        『Kwangje-bikeup(廣濟秘?)』 is the medical volume that was written in 1790 just in a span of three months by Lee-kyunghwa under the auspices of Lee-byungmo, the then governor of Hamkyung province. The short time of three months is deemed the urgent need of medical book that dealt with famine and epidemic rampant around the northern provinces of Joseon Dynasty at the time. 『Kwangje-bikeup』 focuses on emergent diseases in such a way that the first-aid treatments of those was arranged in the first part, followed by the chapter of "miscellaneous diseases", "obstetrics and gynecology" and "pediatrics". This book is composed of the division four and postscript of the 『Kwangje-bikeup』, reconfigured into Korean vernacular language in order to maximize the comprehensibility and usefulness of first-aid medicines. Vernacular translation was meant for the easy access to medical treatments by grass-roots that were illiterate to abstruse Chinese. This was the exact same case with first-aid writings such as 『Eonhae-gugeupbang(諺解救急方)』 and 『Gugeup-kanyibang(救急簡易方諺解)』. One of the medical challenges in Joseon Dynasty was the shortage of medicines. Accordingly medicines easily found in Korean Peninsula were generally favored as a treatment, which was exemplifiedd as a unique feature in medical books mentioned above. Another feature of Korean medical writings was the manual-like explanation of treatment practices in medical writing rather than long and unintelligible ones of etiology and pathology, and the adoption of "one medical stuff for cure" policy. It represents the localization of imported medical knowledge and practices from outside. This book originated from such historical stream that was marked by the characteristics listed above.

        Comparison of Regional Myocardial Perfusion, Left Ventricular Volume and Ejection Fraction between 8- and 16-Frame Gated SPECT

        목적 : 게이트 심근관류 스펙트는 심근의 관류와 좌심실 기능을 동시에 평가할 수 있는 유용한 방법이다. 현재 게이트 영상 획득시 8 단위영상이 널리 사용되지만, 아직 가장 적절한 단위영상의 수에 대해서는 논의의 여지가 있다. 본 연구는 동일한 환자에서 8 단위영상과 16 단위영상으로 게이트 심근관류 스펙트를 2회 연속적으로 시행하여 국소적 심근관류, 좌심실 용적과 구혈률등을 비교하였다. 대상 및 방법: 42명(평균 연령: 55세, 남: 녀=26: 16)의 관상동맥 질환이 의심되거나 알려진 환자를 대상으로 안정시 Tc-99m MIBI를 이용한 게이트 스펙트를 8 단위영상과 16 단위영상으로 2회 연속하여 시행하였다. 좌심실의 확장기밀 용직, 수축기말 용적과 구혈률을 각각 얻었으며 확장기말의 단층영상을 18 분절로 나누고 각 분절마다 심근의 관류정도를 4등급으로 나누어 국소적 심근관류를 평가하였다. 결과: 8 단위영상과 16 단위영상 게이트 스펙트에서 국소적 심근관류의 일치도는 85.7% (tau-b=0.786, p>0.001)로 우수하였다. 두 방법간에 1등급의 차이는 100 분절(13.2%), 2등급 차이는 8 분절(1.1%)이 있었고 3등급 차이는 없었다. 좌심실의 확장기말 용적, 수축기말 용적, 구혈률은 두 방법간에 높은 상관성을 보였으며(r=0.993, 0.989, 0.935), 좌심실 구혈률은 8 단위영상을 사용할 때 평균 1.8% 낮게 측정되어 통계적인 유의성은 있었으나(p<0.05), rm 차이는 크지 않았다. 결론: 8 단위영상과 16 단위영상을 사용한 게이트 심근관류 스펙트에서 국소적 심근관류의 일치도는 우수하였으며, 좌심실의 기능적 계수의 차이도 크지 않았다. 비록 높은 단위영상을 사용했을 때 더 정확하고 자세한 정보를 얻을 수 있다 하더라도, 구혈률과 용적 등을 얻는 일상적인 임상검사에는 8 단위영상 또한 그 역할을 충분히 할 수 있을 것으로 사료된다.

        HSP25 Inhibits Protein Kinase Cδ-mediated Cell Death through Direct Interaction

        <P>Heat shock protein 25 (HSP25) interferes negatively with apoptosis through several pathways that involve its direct interaction with cytochrome c or Akt. Here we show that HSP25 inhibits protein kinase C (PKC) delta-mediated cell death through direct interaction. HSP25 binds to kinase-active PKCdelta to inhibit its kinase activity and translocation to the membrane, which results in reduced cell death. Deletion constructs of HSP25 and PKCdelta identified amino acids 90-103 of HSP25 and the C-terminal V5 region of PKCdelta as binding sites. In addition, the interaction between HSP25 and PKCdelta induced HSP25 phosphorylation at Ser-15 and Ser-86, and these phosphorylations permitted HSP25 release from PKCdelta. Based on these observations, we propose that after PKCdelta activation, HSP25 binds to the exposed V5 region of PKCdelta. This novel function of HSP25 accounts for its cytoprotective properties via the inhibition of PKCdelta and the enhancement of HSP25 phosphorylation.</P>

      • Next-generation sequencing reveals novel resistance mechanisms and molecular heterogeneity in EGFR-mutant non-small cell lung cancer with acquired resistance to EGFR-TKIs

        <P><B>Abstract</B></P> <P><B>Objectives</B></P> <P>Despite initial responses to epidermal growth factor receptor tyrosine kinase inhibitors (EGFR-TKIs) in EGFR mutant non-small cell lung cancer, patients invariably develop acquired resistance. In this study, we performed next-generation sequencing in pre- and post-EGFR-TKI tumor samples to identify novel resistance mechanisms to EGFR-TKIs.</P> <P><B>Material and methods</B></P> <P>We collected tumor tissues before EGFR-TKI treatment and after progression from 19 NSCLC patients to analyze genomic alterations in 409 cancer related genes. Bioinformatics analyses were used to identify mutations in which the allele frequencies are significantly changed, or newly appeared after progression.</P> <P><B>Results</B></P> <P>Overall, mutation rates and compositions were similar between pre- and post-EGFR-TKI tumors. We identified EGFR T790M as the most common mechanism of acquired resistance (63.2%). No pre-EGFR-TKI tumor had a preexisting T790M mutation, suggesting that tumors acquired T790M mutations following progression on EGFR-TKIs. Compared to T790M-positive tumors, T790M-negative tumors showed relatively high tumor mutation burden and shorter survival, suggesting T790M-negative patients as a potential candidate for immune checkpoint inhibitors. TP53 mutation was also significantly enriched in the T790M-negative tumors. Finally, we described here for the first time a novel missense mutation (T263P), which occurred concurrently with an activating G719A mutation, in the extracellular domain II of EGFR in a patient with poor response to erlotinib. Ba/F3 cells harboring EGFR T263P/G719A mutation showed higher sensitivity to afatinib, compared to gefitinib due to inhibition of EGFR/HER2 heterodimerization.</P> <P><B>Conclusion</B></P> <P>Comprehensive genomic analysis of post-EGFR-TKI tumors can provide novel insight into the complex molecular mechanisms of acquired resistance to EGFR-TKIs.</P> <P><B>Highlights</B></P> <P> <UL> <LI> NGS in paired NSCLC before and after EGFR-TKIs was performed. </LI> <LI> EGFR-TKI resistant T790M (+) NSCLC patients had significantly longer survival. </LI> <LI> Hypermutation/TP53 mutation seem enriched in EGFR-TKI resistant T790M (−) patients. </LI> <LI> Tumor mutation burden seem to be higher in EGFR-TKI resistant T790M (−) patients. </LI> <LI> T263P of EGFR was identified as the de novo mechanism of resistance to EGFR-TKIs. </LI> </UL> </P>

        Cloning metallothionein gene in Zacco platypus and its potential as an exposure biomarker against cadmium.

        <P>Zacco platypus, pale chub, is an indigenous freshwater fish of East Asia including Korea and has many useful characteristics as indicator species for water pollution. While utility of Z. platypus as an experimental species has been recognized, genetic-level information is very limited and warrants extensive research. Metallothionein (MT) is widely used and well-known biomarker for heavy metal exposure in many experimental species. In the present study, we cloned MT in Z. platypus and evaluated its utility as a biomarker for metal exposure. For this purpose, we sequenced complete complementary DNA (cDNA) of MT in Z. platypus and carried out phylogenetic analysis with its sequences. The transcription-level responses of MT gene following the exposure to CdCl2 were also assessed to validate the utility of this gene as an exposure biomarker. Analysis of cDNA sequence of MT gene demonstrated high conformity with those of other fish. MT messenger RNA (mRNA) expression and enzymatic MT content significantly increased following CdCl2 exposure in a concentration-dependent manner. The level of CdCl2 that resulted in significant MT changes in Z. platypus was within the range that was reported from other fish. The MT gene of Z. platypus sequenced in the present study can be used as a useful biomarker for heavy metal exposure in the aquatic environment of Korea and other countries where this freshwater fish species represents the ecosystem.</P>

      • HSP25 inhibits radiation-induced apoptosis through reduction of PKCδ-mediated ROS production

        Since radiation-induced caspase-dependent apoptosis and ROS generation were partially prevented by HSP25 overexpression, similar to the treatment of control cells with antioxidant agents such as DPI and tiron, questions arise whether radiation-mediated ROS generation contributes to the apoptotic cell death, and also whether HSP25 overexpression can reduce ROS mediated apoptotic cell death. In the present study, radiation-induced cytochrome c release from mitochondria and activation of caspases accompanied by a decrease of mitochondrial membrane potential in Jurkat T cells were shown to be inhibited by mitochondrial complex I inhibitor rotenone, suggesting that mitochondrial ROS might be important in radiation-induced caspase-dependent apoptosis. When HSP25 was overexpressed, effects similar to the treatment of cells with the antioxidants were obtained, indicating that HSP25 suppressed radiation-induced mitochondrial alteration that resulted in apoptosis. Furthermore, activation of p38 MAP kinase by radiation was associated with radiation-induced cell death and ROS production and PKCδ was an upstream molecule for p38 MAP kinase activation, ROS generation and subsequent caspase-dependent apoptotic events. However, in the HSP25 overexpressed cells, the above-described effects were blocked. In fact, radiation-induced membrane translocation of PKCδ and tyrosine phosphorylation were inhibited by HSP25.       • Targeted Panel Exome Sequencing in Suspected Monogenic Diabetes: Single-Center Pilot Study

        Purpose:Maturity onset diabetes of the young (MODY) is the most common hereditary form of diabetes mellitus (DM), with similar clinical manifestations to type 1 or type 2 DM, leading to diagnostic ambiguity. Despite increased genetic research on monogenic DM, studies with Asian populations are limited. Therefore, we investigated mutation in possible monogenic DM and MODY in Korean children and aldolescents. Methods: Targeted panel exome sequencing including 32 targets genes was performed for 41 patients with suspected monogenic DM at Kyungpook National University Children's Hospital. Results: Variants were detected in 19 patients, including those in known MODY-associated genes (HNF4A, GCK, HNF1A, CEL, PAX4, INS, and BLK) and monogenic DM-associated genes (WFS1, FRX6, and GLIS3). Conclusion: MODY variants were detected more than expected. Targeted exon sequencing is helpful in diagnosing MODY or possible monogenic DM patients.

        In recent years, understanding the dynamics of DC distribution system has become critically important due mainly to the increasing needs for the interconnection of DC distributed generators and the (DC-based) electric vehicle (EV) charging systems. In this paper, the characteristics of the DC grid system connected to the compact proton exchange membrane fuel cell (PEMFC) has been studied. In particular, the voltage and current transient phenomena were measured by varying the load of the DC grid system. Also, the voltage and current ripple were measured at the different load conditions. Our experimental results clearly manifested that the study contributes to the establishment of fundamental method to characterize the small DC grid system including distributed generation.
