        Seeking the unseen: Localization and surgery for an occult sporadic insulinoma

        Insulinomas are rare pancreatic neuroendocrine tumours and the commonest cause for endogenous hyperinsulinaemic hypoglycemia. Small tumours are not easily detected by conventional cross-sectional imaging making localization prior to surgical removal a challenge. Selective arterial calcium stimulation is an invaluable adjunct to localization in such circumstances. This is further supplemented by intraoperative ultrasonography. A 39-year-old male was referred with features of Whipple’s triad of 10 months duration. Clinical and biochemical evaluation including C-peptide and serum insulin levels during supervised hypoglycemia concluded endogenous hyperinsulinaemia as the underlying aetiology. Contrast CT and MRI of the abdomen failed to localize the tumour. Selective arterial calcium stimulation localized the lesion in distal pancreas. During the surgery, tumour was further localized to the tail of the pancreas using intraoperative ultrasonography and enucleated. Histology confirmed an insulinoma and patient made an unremarkable recovery and was well more than a year after the surgery.

        Estimating regional bone mineral density-based T-scores using clinical information; tools validated for postmenopausal women in Sri Lanka

        Objectives: This study aims to develop and validate a country specific osteoporosis risk assessing tool for Sri Lankan postmenopausal women. Methods: Community-dwelling postmenopausal women were enrolled to development (n ¼ 602) and validation (n ¼ 339) samples. Clinical risk factors (CRFs) of osteoporosis were assessed. Bone mineral densities (BMD) of femoral neck, total hip and lumbar spine were assessed by dual energy X-ray absorptiometry (DXA) scan. Radial ultrasound (US) bone scan was done. Linear regression analysis was performed in development sample considering regional BMDs as dependent and CRFs as independent variables. Regression equations were developed to estimate regional BMDs using best predictive CRFs. Sensitivity, specificity, positive and negative predictive values (PPV and NPV) were assessed to validate the new tools. Results: Age, body weight and US T-scores showed positive correlations with BMDs of all 3 sites. Two osteoporosis risk assessing tools (OPRATs) were developed as OPRAT-1 and OPRAT-2. Prevalence of osteoporosis, in the validation sample was 74.3%. Sensitivity were high in both tools (OPRAT-1 and OPRAT-2; 83.2% and 82.5%) while specificity were moderate (44.8% for both). PPV of OPRAT-1 and OPRAT2 were 79.5% and 81.2%. Both tools showed moderate NPV (OPRAT-1 and OPRAT-2; 51% and 47%). Conclusions: Both OPRAT-1 and OPRAT-2 have high performance in screening postmenopausal women in Sri Lanka for risk of osteoporosis. OPRAT-2 is more convenient and can be used in any healthcare setting with limited resources to identify women who will be benefitted by DXA. OPRAT-1 can be used if the radial US facility is available.

        Pterocarpus santalinus Linn. f. (Rath handun): A Review of Its Botany, Uses, Phytochemistry and Pharmacology

        Pterocarpus santalinus Linn.f., commonly known as Red sanders, belongs to the family Fabaceae. It is endemic to India and considered globally endangered, with illegal harvest being a key threat. The plant is renowned for its characteristic timber of exquisite color, beauty, and superlative technical qualities. The red wood yields a natural dye santalin, which is used in coloring pharmaceutical preparations and foodstuffs. In the traditional system of medicine, the decoction prepared from the heartwood is attributed various medicinal properties. It has been used in inducing vomiting and treating eye diseases, mental aberrations, and ulcers. The heartwood of Red sanders is known to have antipyretic, anti-inflammatory, anthelmintic, tonic, hemorrhage, dysentery, aphrodisiac, and diaphoretic activities. It has also been used as a cooling agent. Ethanol extract of stem bark was reported to possess anti-hyperglycaemic activity. The wood in combination with other drugs is also prescribed for snake bites and scorpion stings. Phytochemical investigations of aqueous and ethanol extracts of stem bark revealed the presence of alkaloids, phenols, saponins, glycosides, flavonoides, triterpenoides, sterols, and tannins. The heart wood contains isoflavone glucosides and two anti-tumour lignans, viz., savinin and calocedrin. However, the species has remained unexplored for many pharmacological activities claimed. The present paper reviewed botany, uses, phytochemistry, and pharmacology of P. santalinus.

        Parenchymal-sparing hepatectomy for multiple bilobar colorectal liver metastases in a Jehovah’s witness: a case report

        Parenchymal-sparing hepatectomy (PSH), though technically challenging, is emerging as a choice of treatment for colorectal liver metastases (CRLM). PSH in Jehovah’s witness (JW) patients, for whom transfusion is not an option, involves complex surgical and medicolegal issues. A 52-year-old JW male with synchronous, multiple, bilobar liver metastases from a rectal adenocarcinoma was referred following neoadjuvant chemotherapy. At surgery, 10 metastatic deposits were observed and confirmed by intraoperative ultrasonography. Parenchymal-sparing non-anatomical resections were performed using a cavitron ultrasonic aspirator with the application of intermittent Pringle maneuvres. Histology confirmed multiple CRLMs with tumor-free resection margins. PSH is increasingly employed for CRLMs to preserve residual liver volume and minimize morbidity without compromising oncological outcomes. It is technically challenging, especially in the presence of bilobar, multi-segmental disease. This case illustrates the feasibility of performing complex hepatic surgery in special patient groups by meticulous planning and preparation involving multiple specialties and the patient.

        Fine coal beneficiation by column flotation

        The amenability of beneficiating a fine hard coal from Hwa-Sun coal mine using column flotation has been studied using a CoalPro flotation column developed by Canadian Process Technologies (CPT). After initial testwork using a batch flotation cell to determine the optimal flotation conditions, tests were carried out to compare its performance with the column. The results showed that the performance of the column was far superior to that of the conventional cell owing to the large fines content present in the coal. It was established that a stable froth column of 40cm in height and a suitable balance between froth and collection zone could be maintained at 1.0cm/s superficial gas rate. A suitable reagent scheme and the optimal operating conditions that minimized adverse effects generally encountered in treating fine coal, such as gangue entrainment, have been identified. An increase in water recovery significantly decreased ash rejection implying entrainment of fine ash particles. Column flotation is capable of producing an acceptable clean coal concentrate of 85% combustible recovery with 81% ash rejection at a maximum separation efficiency of 62%, compared to conventional flotation which has 70% recovery with 70% ash rejection at an efficiency of 42%.

        Ability of FRAX Sri Lanka adjusted for trabecular bone score to discriminate between postmenopausal women with a recent fracture and without a fracture

        Objectives: We evaluated the ability of fracture risk assessment tool (FRAX) Sri Lanka to discriminate between women with a recent fracture and without a fracture, when trabecular bone score (TBS) is added to the calculation. Methods: We studied 394 women without previous fractures and 87 women who underwent dual energy X-ray absorptiometry within 3 months after the first fragility fracture. Fracture probabilities (FP) were estimated with and without TBS using Sri Lankan FRAX model and their ability to discriminate those with and without fracture was tested. Results: Women without fractures had higher bone mineral densities (BMDs) and lower FPs, compared to those with a recent fracture. Area under curves of receiver operating characteristic for FPs unadjusted were not different from those adjusted for TBS. The odd ratios of FPs unadjusted were not different from those of adjusted. The FPs estimated with TBS were higher, hence the intervention thresholds (ITs) were higher compared to FPs estimated without TBS. Thirty-two percent of women without previous fracture were above the ITs and the inclusion of TBS increased this to 36%. The integrated discriminatory index analysis showed a 8% increase in the discriminatory slope. Conclusions: The inclusion of TBS to Sri Lankan FRAX did not show an added advantage in discriminating between postmenopausal women with a recent fracture and without a fracture. TBS inclusion in fracture risk calculation among those without previous fractures, however, showed a marginal increase in the number of women above ITs.
