        Comparison of Coskun and Lichteinstein hernia repair methods for groin hernia

        Purpose: Coskun hernia repair technique has been reported to be an effective new fascia transversalis repair with its short-term follow-up results. Our aim is to determine the results of Coskun hernia repair technique and to compare it with Lichtenstein technique. Methods: At this comparative retrospective study a total of 493 patients, who had groin hernia repair procedure using Coskun or Lichtenstein technique, between January 1999 and March 2010 were enrolled into the study. Patients were reached by telephone and invited to get a physical examination. Results: Out of 493 groin hernia repairs, 436 (88.5%) were carried out by residents and 57 (11.5%) by attending surgeons. Lichtenstein technique was the choice in 241 patients and 252 patients underwent Coskun hernia repair technique. Groin hernia recurrence was detected in 8 patients (3.1%) in Coskun hernia repair group and 7 patients (2.9%) in Lichtenstein group. Comparison of early complication rates in Coskun group (3.9%) and Lichtenstein group (4.5%) showed no significant difference. Late complication rates were significantly higher in Lichtenstein group (1.2% vs. 4.9%). The operation time was shorter in Coskun group (44 minutes) than in Lichtenstein group (60 minutes). Subgroup of patients, whose hernia repair operations were carried out by attending surgeons, had a recurrence rate of 0% and 3.8%, in Coskun group and Lichtenstein group, respectively. Conclusion: This study showed that Coskun hernia repair technique has a similar efficacy with Lichtenstein repair, on follow-up.

        Novel active and passive balancing method‑based battery management system design and implementation

        In this study, a novel battery management system (BMS) circuit topology based on passive and active balancing methods was created and implemented for battery-based systems. The circuit topology was designed so that both of the control methods can be applied when suitable software is used. A resistance-based passive control method was used. MOSFET switching elements were used as a load instead of a resistance. A bypass technique-based active balancing method was applied to the designed BMS. The applicability of these methods was separately tested as passive and active-based experimental systems. During the charging process, a voltage increase of the cells was observed at approximately 0.140 V after 16 min in the passive balancing mode. On the other hand, the voltage increase was measured at 0.155 V after 14 min in the active balancing mode. The average temperature of the cells in charge states was measured as approximately 25 °C. The obtained experimental results verified that the proposed BMS topology can work successfully in both active and passive balancing operations. As a result of this study, active and passive balancing circuit structures were combined, and a new circuit topology has been introduced.

        Can We Prevent Coffee Stains on Teeth?

        This study evaluates whether some common beverages treated before coffee could protect or increase tooth staining caused by coffee. Initial color of 50 incisor teeth were measured with a spectrophotometer and recorded according to CIELAB color system. Teeth were randomly divided into five groups, water (control), milk, green tea, orange juice, and cola (n = 10) and were kept in selected beverage for 10 min. Immediately afterward, they were immersed in coffee and allowed to stand for 24 h. The treatment was repeated for 5 days. At the end of the fifth day, L*a*b* color measurements of the teeth were repeated. Calcium, phosphorus, potassium, and magnesium changes on representative teeth surfaces were also investigated with X-ray fluorescence spectroscopy. Color differences were calculated with both CIEab and CIE00 formulas. Groups were compared with Kruskal–Wallis test complemented by the Bonferroni correction and Mann–Whitney U test for pairwise comparisons (P = .05). The teeth submitted to coffee challenges after distillated water or beverages showed a perceptible color change. Soaking in cola or orange juice before coffee immersion caused severe tooth discoloration. All the beverages tested here were not able to protect the tooth from coffee staining. People should be informed that some acidic beverages consumed before a coffee can worsen the coffee-based tooth discolorations.

      • Detection of PIK3CA Gene Mutations with HRM Analysis and Association with IGFBP-5 Expression Levels in Breast Cancer

        Breast cancer is the second most common cancer and second leading cause of cancer deaths in women. Phosphatidylinositol-3-kinase (PI3K)/AKT pathway mutations are associated with cancer and phosphatidylinositol-4, 5-bisphosphate 3-kinase catalytic subunit alpha (PIK3CA) gene mutations have been observed in 25-45% of breast cancer samples. Insulin growth factor binding protein-5 (IGFBP-5) can show different effects on apoptosis, cell motility and survival in breast cancer. We here aimed to determine the association between PIK3CA gene mutations and IGFBP-5 expressions for the first time in breast cancer patients. Frozen tumor samples from 101 Turkish breast cancer patients were analyzed with high resolution melting (HRM) for PIK3CA mutations (exon 9 and exon 20) and 37 HRM positive tumor samples were analyzed by DNA sequencing, mutations being found in 31. PIK3CA exon 9 mutations (Q546R, E542Q, E545K, E542K and E545D) were found in 10 tumor samples, exon 20 mutations (H1047L, H1047R, T1025T and G1049R) in 21, where only 1 tumor sample had two exon 20 mutations (T1025T and H1047R). Moreover, we detected one sample with both exon 9 (E542Q) and exon 20 (H1047R) mutations. 35% of the tumor samples with high IGFBP-5 mRNA expression and 29.4% of the tumor samples with low IGFBP-5 mRNA expression had PIK3CA mutations (p=0.9924). This is the first study of PIK3CA mutation screening results in Turkish breast cancer population using HRM analysis. This approach appears to be a very effective and reliable screening method for the PIK3CA exon 9 and 20 mutation detection. Further analysis with a greater number of samples is needed to clarify association between PIK3CA gene mutations and IGFBP-5 mRNA expression, and also clinical outcome in breast cancer patients.
