Pattern of Fractures in Non-Accidental Injuries in the Pediatric Population in Singapore

fracBackground:Fractures as a result of non-accidental injuries (NAI) are not uncommon among children. The purpose of our study was to describe the incidence, demographic characteristics, and associated risk factors in patients with NAI in a multiethnic Asian cohort. Methods: A retrospective record review of patients admitted to our hospital between September 2007 and 2009 with the diagnosis of NAI was conducted. Results: A total of 978 children were reported with suspicion of NAI. Among them, 570 patients (58.28%) were diagnosed with NAI. Fractures were observed in 35 children (6.14%). NAI fractures were highest among female infants (73.3%). The biological father was the most common known perpetrator of NAI (n = 155, 29.0%). The most common perpetrator sadly remained unknown (n = 14, 40%). All NAI fractures were closed (n = 35, 6.14%), and the most commonly affected bone was the humeral shaft (n = 10, 28.57%) with an oblique configuration. Age < 1 year and parental divorce were significant risk factors associated with these fractures. Conclusions: The skeletal injury pattern and risk factors highlighted in our study will help treating physicians identify patients susceptible to NAI, as many of these patients are young and vulnerable. Protective measures can be initiated early by recognizing these injuries and preventing further physical and psychological harm to the child.
Study on Aero-derivative Turbo Shaft Engine Development for MGET™ Test Bench

MGETTM 테스트 벤치는 추력 140N급의 마이크로 터보 제트 엔진을 이용하여 가스터빈 엔진의 지상성능 시험을 수행할 수 있도록 개발된 교육용 시험장치 이다. 현장 에서 대상 엔진의 다양화가 요구되어 MGETTM 테스트 벤치의 터보 제트 엔진을 터보 축 및 터보 프롭 엔진으로 개조에 대한 연구가 대두 되었다. 상용으로 개발된 마이크로 엔진은 대부분 100~200 N급 터보 제트 엔진이며 일부 제작사만이 터보 프롭 엔진으로 개조하여 제공하고 있고 터보 축 엔진은 상용화 되어 있지 않은 실정이다. 따라서 본사에서는 MGETTM 테스트 벤치에 터보 축 엔진을 사용하기 위해 140 N급의 터보 제트 엔진을 7 ㎾급의 터보 축 엔진으로 개조하는 연구에 착수하였다. 본 논문에서는 개조된 터보 축 엔진의 성능해석 및 기본 설계에 관하여 기술하였다. MGETTM Test bench acquires micro jet engine, of thrust class 140 N. Test bench used for educational purpose, for study of micro jet engine propulsion and its performance. Further developments of MGETTM Test bench is considered to integrate turbo shaft/ turbo prop engines, to study its performance and power offtake. Micro engines which are available commercially of thrust class 100~200 N doesn"t support for additional power offtake, as they designed for full performance usage for thrust generation. And only few OEM" s have turbo prop designs which differ from turboshaft configuration. Thus, we developed power offtake from commercially available micro engines with few modifications. This, paper includes brief study of 7 ㎾ power extraction from 140 N thrust class micro jet engine. Successful concepts with its performance model data and engine modifications details are included.
Design and Simulation of Noise Suppressor for MGET™ Test Bench

MGET™ 테스트 벤치에는 마이크로 제트 엔진이 사용되며 회전수 100% rpm에서 최대 120 db의 소음이 발생한다. 그러나 OSHA에 따르면 산업 소음 기준은 90-100 PNDB 이하 및 최대 100-115 PNDB 이내여야 하고 ICAO에 따른 터보 제트 엔진의 소음 기준은 더욱 엄격하여 105 db을 초과해서는 안된다. 마이크로 제트 엔진은 주로 배기가스와 압축기에서 소음이 발생되지만 본 연구에서는 배기가스 소음 감소기에 대하여 연구하였다. 본 연구에서는 MGET™ 테스트 벤치의 소음 감소기 목표 값을 10db 감소로 하였고 배기가스 소음이 최대 105db 정도로 예상되기 때문에 OSHA 등에서 규정한 소음 기준에 합당할 것으로 사료된다. 이에 목표 값을 만족하는 multilobe 형태의 소음 감소기를 설계하였고, CFD 해석을 수행하여 비교한 결과 10-13 db 감소함을 확인하였다. 추후 본 연구를 통하여 설계한 multilobe 소음 감소기를 제작하여 검증할 예정이다. MGET™ Test Bench acquires micro jet engine, which generates a peak noise level of 120db at 100% RPM. But according to OSHA (Occupational Safety and Health Administration), industry noise level is below 90-100 PNDB (Perceived Noise in Decibel) and 100?115 peak PNDB. Also turbojet engines are certified in aviation with ICAO (International Civil Aviation Regulations) regulations. Where noise limits are very stringent PNDB should not exceed 105 dB. Micro jet engine is a pure jet engine which has its major noise generated from exhaust jet and Compressor. But, here we study exhaust jet noise suppression from nozzle. For developing noise suppression concepts a target noise 10db is considered, as such perceived noise after suppression can be around 105 dB which is acceptable by OSHA and other organizations. All research data and concepts testing data is predicted and compared, 10-13 db noise suppression is observed from multilobe suppressor design. Further study, can be done by prototype testing of multilobe suppressor.

Designing the time domain training sequences is very critical in multi carrier transmission which degrades the performance as it is contaminated by diff erent blocks in diff erent cells. To improve the spectral effi ciency and high accuracy, MU-MIMO needs the sensing matrix to be reduced by using the training sequence design and optimization. Integrating the training sequence design and sparse channel estimation improves the capacity of the system. The capacity can be enhanced by reducing the bit error rate. The system capacity for multi user- multi-input and multi output (MU-MIMO) is studied by proper channel estimation with compressed sensing model. The design and optimization of training sequence is analysed for MUMIMO model using auto coherence and block coherence matrices. The block coherence matrix is optimized using cuckoo algorithm for obtaining lower coherence value for diff erent sparsity values. The performance improvement in terms of signal to noise ratio is 1 dB for single user- multi-input and multi output (SU-MIMO) using genetic algorithm and the performance of MU-MIMO is observed to be 0.93 dB using cuckoo algorithm.

Selective Unilateral Inner Hair Cell Injury: A Case Report

Sensorineural hearing loss (SNHL) is known to be associated with lesions at various sites, including outer hair cell (OHC) and inner hair cell (IHC) injury, as well as auditory dyssynchrony or vestibulocochlear nerve tumors. Therefore, it is important to establish the differential diagnosis of SNHL, which however is particularly challenging in patients with unilateral SNHL. The test battery approach is effective for accurate diagnosis in such cases. In this report, we discuss the usefulness of the test battery approach to accurately detect the site of the lesion in a patient with unilateral hearing loss. A 14-year-9-month-old adolescent who observed decreased hearing sensitivity in the left ear accompanied by difficulty with understanding speech at a distance was diagnosed with unilateral hearing loss in the left ear. In this report, we describe a rare and diagnostically challenging case of unilateral SNHL in a patient with normal OHC function and selective injury to IHCs, which was diagnosed using a test battery approach.
Central venous catheter malposition due to dialysis catheter -a case report-

A 56-year-old man on maintenance hemodialysis was admitted to the intensive care unit with septic shock and coagulopathy. As there was a dialysis catheter in the right internal jugular vein, the left internal jugular vein was cannulated with a central venous catheter to initiate vasopressor therapy. A chest X-ray showed formation of a catheter loop inside the left brachiocephalic vein, probably due to hindrance by the dialysis catheter. This report describes the hurdles encountered, repeated cannulation attempts, and serial chest X-ray findings required to obtain acceptable placement of the catheter tip.

Newly isolated strains Bacillus sp. FME 1 and FME 2 were evaluated for the cellulolytic enzymes production during submerged fermentation (SmF) of different substrates including rice husk, Whatman filter paper and cellulose powder CF 11. Extracellular enzyme assays for CMCase, FPase and β-glucosidase were examined up to 8 days of submerged fermentation. Among the three substrates, rice husk was the most suitable substrate for higher production of cellulolytic enzymes. Maximum titers of 100, 45, and 3.5 U/mL in respect of CMCase, FPase and β-glucosidase in Bacillus sp. FME 2 were recovered as against 45, 12, and 0.39 U/mL in Bacillus sp. FME 1 respectively, at their respective peak time intervals. Bacillus sp. FME 2 was found to produce higher cellulolytic enzyme activities than Bacillus sp. FME 1.
