and robust strategies for wind farm operation and maintenance in order to maximize economic and environmental benefits concurrently. The efficacy of the proposed prognosis-informed maintenance strategy is compared with the condition-based maintenance strategy and demonstrated with a wind farm case study.

        A methodology to define risk matrices – Application to inland water ways autonomous ships

        The autonomous ships’ introduction is associated with a number of challenges including the lack of appropriate risk acceptance criteria to support the risk assessment process during the initial design phases. This study aims to develop a rational methodology for selecting appropriate risk matrix ratings, which are required to perform the risk assessment of autonomous and conventional ships at an early design stage. This methodology consists of four phases and employs the individual and societal risk acceptance criteria to determine the risk matrix ratings for the groups of people exposed to risks. During the first and second phase, the required input parameters for the risk matrix ratings based on the individual risk and societal risk are calculated, respectively. During the third phase, the risk matrix ratings are defined using input from the first and second phases. During the fourth phase, the equivalence between the different types of consequences is specified. The methodology is applied for the case study of a crewless inland waterways ship to assess her typical operation within north-European mainland. The results demonstrate that the inclusion of societal risk resulted in more stringent risk matrix ratings compared to the ones employed in previous studies. Moreover, the adequacy of the proposed methodology and its effectiveness to provide risk acceptance criteria aligned with societal and individual risk acceptance criteria as well as its applicability to conventional ships are discussed.
