      • Predictive Role of Neutrophil Gelatinase-Associated Lipocalin in Early Diagnosis of Platin-Induced Renal Injury

        Background: Acute kidney injury is an important issue in chemotherapy receiving patients an neutrophil gelatinase-associated lipocalin has been proposed as a novel marker. We here aimed to assess the role of urinary levels for assessment after platin exposure. Materials and Methods: Patients who had treated with cisplatin or carboplatin or oxaliplatin containg regimens were included in this study. Baseline and postchemotherapy serum urea, creatinine, urine neutrophil gelatinase-associated lipocalin and urine creatinine levels were determined. To avoid the effects of hydration during chemotherapy infusion the urinary neutrophil gelatinase-associated lipocalin/urine creatinine ratio was used to determine acute kidney injury. Results: Of a total of 42 patients receiving platin compounds,14 (33.3%) received cisplatin containing regimens, 14 (33.3%) received carboplatin and 14 (33.3%) oxaliplatin. The median age was 60 (37-76) years. Nineteen of the patients (45.2%) had lung cancer, 12 (28.6%) colorectal cancer and 11 (26.2%) others. The median pre and post chemotherapy urine neutrophil gelatinase-associated lipocalin/urine creatinin ratio was 15.6 ng/mg and 35.8 ng/mg (p=0.041) in the cisplatin group, 32.5 ng/mg and 86.3 ng/mg (p=0.004) in the carboplatin group and 40.9 ng/mg and 62.3 ng/mg (p=0.243) in the oxaliplatin group. Conclusions: Nephrotoxicity is a serious side effect of chemotherapeutic agentslike cisplatin and carbopaltin, but only to a lower extent oxaliplatin. All platin compounds must be used carefully and urine neutrophil gelatinase-associated lipocalin measurement seems to be promising in detecting acute kidney injury earlier than with creatinine.

      • Treatment and Outcomes of Ewing Sarcoma in Turkish Adults: A Single Centre Experience

        Background: Ewing sarcoma is a small round cell tumor arising from soft tissue and bone that predominantly affects children and adolescents. The most unfavorable prognostic factor is the presence of distant metastasis at the time of diagnosis. Materials and Methods: The records of 26 Ewing sarcoma patients (14 male, 12 female) were re-evaluated retrospectively. Results: The median age was 26.5 (19-42) years. Eight patients (31%) showed a primary tumor in their extremities, 8 (31%) in the thorax, 4 (15%) at the vertebra, 4 (15%) in the head and neck, and 2 (8%) in the abdomen. Five patients (19%) had distant metastasis at diagnosis. The median progression-free survival was 72 months and 10 months in localized and metastatic disease, respectively (p=0.005). The overall survival rate was 19 months in metastatic disease, and the 5-year overall survival rate was 64% in localized disease (p=0.006). Patients who had localized disease in the extremities and were under age 30 had a favorable prognosis. Conclusions: Although Ewing sarcoma is a tumor affecting children and adolescents, it may be seen in adults, where the prognosis is generally worse. Although it is a highly malignant tumor, it is possible to achieve improved survival with combined modality treatments.

      • Clinicopathologic Features and Prognosis of Osteosarcoma in Turkish Adults

        Background: Osteosarcomas are the most common solid malignancies of bone. In the last two decades there have been no concrete developments in their systemic treatment. In this trial we aimed to present our osteosarcoma patient clinical and demographic outcomes. Materials and Methods: Patients treated and followed up for osteosarcoma in Ankara Numune Education and Research Hospital from 2002 to 2012 were reviewed retrospectively. Results: A total of 21 patients (15 male, 6 female) were diagnosed with osteosarcoma. The disease was located at extremities in 76% and in 14% was metastatic at the time of diagnosis. Median disease free survival (DFS) was 36 months in non-metastatic patients and median progression free survival (PFS) was 2 months in metastatic patients (p<0.0001). Median overall survival (OS) was 80 months and 4 months, respectively (p=0.012). There were no survival differences in terms of presentation with pathological fracture, tumor size, tumor grade, alkaline phosphatase and lactate dehydrogenase level and type of chemotherapy regimen. Conclusions: Tumor site and stages are the most important prognostic factors for osteosarcoma. Extremity primary tumors have beter survival rates than non-extremity tumors. As a result of the use of effective chemotherapy the long term survival rates have improved from 10-20% to 60-70% in the last decades but we need more active agents, especially for metastatic cases.

        An Incidentally Detected Breast Cancer on Tc-99m MIBI Cardiac Scintigraphy

        Tc-99m methoxyisobutylisonitrile (MIBI) scintigraphy is generally used as a second-line diagnostic tool for obscured breast lesions. When the entire field of view is examined carefully, it is often possible to detect additional lesions unrelated to the initial intent and purpose of the examination. Herein we present a case of breast cancer incidentally detected by cardiac Tc-99m MIBI scintigraphy. An area of uptake was detected in the breast during a cardiac imaging test. Further evaluation of this lesion revealed a histopathological diagnosis of invasive ductal carcinoma of the breast. Sensitivity of this scintigraphic technique is not enough sufficient to use this test as a screening test for breast cancer, but it may provide supplemental information. Since it is not uncommon to find incidental lesions during imaging studies, examination of the image field may help clinicians find otherwise unrecognized or undiagnosed pathologies.

        Effect of atmospheric plasma versus conventional surface treatments on the adhesion capability between self-adhesive resin cement and titanium surface

        PURPOSE. The aim of this study was to evaluate the effects of atmospheric plasma (APL) versus conventional surface treatments on the adhesion of self-adhesive resin cement to Ti-6Al-4V alloy. MATERIALS AND METHODS. Sixty plates of machined titanium (Ti) discs were divided into five groups (n=12): 1) Untreated (CNT); 2) Sandblasted (SAB); 3) Tribochemically treated (ROC); 4) Tungsten CarbideBur (TCB); 5) APL treated (APL). SEM analysis and surface roughness (Ra) measurements were performed. Self-adhesive resin cement was bonded to the Ti surfaces and shear bond strength (SBS) tests, Ra and failure mode examinations were carried out. Data were analyzed by one-way analysis of variance and chi-squared test. RESULTS. The lowest SBS value was obtained with CNT and was significantly different from all other groups except for APL. The ROC showed the highest SBS and Ra values of all the groups. CONCLUSION. It was concluded that the effect of APL on SBS and Ra was not sufficient and it may not be a potential for promoting adhesion to titanium.

        The main object of this paper is to introduce and investigate new subclasses of normalized analytic functions in the open unit disc $\mathbb{U}$, which generalize the familiar class of k-starlike functions. The various properties and characteristics for functions belonging to these classes derived here include (for example) coefficient inequalities, distortion theorems involving fractional calculus, extreme points, integral operators and integral means inequalities.

        Effects of Fiber-rich Apple and Apricot Powders on Cookie Quality

        Apple and apricot powders (APL-P and APR-P) were produced from apple and apricot fruits and they were used in cookie formulation at the levels of 10-40% (in flour bases). The APL-P and APR-P were rich in terms of total dietary fiber (TDF) and antioxidant power. The APR-P supplemented cookies had higher spread ratio and lower hardness values than the APL-P supplemented ones at all addition levels. The color values of the APR-P supplemented cookies were all acceptable. Overall sensory scores of the cookies supplemented with APL-P and APR-P were not significantly different from the control up to 20% addition. TDF contents of the supplemented cookies increased significantly with increasing addition level (p<0.01). The replacement of flour by APL-P and APR-P in wire-cut cookies showed that the physical characteristics and textural properties of the cookies were significantly affected (p<0.01) and APR-P appeared to be a more suitable replacer of flour than APL-P. Addition of both fruit powders upto 20% into the cookie formulation were evaluated as acceptable in terms of the sensory properties.

        Cross section generation for a conceptual horizontal, compact high temperature gas reactor

        A macroscopic cross section generation model was developed for the conceptual horizontal, compact high temperature gas reactor (HC-HTGR). Because there are many sources of spectral effects in the design and analysis of the core, conventional LWR methods have limitations for accurate simulation of the HC-HTGR using a neutron diffusion core neutronics simulator. Several super-cell model configurations were investigated to consider the spectral effect of neighboring cells. A new history variable was introduced for the existing library format to more accurately account for the history effect from neighboring nodes and reactivity control drums. The macroscopic cross section library was validated through comparison with cross sections generated using full core Monte Carlo models and single cell cross section for both 3D core steady-state problems and 2D and 3D depletion problems. Core calculations were then performed with the AGREE HTR neutronics and thermal-fluid core simulator using super-cell cross sections. With the new history variable, the super-cell cross sections were in good agreement with the full core cross sections even for problems with significant spectrum change during fuel shuffling and depletion.

        Seismic Assessment of a Historical Masonry Mosque by Experimental Tests and Finite Element Analyses

        The mosques are the most important pieces of Islamic culture in terms of their architectural and structural properties. Although,thousands of these mosques are still being used all over the world, many historical mosques are at risk in terms of seismic events. This study mainly focuses on historical Lala Pasha Mosque, which is located in the city center of Erzurum, Turkey. The main purposeof this study is to assess the structural behaviors and to investigate the architectural features and components of the mosque. Thestudy was carried out in two main steps. Firstly, compression and three-point bending tests on the construction materials wereconducted to determine the mechanical properties of the materials. In the second step, a three-dimensional finite element model of themosque was developed and structural responses for the model were investigated by means of static and dynamic analyses. The resultsof the analyses show that the most critical parts of the mosque are the dome, pulley, and the supports of the main arch structures thatcarry the main dome. Additionally, the dynamic analyses prove that the most critical parts of the mosque are the sub-sections of themain columns, the window edges, and small domes.

      • Transient simulation of an endothermic chemical process facility coupled to a high temperature reactor: Model development and validation

        A high temperature reactor (HTR) is a candidate to drive high temperature water-splitting using process heat. While both high temperature nuclear reactors and hydrogen generation plants have high individual degrees of development, study of the coupled plant is lacking. Particularly absent are considerations of the transient behavior of the coupled plant, as well as studies of the safety of the overall plant. The aim of this document is to contribute knowledge to the effort of nuclear hydrogen generation. In particular, this study regards identification of safety issues in the coupled plant and the transient modeling of some leading candidates for implementation in the Nuclear Hydrogen Initiative (NHI). The Sulfur Iodine (SI) and Hybrid Sulfur (HyS) cycles are considered as candidate hydrogen generation schemes. Three thermodynamically derived chemical reaction chamber models are coupled to a well-known reference design of a high temperature nuclear reactor. These chemical reaction chamber models have several dimensions of validation, including detailed steady state flowsheets, integrated loop test data, and bench scale chemical kinetics. The models and coupling scheme are presented here, as well as a transient test case initiated within the chemical plant. The 50% feed flow failure within the chemical plant results in a slow loss-of-heat sink (LOHS) accident in the nuclear reactor. Due to the temperature feedback within the reactor core the nuclear reactor partially shuts down over 1500s. Two distinct regions are identified within the coupled plant response: (1) immediate LOHS due to the loss of the sulfuric acid decomposition section and (2) continuing slow LOHS due to the chemical species cascade throughout the plant.
