      • Ganglion Cell Neoplasm of the Neurohypophysis: A Rare Sellar Tumor With Pituitary Microadenoma

        Gangliocytomas are non-metastasizing and slow-growing tumor. In the sellar region, the sympathetic nerve fibers normally do not exist. Therefore, isolated gangliocytomas had been rarely reported in the pituitary fossa. Herein, we present and review the gangliocytoma arising in sellar region associating with pituitary adenoma which is the most common tumor in pituitary gland.

      • A Case of Carcinosarcoma ex Pleomorphic Adenoma

        Malignant mixed tumor is an aggressive malignant neoplasm composed of heterologous malignant epithelial and stromal components. It is a rare neoplasm, accounting for 0.05 % of all salivary gland tumors and 0.16 % of all malignant salivary gland tumors. The tumor may occur in the background of a preexisting pleomorphic adenoma or may arise de novo. We report on an 84-year-old female who was diagnosed with carcinosarcoma on her left parotid gland. Microscopically, the tumor consisted of two components - mainly myxoid chondrosarcoma (75%), poorly differentiated carcinoma and a few remnants of benign pleomorphic adenoma in the periphery of the tumor.

      • Angiofibroma with Low Cellularity of the Spermatic Cord

        Cellular angiofibroma is a benign mesenchymal tumor with uniform, bland, spindle cell differentiation accompanied by thick walled vessels. We report a 17-year-old male who was diagnosed cellular angiofibroma on his right spermatic cord. He had small (about 1cm sized), non-tender, painless, movable mass for 3 years but 1 year ago insidious size of mass increased. Histological finding showed features characteristic of cellular angiofibroma however that occurs almost exclusively in vulvar of middle aged women. In addition, benign mesenchymal neoplasms had similar histologic findings with angiofibroma, especially aggressive angiofibroblastoma, aggressive angiomyxoma and solitary fibrous tumor. Specific immunohistochemical stain profile of angiofibroma helpful of differential diagnosis. Local excision with clear resection margin and long-term follow up are the treatment of choice as local recurrence has been reported in a few case.

      • Predictive Factors affecting Central Lymph Node Metastasis in Papillary Thyroid Carcinoma

        The purpose of this study is to evaluate which clinicopathologic factors are associated with central lymph node metastasis (CLNM) in papillary thyroid carcinoma (PTC). A total of 402 cases of PTC were divided into two groups: CNLM (+) and CLNM (-). In univariate analysis, CLNM showed correlation with sex, tumor size, multifocality, bilaterality, extrathyroid extension, and distance from the capsule. In multivariate analysis, male gender, >1 cm of tumor size, multifocallity and extrathyroid extension were significant risk factors for CLNM. Findings of this study suggest that clinicians should pay attention to the above mentioned significant risk factors when examining lymph node (LN) for deciding LN dissection.

        Anesthesia guidelines for COVID-19 patients: a narrative review and appraisal

        The coronavirus disease 2019 (COVID-19) pandemic has challenged health systems globally and prompted the publication of several guidelines. The experiences of our international colleagues should be utilized to protect patients and healthcare workers. The primary aim of this article is to appraise national guidelines for the perioperative anesthetic management of patients with COVID-19 so that they can be enhanced for the management of any resurgence of the epidemic. PubMed and EMBASE databases were systematically searched for guidelines related to SARS-CoV and SARS-CoV-2. Additionally, the World Federation Society of Anesthesiologists COVID-19 resource webpage was searched for national guidelines; the search was expanded to include countries with a high incidence of SARS-CoV. The guidelines were evaluated using the Appraisal of Guidelines for Research and Evaluation II tool. Guidelines from Australia, Canada, China, India, Italy, South Africa, South Korea, Taiwan, the United Kingdom, and the United States of America were evaluated. All the guidelines focused predominantly on intubation and infection control. The scope and purpose of guidelines from China were the most comprehensive. The UK and South Africa provided the best clarity. Editorial independence, the rigor of development, and applicability scored poorly. Heterogeneity and gaps pertaining to preoperative screening, anesthesia technique, subspecialty anesthesia, and the lack of auditing of guidelines were identified. Evidence supporting the recommendations was weak. Early guidelines for the anesthetic management of COVID-19 patients lacked quality and a robust reporting framework. As new evidence emerges, national guidelines should be updated to enhance rigor, clarity, and applicability.

        The Relationship between Environmental Uncertainty and Opportunism in Marketing Channels: The Moderating Effect of Network Embeddedness

        Under environmental uncertainty, firms face opportunism of their exchange partners and have difficulty in expecting benevolence from their exchange partners because the participating firms are unable to predict changes in the external environments due to lack of information, knowledge and understanding. Meanwhile, firms’ economic actions and outcomes, like all social action and outcomes, are affected by the actors’ dyadic relations as well as by the structure of the overall relations, which is referred to as network embeddedness. Firms are embedded in the network by maintaining strong ties or weak ties with their exchange partners. As such, firms’ participating network embeddedness influences the inter-firm relationship under environmental uncertainty. This study hypothesizes that environmental uncertainty is positively related to opportunism and is negatively related to benevolence. This study also hypothesizes that these relations are moderated by the network embeddedness. The statistic results demonstrate that all of the hypotheses of this study turn out to be significant. The implication of this study is that network embeddedness acts as a moderator by way of managing opportunism when firms are under environmental uncertainty. Specifically, firms embedded in strong tie networks are unlikely to seek opportunism because firms’ participation in such strong ties serves as part of the control mechanism of environmental uncertainty. This study also portrays that firms’ participation in the network embeddedness promotes the benevolence of the exchange partners. Firms’ participation in strong tie networks enables the exchange partners to exchange fine-grained information and to deeply understand the operations of the partners. As such, firms have a belief that the exchange partners have the intentions and motives beneficial to the firms even in uncertain environments. Therefore, firms should carefully consider how they are embedded in the network with their exchange partners when they make a business decision under environmental uncertainty.

      • Ultrastructure of a Mobile Threadlike Tissue Floating in a Lymph Vessel

        <P>Observations of the primo vascular system (PVS) floating in lymph ducts were reported by various groups. There have been, however, no studies on the ultrastructure of the entire cross section of a primo vessel (PV) inside a lymph vessel with a transmission electron microscope (TEM). In the current study we took the TEM images of a cross section of the PV inside a lymph vessel. We used the Alcian blue staining method for the finding of the target PV in a lymphatic vessel by injecting the dye into the inguinal lymph nodes. The stained PV was harvested together with the lymph vessel and some parts of the specimens were used for studying with optical microscopes. Some other parts were treated according to a standard protocol for TEM. As the results the TEM study revealed the loosely distributed collagen fibers with plenty of empty spaces and the lumens with the endothelial nuclei. It turned out to be very similar to the ultrastructure of the PVs observed on the surfaces of internal organs. It also showed how compactly the PV is surrounded with lymphocytes. In conclusion, the detailed morphological features like the distribution of fibers in the PV were revealed and shown to be similar to another kind of the PV on the surfaces of internal organs.</P>

        The earliest timing of ultrasound in screening for developmental dysplasia of the hips

        Purpose: The current study aimed to evaluate the results of ultrasound screening for developmental dysplasia of the hips (DDH) done at various weeks of life, to determine the earliest time that ultrasound screening can be performed reliably. Methods: In this 17-year cohort study, all neonates who underwent ultrasound screening prior to the 12th week of life with subsequent follow-up radiography done at 1 year of life were included. The ultrasound images were evaluated according to the Graf classification, Harcke’s dynamic ultrasound screening method, and Terjesen’s femoral head coverage method. The radiographic images were evaluated according to the acetabular index and the femoral head position. The accuracy and correlation between the ultrasound findings from various weeks of life with the radiographic findings at 1 year of life were evaluated. Results: A total of 348 neonates were included in the study, of whom 92 had abnormal ultrasound findings and 42 had abnormal radiographic findings at 1 year. Significant differences were identified between the findings of ultrasound screening examinations performed prior to the fourth week of life (day 21 and before) and the radiographic findings at 1 year of life (P<0.05). In contrast, no significant differences were identified when ultrasound screening was performed between the fourth and 12th weeks of life (day 22 and beyond) (P>0.05). The accuracy of ultrasound screening was 79.2% or higher when performed during or after the fourth week of life (day 22 and beyond). Conclusion: The earliest that ultrasound screening for DDH can be performed reliably is during the fourth week of life (day 22 and beyond).
