Purpose: We evaluated the impact of benign prostatic hyperplasia (BPH) and prostatitis on the risk of prostate cancer (PCa) in patients using nationally representative data of the Korean population from the National Health Insurance Service. Materials and Methods: A total of 5,580,495 Korean men, aged >20 years, who had undergone health screening in 2009 were followed-up for 9 years until 2017. Multivariate adjusted Cox regression analysis was conducted to determine the hazard ratio (HR) and 95% confidence interval (CI) for the association between BPH and/or prostatitis and PCa. The HR for PCa according to the presence of BPH and/or prostatitis was stratified by a combination of BPH and prostatitis in multivariableadjusted models. Results: The HR for PCa significantly increased in patients with BPH and prostatitis than in patients without BPH and prostatitis (adjusted HR, 1.626; 95% CI, 1.567–1.688 and adjusted HR, 1.557; 95% CI, 1.500–1.618, respectively). In particular, for the combination of BPH and prostatitis, the adjusted HR was 1.856 (95% CI, 1.743–1.976), which was the highest when a diagnosis of both BPH and prostatitis was made. Conclusions: BPH and/or prostatitis are associated with an increased incidence for PCa in Korean patients, which is likely associated with similar effects to prostate-specific antigen (PSA) screening, so care must be taken in the interpretation. However, if follow-up survival studies demonstrate differences between the two groups (BPH and prostatitis vs. general), it could be one of the evidence for the introduction of PSA screening in Korea.

유도전동기 벡터제어의 특성 개선을 위하여 벡터제어의 문제점을 개선할수 있는 저항보상 알고리즘을 적용해 보았다. 이 알고리즘에서 저항변동의 추정 및 보상은 유도전동기의 단자전압과 전류를 바탕으로 동기좌표계에서 저항변동분을 추정하고 그 처정값을 간접벡터제어 슬립계산기의 이득값과 전향보상항에 포함된 고정자 저항값을 보상하는 방법을 이용하였다. 그리고 지령속도에 대한 추정속도의 응답시간지연 및 과도상태동안의 토오크의 맥동현상은 적옹한 알고리즘을 실해함으로 해서 응답시간지연을 줄이고 토오크의 맥동현상이 개선될수 있음을 보일 수 있었다.

Polymer Electrolyte Membrane fuel cell (PEMFCs) are regarded as an attractive alternative to replace the existing automotive power sources. To guarantee high performance and to achieve the improved durability of PEMFC system, novel water management is essential. To this end, a comprehensive understanding of the dynamic behavior of liquid water inside the operating PEMFC plays an significant role. In this work, direct visualization of water droplet behavior in the ex situ unit flow channel of PEMFC including gas diffusion layer (GDL) is carried out as one of the basic water management studies. Water droplet dynamics such as the movement and growth of liquid water droplet are mainly presented and discussed. The data obtained as a result of this study can contribute to set up the fundamental strategy including efficient water removal from automotive PEMFC stack.
Lifestyle Risk Prediction Model for Prostate Cancer in a Korean Population

<P><B>Purpose</B></P><P>The use of prostate-specific antigen as a biomarker for prostate cancer (PC) has been controversial and is, therefore, not used by many countries in their national health screening programs. The biological characteristics of PC in East Asians including Koreans and Japanese are different from those in the Western populations. Potential lifestyle risk factors for PC were evaluated with the aim of developing a risk prediction model.</P><P><B>Materials and Methods</B></P><P>A total of 1,179,172 Korean men who were cancer free from 1996 to 1997, had taken a physical examination, and completed a lifestyle questionnaire, were enrolled in our study to predict their risk for PC for the next eight years, using the Cox proportional hazards model. The model’s performance was evaluated using the C-statistic and Hosmer‒Lemeshow type chi-square statistics.</P><P><B>Results</B></P><P>The risk prediction model studied age, height, body mass index, glucose levels, family history of cancer, the frequency of meat consumption, alcohol consumption, smoking status, and physical activity, which were all significant risk factors in a univariate analysis. The model performed very well (C statistic, 0.887; 95% confidence interval, 0.879 to 0.895) and estimated an elevated PC risk in patients who did not consume alcohol or smoke, compared to heavy alcohol consumers (hazard ratio [HR], 0.78) and current smokers (HR, 0.73) (p < 0.001).</P><P><B>Conclusion</B></P><P>This model can be used for identifying Korean and other East Asian men who are at a high risk for developing PC, as well as for cancer screening and developing preventive health strategies.</P>

<P><B>Purpose</B></P><P> This study aimed to evaluate the statuses of P16INK4A expression and human papillomavirus (HPV) infection among patients with penile cancer at a single Korean institution.</P><P><B> Patients and Methods</B></P><P> Fourteen patients with penile cancer at our center were retrospectively identified and their clinicopathological data were analyzed. The patients' HPV and P16INK4A expression status (a known tumor suppressor protein) were tested using genotyping with a DNA chip assay and immunohistochemical staining, respectively. The results regarding HPV status were compared to those from another Asian study.</P><P><B> Results</B></P><P> The mean age at diagnosis was 60 years (range: 34–86 years). The median tumor size was 3.0 cm (range: 0.6–4.7 cm). Ten tumors were located on the penile glans. Five patients tested positive for HPV DNA (5/14, 36%) and all cases involved HPV type 16 (5/5, 100%). Positive expression of P16INK4A was observed in 6 cases (6/14, 43%). Among the HPV positive cases, 80% of cases (4/5) were also positive for P16INK4A. The prevalence of HPV infection in our study (36%) was higher than in a previous Asian study (23%).</P><P><B> Conclusions</B></P><P> This is the first study to evaluate the prevalence of HPV infection and P16INK4A expression among patients with penile cancer at a single Korean institution. The prevalence of HPV (36%) was slightly higher than the results from a previous Asian study. Additional multi-center studies are needed to better understand penile cancer in Korea and to identify biomarkers that can determine high-risk cases and predict their prognosis.</P>
A Case of Pleural Paragonimiasis Confused with Tuberculous Pleurisy

Here, we report a case of pleural paragonimiasis that was confused with tuberculous pleurisy. A 38-year-old man complained of a mild febrile sensation and pleuritic chest pain. Radiologic findings showed right pleural effusion with pleural thickening and subpleural consolidation. Adenosine deaminase (ADA) activity in the pleural effusion was elevated (85.3 IU/L), whereas other examinations for tuberculosis were negative. At this time, the patient started empirical anti-tuberculous treatment. Despite 2 months of treatment, the pleural effusion persisted, and video-assisted thoracoscopic surgery was performed. Finally, the patient was diagnosed with pleural paragonimiasis based on the pathologic findings of chronic granulomatous inflammation containing Paragonimus eggs. This case suggested that pleural paragonimiasis should be considered when pleural effusion and elevated ADA levels are observed.

<P><B>Abstract</B></P> <P><B>Background</B></P> <P>In the present study we examined the effect of the Prognostic Nutritional Index (PNI) on the overall survival (OS) and progression-free survival (PFS) of patients with metastatic renal cell carcinoma (RCC) treated with targeted therapy.</P> <P><B>Patients and Methods</B></P> <P>The study included 125 patients with metastatic RCC. Pretreatment PNI was calculated as 10 × serum albumin (g/dL) + 0.005 × total lymphocyte count (per mm<SUP>3</SUP>). Univariable and multivariable Cox proportional hazards models were used to assess the correlation between pretreatment PNI and OS and PFS. Harrell concordance index was used to measure discrimination.</P> <P><B>Results</B></P> <P>The median follow-up time was 45.3 months (interquartile range, 23.7-77.3 months). Decreased PNI was significantly associated with older female patients, poor Eastern Cooperative Oncology Group performance status, types of initial drug, and increased Memorial Sloan Kettering Cancer Center (MSKCC) and Heng risk score (<I>P</I> < .05). An increase in the PNI of 1 unit was associated with a 10% decrease in the risk of death from RCC (hazard ratio, 0.90; <I>P</I> < .001). In the multivariable analysis, the PNI was an independent prognostic factor for OS (<I>P</I> < .001). In intermediate-risk patients according to MSKCC and Heng risk criteria, OS was better in the high PNI group than in the low PNI group (<I>P</I> = .0136 and <I>P</I> = .0009, respectively).</P> <P><B>Conclusion</B></P> <P>PNI is an independent prognostic factor in patients with metastatic RCC treated with targeted therapy. When used as an adjunct, it increases the prognostic accuracy of established factors and could be a valuable tool for tailoring surveillance, patient counseling, and clinical trial design.</P>

국내 대부분의 교량구조물이 노후화 되어 시설물의 유지관리의 중요성이 증가하고 있다. 국내 유지관리는 방대한 양의 구조물 안전진단 이력데이터의 관리가 어렵고, 체계적인 평가기준의 부재로 객관적이고 합리적인 유지관리를 할 수 없는 문제점을 가지고 있다. BIM(Building Information System)을 유지관리에 적용할 경우 이력데이터의 효율적인 관리 및 정량적인 평가를 할 수 있으나, BIM 도입 시 기술적인 문제점과 BIM 적용을 위한 가이드라인의 부족으로 활용사례가 없는 실정이다. 이에 본 연구에서는 국내 유지관리에 BIM 적용을 위한 가이드라인의 정립에 필요한 구체적이고 객관적인 유지관리 항목들을 도출하기 위하여 국내⋅외 시설물 유지관리 매뉴얼의 유지관리 항목들을 비교 분석하여 제시하였다. Most of structures in Korea have reached the end of their lifespan making the maintenance of these facilities even more vital. However, domestic maintenance is faced with problems such as dealing with amount of historical data and non-objective assessment and unused historical data. When BIM (Building Information System) is applied to maintenance, efficient management evaluation of historical data can be done. But there is no precedent of its application due to seldom guidelines and lack of technical problems for applying BIM. Therefore, in this study to Comparison and analysis have been done on maintenance manuals to derive maintenance provisions for development guidelines on BIM application.
