        A Multi-Center Educational Research Regarding Breastfeeding for Pediatrics Residents in Korea

        Purpose: Pediatricians have a significant responsibility to educate mothers about the importance of breastfeeding. However, there have been minimal efforts in the courses of resident training in Korea. The purpose of this study is to evaluate the change in knowledge and attitude before and after a 4-week breastfeeding educational intervention among multicenter residents. Methods: Prospective interventional educational research was designed for residents at eight training hospitals in Korea. Institutional reviews were obtained in each hospital. The education curriculum consisted of 14 courses regarding breastfeeding theory and practice. These materials were used to teach pediatric residents for 4 weeks. Knowledge-based tests were administered before the course, and re-tests were administered after the course using different test items of similar levels. Test scores and survey responses were compared before and after the intervention. Results: A total of 73 residents (1st year 20, 2nd year 23, 3rd year 16, and 4th year residents 14) from eight training hospitals completed the intervention. Their average age was 30.3±2.9 years, 17 (23.3%) were male, 22 (30.1%) were married, and eight had more than one child of their own. The mean pre-test score was 61.8±13.4 and the mean post-test score was 78.3±7.5 (P<0.001). The inter-grade difference in the score was significant in the pre-test (P=0.005), but not significant in the post-test (P=0.155). There were more responses of obtaining confidence after the intervention (P<0.001). Conclusion: In our study, pediatric residents showed improvement in their knowledge and confidence level after 4 weeks of the breastfeeding curriculum. Sang-Woo Kim, Jae-Young Choi, and co-workers report on p. 2187 the electrical and structural stability of the nanogenerators, with excellent charge scavenging performance under external mechanical loads such as bending and rolling.         Anti-allergic and anti-inflammatory effects of aqueous extract of Pogostemon cablin

        <P>Allergic disease is caused by exposure to normally innocuous substances that activate mast cells. Mast cell-mediated allergic inflammation is closely related to a number of allergic disorders, such as anaphylaxis, allergic rhinitis, asthma and atopic dermatitis. The discovery of drugs for treating allergic disease is an interesting subject and important to human health. The aim of the present study was to investigate the anti-allergic and anti-inflammatory effects of the aqueous extract of Pogostemon cablin (Blanco) Benth (AEPC) (a member of the Labiatae family) using mast cells, and also to determine its possible mechanisms of action. An intraperitoneal injection of compound 48/80 or a serial injection of immunoglobulin E and antigen was used to induce anaphylaxis in mice. We found that AEPC inhibited compound 48/80-induced systemic and immunoglobulin E-mediated cutaneous anaphylaxis in a dose-dependent manner. The release of histamine from mast cells was reduced by AEPC, and this suppressive effect was associated with the regulation of calcium influx. In addition, AEPC attenuated the phorbol 12-myristate 13-acetate plus calcium ionophore A23187 (PMACI)-stimulated expression of pro-inflammatory cytokines in mast cells. The inhibitory effects of AEPC on pro-inflammatory cytokines were dependent on the activation of nuclear factor (NF)-kappa B and p38 mitogen-activated protein kinase (MAPK). AEPC blocked the PMACI-induced translocation of NF-kappa B into the nucleus by hindering the degradation of I kappa B alpha and the phosphorylation of p38 MAPK. Our results thus indicate that AEPC inhibits mast cell-mediated allergic inflammation by suppressing mast cell degranulation and the expression of pro-inflammatory cytokines caused by reduced intracellular calcium levels and the activation of NF-kappa B and p38 MAPK.</P>

        <P>Hepatitis C virus (HCV) is a positive-stranded RNA virus that infects approximately 130–170 million people worldwide. In 2005, the first HCV infection system in cell culture was established using clone JFH-1, which was isolated from a Japanese patient with fulminant HCV infection. JFH-1 replicates efficiently in hepatoma cells and infectious virion particles are released into the culture supernatant. The development of cell culture-derived HCV (HCVcc) systems has allowed us to understand how hosts respond to HCV infection and how HCV evades host responses. Although the mechanisms underlying the different outcomes of HCV infection are not fully understood, innate immune responses seem to have a critical impact on the outcome of HCV infection, as demonstrated by the prognostic value of IFN-λ gene polymorphisms among patients with chronic HCV infection. Herein, we review recent research on interferon response in HCV infection, particularly studies using HCVcc infection systems.</P>

        <P>Tuberculosis (TB) is one of three major infectious diseases, and the control of TB is becoming more difficult because of the emergence of multidrug-resistant and extensively drug-resistant strains. In this study, we explored the <SUP>1</SUP>H NMR-based metabolomics of TB using an aerobic TB infection model. Global profiling was applied to characterize the responses of C57Bl/6 mice to an aerobic infection with virulent <I>Mycobacterium tuberculosis</I> (MTB). The metabolic changes in organs (i.e., the lung, the target organ of TB, and the spleen and liver, remote systemic organs) and in serum from control and MTB-infected rats were investigated to clarify the host–pathogen interactions in MTB-infected host systems. Principal components analysis (PCA) and orthogonal partial least-squares discriminant analysis (OPLS-DA) score plots showed distinct separation between control and MTB-infected rats for all tissue and serum samples. Several tissue and serum metabolites were changed in MTB-infected rats, as compared to control rats. The precursors of membrane phospholipids, phosphocholine, and phosphoethanolamine, as well as glycolysis, amino acid metabolism, nucleotide metabolism, and the antioxidative stress response were altered based on the presence of MTB infection. This study suggests that NMR-based global metabolite profiling of organ tissues and serum could provide insight into the metabolic changes in host infected aerobically with virulent <I>Mycobacterium tuberculosis</I>.</P><P>NMR-based metabolic profiling was used to characterize the responses of C57Bl/6 mice to an aerobic infection with virulent MTB.       • Measurement of Fractional Exhaled Nitric Oxide in Adults: Comparison of Two Different Analyzers

        Background Fractional exhaled nitric oxide (FeNO) is a recognized marker for eosinophilic airway inflammation and treatment decisions for responses to corticosteroids. Our study evaluated the differences between two FeNO analyzers and derived a correction equation for adults. Methods A total of 99 participants with the chief complaint of cough and dyspnea, who attended the outpatient clinics, were selected. They underwent concurrent FeNO measurement in a random order using NIOX VERO<sup>®</sup> (Circassia AB, Solna, Sweden) and NObreath<sup>®</sup> (Bedfont, Kent, UK). Agreement between the devices was evaluated by Wilcoxon tests, Spearman’s tests, and Bland-Altman plots. Results: Of the recruited 99 participants, they had a middle-aged female predominant feature (mean age: 51.2 ± 17.1 years and female 58.6%). Patients with asthma were found in 58.6 % of all participants. The median FENO level measured with the NIOX VERO<sup>®</sup> (27, interquartile range, 15-45) was significantly lower than that measured with NObreath<sup>®</sup> (38, 22-58, p < 0.001 ). There was a strong positive correlation in the measurement of FENO level between the devices (r= 0.787, p < 0.001). Bland- Altman plots confirmed this agreement. The following conversion equation was derived using linear regression: Ln (NObreath<sup>®</sup>) = 0.728 × Ln (NIOX VERO<sup>®</sup>) + 1.244. Conclusions The present study demonstrated that the values of two FeNO analyzers were in good agreement and had positive correlations. The correction equation that we provided could help assess the data obtained by these two analyzers.

      • Nano-patterned visible wavelength filter integrated with an image sensor exploiting a 90-nm CMOS process

        <P><B>Highlights</B></P><P>► We propose a highly efficient visible wavelength filter enabling a homogeneous integration with an image sensor, which can be fabricated by employing an advanced standard 90-nm CMOS process. ► We design and implement the proposed device incorporating a metallic subwavelength grating embedded in a CMOS image sensor. ► We analyze the characteristics of the visible wavelength filter by referring to the practical metal-grating structures.</P> <P><B>Abstract</B></P><P>A highly efficient visible wavelength filter enabling a homogeneous integration with an image sensor was proposed and manufactured by employing a standard 90-nm CMOS process. A one dimensional subwavelength Al grating overlaid with an oxide film was built on top of an image sensor to serve as a low-pass wavelength filter; a microlens was then formed atop the filter to achieve beam focusing. The structural parameters for the filter were: a grating pitch of 300nm, a grating height of 170nm, and a 150-nm thick oxide overlay. The overall transmission was observed to reach up to 80% in the visible band with a decent roll-off near ∼700nm. Finally, the discrepancy between the observed and calculated result was accounted for by appropriately modeling the implemented metallic grating structure, accompanying an undercut sidewall.</P>
