Erythema annulare centrifugum consists of centrifugally erythematous eruption with central clearing producing arcuate, festooned, and polycyclic patterns. Its etiology is mostly obscure. However, a few of the patients seem to be related to some underlying diseaseas, such as autoimmune diseases, internal malignancies, drugs, or infections and often its course parallels to the process of the underlying diseases. 27 year old female patient showed eruption over the trunk and both extremities, enlarging centrifugally with clearing in the center with frequent upper respiratory tract infections from childhood. On the chest P-A, tumor mass was revealed, and it was confirmed as the hamartoma later. The eruption was disappeared totally 24 hours after surgical removal of the hamartoma. Hence, the hamartoma seemed to be responsible in this patient for the flare-up of the erythema annulare centrifugum, but its pathogenesis is not clearly understood. The diagnosis of the erythema annulare centrifugum was assisted by the histopathological finding.
Transmission of Oral Desensitization : From Mother to Offspring

Oral desensitization and tolerance to dinitrochlorobenzene (DNCB) contact sensitization were induced in two pregnant guinea pigs by repeated feeding of specific antigen. The litter borne by Mother A that was in the state of desensitization to DNCB was divided into two groups: line was sensitized and challenged by DNCB and the other by trinitrochlorobenzene (TNCB). The litter borne by Mother B that was in the state of tolerance to DNCB was sensitized and challenged by DNCB. The index of sensitivity of the neonates that were borne by the desensitized mother and sensitized to DNCB was 41.7%, whereas that of the neonates sensitized to TNCB was 54.2%. The index of sensitivity of the litter borne by the tolerant mother was 37.5%. This experiment reveals that oral desensitization as well as immunologic tolerance can be transferred from mother to offspring in guinea pigs.

항원을 경구투여함으로 면역학적 관용상태가 유발된다는 보고는 많으나 이미 감작된 실험동물에게 항원을 경구투여함으로 탈감작이 초래된다는 보고는 흔치 않다. 또한 면역학적 관용상태가 초유를 통하여 새끼에게 전이된다는 보고는 있으나. 탈감작된 모체에서 초유를 통하여 새끼에게 탈감작 상태가 전이된다는 보고는 아직없다. 저자들은 2마리의 임신된 기니픽에서 면역학적 관용상태와 탈감작을 일으킨 후 그들에게서 태어난 새끼에게 이들 상태가 전이되는가를 실험하였다. DNCB 탈감작을 일으킨 어미에서 출생한 4마리새끼를 두군으로 나누어 각각 DNCB와 TNCB로 감작 및 유발시험을 한 결과 탈감작 상태인 DNCB군에서의 감작지수는 41.7%였으며 대조군인 TNCB군에서는 54.2%였다. 관용상태를 일으킨 다른 한 마리에서 출생한 2마리의 새끼는 DNCB로 감작 및 유발시험을 한 결과 감작지수는 37.5%로써 대조군보다 반응의 가?ぐ?떨어짐을 알 수 있었다. 저자들은 이 실험에서 어떠한 경로로 전이가 일어나는지는 밝힐 수 없으나 관용상태 뿐만 아니라 탈감작도 기니픽 어미로부터 새끼에게 전이된다는 사실을 관찰할 수 있었다. Herpes zoster is known to be more frequent and severe in patients during immunosuppressive treatment and various predisposing factors, such as Hodgkin's disease, advanced tumor stage, local X-ray irradiation, splenectomy and cutaneous anergy. This report presents the case of a 23-year-old woman who developed herpes zoater on two separate occasione during the treatment of systemic lupus erythematosus with immunosuppressive agents (corticosteroid and cyclophosphamide). In each occasion, it was involved by different and multiple dermatomes. Treatment with oral and intralesional corticosteroids administration proved to be beneficial without any complication, although some delayed recovery of the disease was noted.

Lichen planus is a common, usually intensely pruritic papulosquamous dermatosis with unknown etiology, which may involve the skin, mucous membranes, hair follicles, and nails. Its lesions are usually a localized form but generalized ones are an unusual presentation of lichen planus. We report herein three cases of generalized lichen planus in one male and two female patients, which was diagnosed on clinical and histopathological examinations in all cases. A Case of Juvenile Spring Eruption of the Ears

Juvenile spring eruption(JSE) of the ears is an unusual type of photodermatosis, which develops on the light exposed areas of the ears of boys and young male adults in the early spring months. JSE has received little attention in the literature, and to our knowledge no cases have been reported in Korea until now. Herein we report a case of JSE occurring in a 17-year-old man who has suffered from a recurrent pruritic erythematous papulovesicular eruption of both helix, followed by crusting and healing without scarring within one to two months early each spring for six years.

Background: Proper education regarding inhaler usage and optimal management of chronic obstructive pulmonary disease (COPD) is essential for effectively treating patients with COPD. This study was conducted to evaluate the effects of a comprehensive education program including inhaler training and COPD management. Methods: We enlisted 127 patients with COPD on an outpatient basis at 43 private clinics in Korea. The patients were educated on inhaler usage and disease management for three visits across 2 weeks. Physicians and patients were administered a COPD assessment test (CAT) and questionnaires about the correct usage of inhalers and management of COPD before commencement of this program and after their third visit. Results: The outcomes of 127 COPD patients were analyzed. CAT scores (19.6±12.5 vs. 15.1±12.3) improved significantly after this program (p<0.05). Patients with improved CAT scores of 4 points or more had a better understanding of COPD management and the correct technique for using inhalers than those who did not have improved CAT scores (p<0.05). Conclusion: A comprehensive education program including inhaler training and COPD management at a primary care setting improved CAT scores and led to patientsc better understanding of COPD management.

Background: Skin acts as the first line of defense against any foreign materials outside of our body. In inflammatory skin disease, the pathogenesis is due to an immune reaction in the keratinocytes, immune cells and soluble mediators. Balneotherapy is widely used for the treatment of inflammatory skin disease, but the mechanisms are only partly understood by immune regulation. Balneotherapy in dermatologic disease can affect the secretion of pro- inflammatory cytokines, IL-1 a and tumor necrosis factor from keratinocytes, and possibly affect the T cell differentiation, Objective: In this study, we evaluated the effect of spa spring water from Yong-gung oncheon on the cells, and investigated the skin immune reaction, Methods: We investigated the immunomodulatory or anti-inflammatory effect of thermal spring water on the expression of pro-inflammatory cytokines in the HaCaT cells under Toll-like receptor (TLR) stimulation, as well as the effect on the differentiation of CD4+T cells under spring water. Results: The treatment of spa spring water from Yong-gung oncheon decreased the expression of pro-inflammatory cytokines under TLR stimulation to the HaCaT cells and antigen presenting cells. In addition, spa spring water attenuated the differentiation process of subsets of CD4+T cells, i.e., Th1, Th2 and Th17 cells. All these immune parameters can be used to evaluate the efficacy of spa spring water in Korea, in terms of the immune modulatory effect. Conclusion: Spa spring water treatment suppressed the inflammatory cytokines production and also modulated the differentiation of CD4+T cells into Th1, Th2, and Th17 cells, but not the Tregs cells.
