        ASK1-ER stress pathway-mediated fibrotic-EV release contributes to the interaction of alveolar epithelial cells and lung fibroblasts to promote mechanical ventilation-induced pulmonary fibrosis

        Recent clinical research has revealed that mechanical ventilation (MV) can initiate pulmonary fibrosis and induce mechanical ventilation-induced pulmonary fibrosis (MVPF). However, the underlying mechanism remains largely uncharacterized. Based on a mouse model of MVPF and an alveolar epithelial cell cyclic strain model, the present study explores the possible mechanism of MVPF. Single-cell RNA-sequencing and EV RNA-sequencing analysis revealed that MV promoted apoptosis signal-regulating kinase 1 (ASK1)-mediated endoplasmic reticulum (ER) stress pathway activation and extracellular vesicle (EV) release from alveolar epithelial cells. Furthermore, the ASK1-ER stress pathway was shown to mediate mechanical stretch (MS)- or MV-induced EV release and lung fibroblast activation in vivo and in vitro. These processes were suppressed by ER stress inhibitors or by silencing ASK1 with ASK1- short hairpin RNA (shRNA). In addition, MVPF was suppressed by inhibiting ASK1 and ER stress in vivo. Therefore, the present study demonstrates that ASK1-ER stress pathway-mediated fibrotic-EV release from alveolar epithelial cells contributes to fibroblast activation and the initiation of pulmonary fibrosis during MV. The inhibited release of EVs targeting the ASK1-ER stress pathway might be a promising treatment strategy for MVPF.

      • Molecular Investigation of Isocitrate Dehydrogenase Gene (IDH) Mutations in Gliomas: First Report of IDH2 Mutations in Indian Patients

        Recent genome wide sequencing has identified mutations in IDH1/IDH2 predominantly in grade II-III gliomas and secondary glioblastomas which are associated with favorable clinical outcome. These mutations have become molecular markers of significant diagnostic and prognostic relevance in the assessment of human gliomas. In the current study we evaluated IDH1 (R132) and IDH2 (R172) in 32 gliomas of various grades and tumor subtypes. Sequencing analysis revealed R132H mutations in 18.7% tumors, while none of the cases showed IDH2 (R172) mutations. The frequency of IDH1 mutations was higher in females (21.4%) than males (11.1%), and it was significantly higher in younger patients. Histological analyses demonstrated presence of necrosis and micro vascular proliferation in 69% and 75% respectively. Interestingly, IDH1 mutations were predominantly present in non-necrotic tumors as well as in cases showing microvascular proliferation. Of the six IDH1 positive cases, three were glioblastomas (IV), and one each were anaplastic oligoastrocytoma (III), anaplastic oligodendroglioma III (n=1) and diffuse astrocytoma. In conclusion, IDH1 mutations are quite frequent in Indian glioma patients while IDH2 mutations are not observed. Since IDH mutations are associated with good prognosis, their use in routine clinical practice will enable better risk stratification and management of glioma patients.

        Remineralizing Agents: A Comprehensive Review

        Remineralizing agents are the most desirable means of early enamel caries management. There is a scope for such agents as they prevent the progression of white spot lesions and reverse the process of demineralization. This need has redirected research to develop novel preventive agents that can act as an adjunct to fluoride or independent of it.

        An In Vitro Study to Evaluate and Compare the Effects of Various Commercially Available Remineralizing Agents on Surface Microhardness of Artificially Produced Enamel Lesions

        Objective: To evaluate the remineralization potential of various commercially available remineralizing agents on artificially produced enamel lesions. Methods: Twenty five human molar teeth extracted for periodontal reasons were collected for this study. Specimens were divided into various groups according to the remineralizing agent used, G1- Colgate Prevident, G2- GC Tooth Mousse, G3- GC Tooth Mousse Plus, G4- Clinpro and G5- SHY-NM Novamin. The baseline surface microhardness (SMH) was measured for all the specimens using the vickersmicrohardness testing machine. Artificial enamel lesions were created by inserting the specimens in demineralizing solution for 96 h. SMH of demineralized specimens was evaluated. Following which 10 days of pH-cycling regimen was carried out and then SMH of remineralized specimens was evaluated. Statistical analysiswas done using Friedman test with Wilcoxon signed rank test for intra group comparison and Kruskal Wallis test with Mann Whitney U test for inter group comparison. p<0.0001 was considered to be significant. Results: Regain in SMH was higher in Clinpro group as compared to Prevident group. Regain in SMH in Clinprogroup & Previdentgroupwas significantly higherthan Novamin group, which was significantly higher than GC Tooth Mousse plus group, which was further significantly higher than GC Tooth Mousse group. Conclusion: All remineralizing pastes used in this study could effectively remineralize the artificially produced early enamel lesions; however, none of them could regain the surface microhardness to the level found at baseline. Clinpro showed best remineralizing efficacy among all test groups.
