We describe a case of successful open-cell stent deployment across the wide neck of a large middle cerebral artery aneurysm using the stent anchor technique. A microcatheter was looped through the aneurysm and navigated into a distal vessel across the aneurysm neck. Although the loop of the microcatheter in the aneurysm straightened as it was gently withdrawn, the microcatheter again protruded into the aneurysm by open-cell stent navigation. The stent was partially deployed in a vessel distal to the aneurysm neck, withdrawn slowly to straighten the loop of the microcatheter in the aneurysm, and completely deployed across the aneurysm neck. After successful stent deployment, stent-assisted coil embolization was performed without complications. The stent anchor technique was successfully used to deploy an open-cell stent across the aneurysm neck in this case of microcatheter protrusion into the aneurysm during stent navigation.

Coronary-subclavian steal (CSS) can occur after coronary artery bypass grafting (CABG) using the internal thoracic artery (ITA). Subclavian artery (SA) stenosis proximal to the ITA graft causes CSS. We describe a technique for cardiac and cerebral protection during endovascular stenting for CSS due to right SA origin stenosis after CABG. A 64-year-old man with a history of CABG using the right ITA presented with exertional right arm claudication. Angiogram showed a CSS and retrograde blood flow in the right vertebral artery (VA) due to severe stenosis of the right SA origin. Endovascular treatment of the right SA stenosis was planned. For cardiac and cerebral protection, distal balloon protection by inflating a 5.2-F occlusion balloon catheter in the SA proximal to the origin of the right VA and ITA through the right brachial artery approach and distal filter protection of the right internal carotid artery (ICA) through the left femoral artery (FA) approach were performed. Endovascular stenting for SA stenosis from the right FA approach was performed under cardiac and cerebral protection by filter-protection of the ICA and balloon-protection of the VA and ITA. Successful treatment of SA severe stenosis was achieved with no complications.
Democracy and Social Discrimination in Japan

In this paper, the author would like to try to reveal the ethnic conflict and dismal human discrimination present in Japan. He would also attempt to disclose their causes in the historical context, and their efforts the Japanese have thus far made to solve such conflict and human discrimination by adopting and implementing governmental policies. Finally, he would like to make his tentative attempt to examine democracy in Japan or Japanese style democracy in relation to those in the advanced industrial societies.
Political Theory of Systemism from Japan

In this article, the author would like to show the readers a new political theory of systemism based upon a new world view after he worked on political theories and philosophies, political economies, and international relations in Japan for 40 years or so. During the period he had questioned or puzzled about an epistemological or ontological construction of basic ideas: atomism and holism in science, and individualism and collectivism in politics and philosophy. Such ideas in science and in politics and philosophy might be effective in the past history of our humanity; they are not effective in the present time to one narrow earth that inter-depends each entity evolved in a great network of living things. The view of systemism, not individualism or collectivism goes beyond the latter two. Systemism as a world view is sketched as the world inter-depending each entity and interrelating each humanity in society or each nation-state in a global community. This means that old political theories based on individualism and collectivism should change to a new political theory of systemism. He will try to examine such changing theories by answering about what systemism is, and what political theory of it is, as following Japanese historical experiences and tradition, and ideas of Mario Bunge and a few systems theorists.

Background: No studies have compared the outcomes of direct perineurial suture with vein graft cuff repair andindirect collagen allograft repair of the lingual nerve following an injury. Therefore, we evaluated and compared theoutcomes of each over a 1-year observation period. We retrospectively assessed 20 patients who had undergonemicroneurosurgical repair of unilateral lingual nerve injuries at the Wakayama Medical University Hospital betweenMay 2015 and March 2019. We utilized two different methods for lingual nerve repair, i.e., direct perineurial repairwith a vein graft cuff and interpositional collagen allograft repair. Sensory and taste function in the lingual nervewere preoperatively evaluated using a static two-point discrimination test, superficial pain/tactile sensation test,tests for the pressure pain threshold (Semmens-Weinstein monofilament), test for thermal discrimination hot andcold sensation, and a taste discrimination test. These tests were performed again at 6 and 12 months postoperatively. Results: Compared to the preoperative conditions, all patients showed improved sensory reactions. Functional sensoryrecovery outcomes were defined by Pogrel’s criteria, Medical Research Council Scale grades, and functional sensoryrecovery. In each group, all patients improved after the operation. However, the operation time was significantly shorterfor an interpositional collagen allograft repair as compared to that for a direct perineurial repair with a vein graft cuff. Conclusions: There were no statistically significant differences between the two repair Methods, except for the operationtime. Both methods led to satisfactory results for all criteria. From an economic point of view, direct perineurial repair witha vein graft cuff ismeaningful; however, the esthetic effect on the donor site should be considered. Conversely,interpositional collagen allograft repair has the advantage of a greatly shortened operation time.
Development of Corrosion Rates Estimation method for CUI using Information Gain Ratio

Almost all chemical and petrochemical plants in Japan have been operated for 30 years or more. Leakage accidents in piping often occur in such a kind of plants because of CUI (Corrosion Under Insulation) progress. In the worst case, inner fluid leakage might affect human bodies or the environment. Proper predictive maintenance work based of corrosion rate estimation should be performed to prevent the accidents. In the previous research, corrosion rates have been estimated based on databases constructed by cases of past corrosion. However, the method of previous research has some problems, such as the estimation accuracy is insufficient. In this research, a selection support system which can estimate corrosion rates with high accuracy using information gain ratio and reliability is developed.
Remote Monitoring System for Rescue Operations with Wireless Sensor Network

When accidents or large fires occur, rapid and appropriate rescue operations should be taken in the scene. It is very important for such operation that rescue team members constantly communicate with each other in the site. Moreover the base station located near the site should be aware of the team members’ positions and situations to direct them effectively. A remote monitoring system with ZigBee devices as a wireless communication method is developed for the rescue operations in disaster areas or special places outside the scope of mobile phones. The system can trace members’ actions and situations by using IMU and GPS, and help every member to communicate. It can also enhance the security and work efficiency of rescue teams’ performances at the scene.
