“Worth One Minute”: an anonymous rewarding platform for crowd-sensing systems
Lorenz Cuno Klopfenstein,Saverio Delpriori,Alessandro Aldini,Alessandro Bogliolo 한국통신학회 2019 Journal of communications and networks Vol.21 No.5
Readily available and affordable consumer-grade electronics, with ever-increasing sensing, computing, and communicationcapabilities, have provided the ground for distributed computationand data collection systems. Crowd-sensing applicationsrely on volunteers providing access to their personal devices—acategory encompassing smartphones, wearables, vehicles, and awide range of ‘Internet of Things’ appliances—and using themas sensors. These systems rely on the willingness of participantsto invest in a common cause, which often entails explicit effortsfrom users, occupation of hardware resources, and risks of sharingprivate data. Incentives and rewarding schemes are adoptedto encourage user participation. This paper introduces the “WorthOne Minute” (WOM) platform: an implementation of a generalpurposerewarding system based on anonymous vouchers. Theplatform is designed to reward user efforts towards the commongood, rewarding their contributions and the intrinsic social valuethey provide, while preserving their anonymity.

Intraductal tubulopapillary neoplasms (ITPNs) of the pancreas and biliary tract are rare pre-malignant entities of the biliary tract and pancreas that are difficult to diagnose preoperatively. While there are imaging characteristics that can differentiate these lesions from more common entities like adenocarcinoma or intraductal papillary mucinous neoplasms (IPMN), ITPNs are not always distinctive. Herein we present two cases of ITPN, one of biliary and the other of pancreatic origin, which had a preoperative diagnosis of cholangiocarcinoma and IPMN, respectively. We discuss our findings in these cases, patient presentation and course, review the radiographic and pathologic findings, and propose a more effective approach to the preoperative workup and diagnosis of ITPN based on our review of the contemporary literature.

Purpose: The surgical management of deep infiltrative endometriosis (DE) involving the rectum remains a challenge. The objective of this study was to assess the outcomes from a single tertiary center over a decade with an emphasis on the role of a protective loop ileostomy (PI).Methods: A retrospective review of outcomes for 168 patients managed between 2008 and 2018 is presented including 57 rectal shaves, 23 discoid excisions, and 88 segmental rectal resections. Results: The nodule size (mean±standard deviation) in the segmental resection group was 32.7±11.2 mm, 23.4±10.5 mm for discoid excision, and 18.8±6.0 mm for rectal shaves. A PI was performed in 19 elective cases (11.3%) usually for an ultra-low anastomosis <5 cm from the anal verge. All Clavien-Dindo grade III/IV complications occurred after segmental resections and included 5 anastomotic leaks, 6 rectovaginal fistulas, 2 ureteric fistulas, and 1 ureteric stenosis. Of 26 stomas (15.5%), there were 19 PIs, 3 secondary ileostomies (after complications), and 4 end colostomies. The median time to PI closure was 5.8 months (range, 0.4–16.7 months) in uncomplicated disease compared with 9.2 months (range, 4.7–18.4 months) when initial postoperative complications were recorded (P=0.019). Only 1 patient with a recurrent rectovaginal fistula had a permanent colostomy. Conclusion: In patients with DE and rectal involvement a PI is selectively used for low anastomoses and complex pelvic reconstructions. Protective stomas and those used in the definitive management of a major postoperative complication can usually be reversed.
Shielding analyses supporting the Lithium loop design and safety assessments in IFMIF-DONES
Gediminas Stankunas,Qiu Yuefeng,Nitti Francesco Saverio,Marugán Juan Carlos 한국원자력학회 2023 Nuclear Engineering and Technology Vol.55 No.4
The assessment of radiation fields in the lithium loop pipes and dump tank during the operation were performed for International Fusion Materials Irradiation Facility – DEMO-Oriented NEutron Source (IFMIF-DONES) in order to obtain the radiation dose-rate maps in the component surroundings. Variance reduction techniques such as weight window mesh (produced with the ADVANTG code) were applied to bring the statistical uncertainty down to a reasonable level. The biological dose was given in the study, and potential shielding optimization is suggested and more thoroughly evaluated. The MCNP Monte Carlo was used to simulate a gamma particle transport for radiation shielding purposes for the current Li Systems’ design. In addition, the shielding efficiency was identified for the Impurity Control System components and the dump tank. The analysis reported in this paper takes into account the radiation decay source from and activated corrosion products (ACPs), which is created by d-Li interaction. As a consequence, the radiation (resulting from ACPs and Be-7) shielding calculations have been carried out for safety considerations.
Quarantine and Appendicitis: A Macro-Area Experience
Nicola, Zampieri,Virginia, Murri,Mauro, Cinquetti,Amedeo, Elio,Saverio, Camoglio Francesco The Korean Society of Pediatric Gastroenterology 2021 Pediatric gastroenterology, hepatology & nutrition Vol.24 No.1
Purpose: Appendicitis is the most frequent urgency in pediatric age; the aim of this study was to investigate the association of quarantine for severe acute respiratory syndrome coronavirus 2 pandemic and the incidence of pediatric appendicitis in a specific macro area. Methods: We retrospectively analyzed the medical records of consecutive patients who underwent surgical exploration for acute appendicitis in the period March-April since 2014. This specific quarantine period was divided into two phases as indicated by National government. Patient data, demographics, characteristics and outcomes were studied and evaluated comparing patients treated during quarantine especially phase 1 vs. phase 2 (March-April 2020). Results: After reviewing medical charts following the inclusion and exclusion criteria, 155 patients were studied; focusing on the final outcome, it is possible to show a decreased amount of appendicitis during phase 1 and a progressive increase during phase 2; respect to previous years, there was a statistical increase in severity of appendicitis during quarantine (gangrenous vs. phlegmonous appendicitis). Conclusion: During this specific quarantine there was a reduction in appendicitis and a progressive increase during phase 2. These results offer new perspective among disease incidences during lockdown.
Hypertrophic Pyloric Stenosis: 10 Years' Experience with Standard Open and Laparoscopic Approach
Zampieri, Nicola,Corato, Valentina,Scire, Gabriella,Camoglio, Francesco Saverio The Korean Society of Pediatric Gastroenterology 2021 Pediatric gastroenterology, hepatology & nutrition Vol.24 No.3
Purpose: Hypertrophic pyloric stenosis (HPS) is the most common cause of gastric obstruction in newborns. Extra-mucosal pyloromyotomy can be performed through a small laparotomy or laparoscopy. The aim of this study was to compare the two surgical techniques. We also analyzed the incidence of HPS in infants in the last 10 years in relation to the demographic trend of our province. Methods: We analyzed all the cases of HPS treated at our Unit between January 2010 and December 2019. The data were obtained from operating systems. Data about the demographic trends, in particular, the number of births and the population residing in the province of Verona from 2010 to 2019, were also retrieved. Results: During the study period, 60 patients were treated for HPS and met the inclusion criteria. Of these, 56 males and 4 females with an average age of 38±14 days at surgery were included. No differences were found in terms of the duration of surgery, post-operative complications, duration of hospitalization, and weight at the time of surgery. The only statistically significant data was the chlorine level in cases with and without post-operative vomiting (97±3.5 vs. 102±3.3 mmol/L, p<0.05). There was a lower incidence of HPS from 2014 to 2019; however, there was no significant evidence regarding the correlation between this and the reduced birth rate recorded in the province of Verona during the same period. Conclusion: Although laparoscopic pyloromyotomy is a highly complex procedure, it is a feasible alternative to the classic open technique.

Carlos Augusto Gomes,Cleber Soares Junior,Salomone Di Saverio,Massimo Sartelli,Poliana Graciele de Souza Silv,Agnes Silva Orlandi,Thais Lacerda Heringer,Felipe Couto Gomes,Fausto Catena 한국간담췌외과학회 2019 Annals of hepato-biliary-pancreatic surgery Vol.23 No.1
Backgrounds/Aims: The prevalence and risk factors of gangrenous cholecystitis in male are unknown. Objective: To verify the prevalence and risk factors of gangrenous cholecystitis in males. Methods: This cross-sectional study includes 95 patients (59.5±17.1 years), with clinical and histopathological diagnosis, operated laparoscopically on 2012-2016. Eligibility was decided based on the variables of age; tachycardia (>100 bpm); leukocytosis (>10,000/mm³); Murphy’s sign; gallbladder wall thickness (>4 mm); biochemical tests, morbidities (diabetes, alcoholism, smoking) and mortality. Multivariate regression, the chi-squared and Prevalent Chances Ratio (PCR) were used to define a few parameters. Results: The prevalence of gangrenous cholecystitis in men older than 50 years was 29.3%. The risk factors for the disease were as follows: diabetes mellitus (p=0.006, RCP=4.191), leukocytosis (p=0.003), gallbladder thickness greater than 4 mm (p=0.035, RCP=3.818), which increased mortality [(p=0.04) (RCP=8.001)]. Murphy’s sign showed a negative association (p=0.002, RCP=0.204). Values close to significance were observed in relation to gamma glutamyl transferase (p=0.083, RCP=3.125) and hospital stay (p=0.061, RCP=2.765). Conclusions: Male gender, and age older than 50 years, were correlated with a high prevalence of necrosis, higher than that reported in females. The risk factors for developing necrosis are the same as those described for female patients.
