        규칙적인 운동이 제1형 당뇨병 및 비만 환아의 아디포카인과 염증반응에 미치는 영향

        본 연구의 목적은 유산소 운동이 비만과 제1형 당뇨병 어린이들에게 adipokines과 염증반응에 미치는 영향을 알아보고자 실시되었다. 피험자들은 비만군(OG, n=9), 제1형 당뇨병군(DG, n=9), 그리고 정상군(NG, n=9) 어린이로 연구하였다. 모든 인자는 체중, 체지방률, 체질량지수(BMI), 최대산소섭취량(㎖/㎏/min), 지질 인자와 adipokines를 분석하였다. 그 결과 체중, 체지방률, BMI, 최대산소섭취량에서 OG에 비해 DG와 NG는 유의한 차이가 나타났다(p<0.05). LDL-C와 HDL-C 역시 OG에 비해 DG와 NG에서 유의한 차이가 나타났다(p<0.05). 게다가 adiponectin과 retinol binding protein (RBP)-4는 12주 운동 후에 DG와 NG에서 유의한 변화가 나타났으며 (p<0.05), OG에 비해 DG와 NG에서 유의한 차이가 나타났다(p<0.05). Monocyte chemoattractant protein (MCP)-1에서 모든 그룹은 12주 운동후 유의하게 증가하였다(p<0.05). 결론적으로 규칙적인 운동은 비만과 제1형 당뇨병 어린이에게 신체조성의 변화는 가져오지 못했으나 제1형 당뇨병 어린이의 혈관 염증을 줄여줄 수 있으며, 비만어린이에서는 대사 증후군을 보호할 수 있을 것으로 사료된다. The purpose of this study was to investigate the effects of aerobic exercise on adipokines and inflammatory reaction in obesity and type 1 diabetes mellitus (T1DM) children. We studied obese (OG, n=9), type 1 diabetic (DG, n=9), and normal (NG, n=9) children groups. Measurement factors included body weight, % fat, body mass index (BMI), VO2max, lipid profiles and adipokines. The results showed significant differences in body weight, % fat, BMI, and VO2max (㎖/㎏/min) among the OG, DG, and NG (p<0.05) groups. There were significant differences in LDL-C and HDL-C between the OG, DG, and NG groups (p<0.05). In addition, adiponectin and retinol binding protein (RBP)-4 were significantly changed in DG and NG after 12 weeks exercise training (p<0.05), and there were also significant differences among the OG, DG, and NG groups (p<0.05). Monocyte chemoattractant protein (MCP)-1 in the OG, DG, and NG groups was significantly increased after 12 weeks exercise training (p<0.05). In conclusion, regulatory aerobic exercise does not change body composition in obese children with T1DM, but exercise and decreased blood inflammatory factors in T1DM may protect obese children from metabolic syndrome.

        당뇨병 환자의 동기 유형과 불안의 관계

        목적: 스포츠 건강 심리학 관점에서 건강에 영향을 미치는 심리적 요인과 건강 행동을 증진하는 방법을 탐구하기 위한 연구가 필요하다. 이 연구는 당뇨병 질환으로 인한 건강 관리와 유지를 위해 규칙적인 운동 참여가 필수적인 당뇨병 환자들의 동기 유형을 파악하고, 동기 유형과 불안과의 관계를 규명했다. 방법: 당뇨병으로 진단받은 255명의 환자를 대상으로 동기 유형과 불안을 측정하고, 이들 변인 간의 관계를 분석했다. 부가적으로 불안 수준과 성별에 따라 동기 유형의 차이를 확인했다. 결과: 연구결과, 확인규제는 불안에 부적 영향을 미쳤으며, 외적규제는 불안에 정적 영향을 미쳤다. 차이검증에서도 통합규제와 확인규제가 높은 당뇨병 환자들은 불안 수준이 낮은 것으로 밝혀졌으며, 외적규제가 높은 당뇨병 환자들은 불안 수준이 높은 것으로 나타났다. 이는 운동의 중요성을 인식하는 동기를 갖고 규칙적인 운동에 참여할 때 불안이 낮아진다는 것을 의미한다. 반면에 외적 보상, 죄책감이나 처벌을 피하려는 동기를 갖고 운동에 참여할 때는 불안이 높아진다는 것을 보여준다. 결론: 이러한 연구결과는 당뇨병 환자들이 불안 수준을 낮추기 위해서는 개인이 지닌 동기 유형이 중요한 역할을 한다는 것을 보여준다. 특히, 통합규제와 확인규제를 지니고 운동에 참여하는 것이 중요하다는 것을 보여준다. 즉, 자신이 원하고 운동의 중요성을 인식하고 자발적으로 운동에 참여하는 것이 심리적 건강에 이롭다는 것을 시사한다. 반대로 가족이나 의사의 지시나 질책으로 운동에 참여하는 것은 심리적 건강에 해롭게 할 수 있다는 것을 예측할 수 있다. 결론적으로 이 연구는 당뇨병 환자들의 운동 상담 및 운동 프로그램을 구성하는 데 있어서 환자들의 동기 특성을 반영하고, 이를 계획하는데 유용한 기초 자료가 될 것이다. Purpose: Health psychology approach is needed to explore psychological factors affecting health and ways to promote healthy behavior in sports setting. The present study was identified the motivation types in the patients with diabetes mellitus, which is essential for regular exercise participation for health management and maintenance due to diabetes mellitus, and the relationship between motivation types and anxiety. Methods: In this study 255 patients diagnosed with diabetes mellitus, the types of motivation and anxiety were measured, and the relationship between these variables was analyzed. Additionally, the groups difference in motivation type according to the level of anxiety and gender was confirmed. Results: As a result of the analysis, identified regulation had a negative effect on anxiety, and external regulation had a positive effect on anxiety. In the groups difference, it was found that diabetic patients with high integrated regulation and identified regulation had a low level of anxiety, and diabetics with high external regulation had high anxiety levels. This results show that recognizing the importance of exercise and engaging in regular exercise lowers anxiety. On the other hand, it shows that anxiety increases when participating in a movement because of external rewards or to avoid punishment. Conclusion: These findings suggested that it is important for diabetics to participate in exercise with integrated and identified regulations in order to reduce anxiety levels. It suggests also that it is beneficial to patients of psychological health if they want to, be aware of the importance of exercise, and voluntarily participate in exercise. Conversely, it can be predicted that participating in exercise under the direction or reprimand of a family or doctor is harmful to psychological health. In addition, the results of this study will be useful basic data to reflect and plan the motivational characteristics of patients in promotion exercise counseling and exercise programs for diabetic patients.

        당뇨 전 단계 고령자의 규칙적인 저항성 운동이 뇌신경가소성 인자에 미치는 영향

        PURPOSE: The purpose of this study was to determine whether regulatory resistance exercise is as effective as aerobic training at improving the blood levels of brain-derived neurotrophic factor (BDNF), and nerve growth factors (NGF) as well as Cathepsin B, which are major factors contributing to the regulation of glucose metabolism and neuroplasticity, in prediabetic elderly. METHODS: Among the elderly aged 65 years or older who attended the B welfare center in C city, 16 patients who had no disease and had HbA1c of 5.7-6.4% were randomly assigned into either control group (CO, n=8, age 69.12±2.07 years) and resistance exercise group (RE, n=8, 71.25±1.69 years). Both groups were exercised three times a week for 12 weeks with the exercise intensity of 200- 250 kcal consumed Body composition and basic physical fitness tests were measured at baseline and at week 12. Blood levels of BDNF, NGF and Cathepsin B were also analyzed by using glucose metabolism factor and enzyme immunoassay. RESULTS: As a result, there were no significant differences in all body composition in both CO and RE. In addition, there were no statistically significant differences in all variables of the glycolysis factor. The blood levels of BDNF and NGF were significantly increased after the 12 weeks exercise program in the RE compared with the CO (p<.05). CONCLUSIONS: Based on these results, regular resistance exercise seems to be effective in increasing the brain neuroplasticity factors BDNF and NGF in prediabetic elderly. Depending on the prediabetes stage or the period of stay in the diabetic stage, further study is needed after considering the exercise period and intensity.

      • Correlation between basic physical fitness and pulmonary function in Korean children and adolescents: a cross-sectional survey

        <P>[Purpose] The purpose of the present study was to determine whether there was a correlation between basic physical fitness and pulmonary function in Korean school students, to present an alternative method for improving their pulmonary function. [Subjects and Methods] Two hundred forty healthy students aged 6–17 years performed physical fitness tests of hand-grip strength, sit and reach, Sargent jump, single leg stance, and pulmonary function tests of forced vital capacity (FVC) and forced expiratory volume in one second (FEV1) using a Quark PFT. [Results] Muscle strength and power of boys improved in the late period of elementary school and middle school. Muscle strength of girls improved in the late period of elementary school. Analysis of factors affecting pulmonary function revealed that height, weight, BMI, and body fat significantly correlated with spirometric parameters. Right hand-grip strength, left hand-grip strength, and Sargent jump also significantly correlated with FVC and FEV1. [Conclusion] In order to improve the pulmonary function of children and adolescents, aerobic exercise and an exercise program to increase muscle strength and power is needed, and it should start in the late period of elementary school when muscle strength and power are rapidly increasing.</P>

        고지방 식이 섭취로 유발된 비만 쥐의 시상하부 BDNF발현과 렙틴 mRNA 발현, 혈청 렙틴과 항비만과의 관계에 대한 운동트레이닝의 효과

        본 연구의 목적은 고지방식으로 유도된 비만이 에너지 항상성 조절인자인 신경영양인자와 섭식조절 호르몬인 렙틴 mRNA 발현에 어떠한 관련성이 있는지를 규명하는 것과 비만 쥐의 시상하부 BDNF 발현과 렙틴 리셉터 mRNA 발현, 혈청 렙틴과 항비만과의 관계에 대한 운동트레이닝과 정상식이로의 전환에 대한 효과를 확인하고자 하였다. 그 결과 8주 후 비만 대조군(HF)에 비해 운동군(HFT), 정상식이군(HFND), 운동 및 정상식이 트레이닝군(HFNDT)에서 각각 유의한 체중 및 혈중 TC 감소를 보였다(P<0.05). 혈중 TG에서는 HF군에 비해 HFNDT군에서 유의한 감소를 보였다(P<0.05). 혈중 렙틴은 HF군에 비해 HFT군에서 유의한 감소를(P<0.05), HFT군에 비해 HFND군에서 유의한 감소를 보였으며(P<0.05), HFND군에 비해 HFNDT군에서 유의한 감소를 나타냈다(P<0.05). 시상하부 BDNF 단백질 발현은 HF군에 비해 모든 군에서 유의한 증가를 보였다(P<0.05). 시상하부 렙틴 리셉터 mRNA는 HF군에 비해 HFT와 HFND군에서 약간의 감소를, HFNDT군에서는 증가를 나타내었다. 이상의 결과는 고지방식이로 유도된 비만쥐에 대한 운동트레이닝은 항비만 효과가 있는 것으로 나타났으며, 운동 또는 정상식이의 병행은 식욕과 체중조절에 최적의 비만예방 및 개선효과가 있을 것으로 사료된다. The purpose of this study is to examine how to relate with hypothalamus protein BDNF and mRNA leptin expression, and test the effect of exercise training upon anti-obesity in high-fat induced obese rats. Weight and plasma TC of the high-fat diet group (HF) significantly reduced in comparison to those in the high-fat diet and training group (HF-T), high-fat diet and normal diet group (HF-ND), and high-fat diet, training, and normal diet group (HF-ND+T) (P<0.05). Plasm TG of the HF group significantly decreased in comparison to the HF-ND+T group (P<0.05). The plasma leptin level significantly reduced in the HF-T group in comparison to the HF group, in the HF-ND group compared to the HF-T group, and the HF-ND+T group in comparison to the HF-ND group (P<0.05, respectively). All groups were significantly increased in hypothalamus BDNF protein expression in comparison to the HF group. In hypothalamus leptin mRNA expression, the HF-T and HF-ND groups reduced, but the HF-NF+T group increased in comparison to the HF group. This result suggests that it shows the effect of exercise training upon anti-obesity in high-fat diet induced obese rats and the combined exercise and/or normal diet may affect the optimal obesity improvement and prevention in appetite and weight control.

        Effect of Regular Plyometric Training on Growthrelated Factors in Obesity Female Teenager

        OBJECTIVES This study aimed to investigate the effect of regular plyometric training on growth-related factors in obese female teenager. METHODS The subjects of the study consisted of elementary school students group (EG, n=5) and middle school students group(MG, n=6), and overweight or obese experimenters were selected based on the ‘2017 Child and Adolescent Growth Chart Age Body Mass’ index. Exercise was conducted for 12 weeks. All measurements were carried out before and after exercise. The data processing was verified using the SPSS 26.0 statistical program to verify the correlation between paired t-test and Pearson in the 12-week pretraining and post-training groups. RESULTS After 12 weeks of plyometric training, there were significant differences in height(p=.002), ASIS(p=.003), body fat percentage(p=.018), and muscle mass(p=.014) among body composition of EG. There was a significant difference in height(p=.015) in body composition of MG. In the evaluation of muscle function, in muscle strength(60°/sec), (R)-FLE PT/bw(p=.011), (L)-FLE PT/bw(p=.017) in EG and muscle power(180°/sec), (R)-FLE PT/bw(p=.024), (L)-EXT PT/bw(p=.001), (R)-FLE TW/bw(p=.004) and (L)-EXT TW/ bw(p=.012) showed a statistically significant difference. In terms of correlation, significant relationships were found between EG body fat mass and IGF-1(p<.05), and between body fat mass and IGF-1/IGF-BP3(p<.05). CONCLUSIONS Regular plyometric training had a positive effect on growth-related factors in obese female teenager.

        유산소 훈련 강도 차이가 비만 어린이의 산화적 조직 손상에 의한 심혈관질환 병태생리적 경로에 미치는 영향

        본 연구는 12주 유산소 운동이 비만 어린이의 혈청지질성분, 항산화효소 활성 및 자율신경계 활동에 미치는 영향을 조사하였다. 피험자들은 16명의 비만어린이와 19명의 정상어린이를 대상으로 12주간 중강도 유산소 운동을 실시하였다. 측정 항목은 VO₂peak, 신체조성, 혈청지질, ox-LDL, 8-OHdG, SOD, GPx 활성, total mRNA, 그리고 자율신경계 활성을 측정하였다. 그 결과 체중은 OW그룹에서 시기간 차이가 나타나지 않았으나, 12주 운동 후 OR과 CO그룹에서 유의하게 증가하였다. WHR은 OR과 CO그룹에서 차이가 나타나지 않았으나, 12주 운동 후 OW그룹에서 유의하게 감소하였다. 비만 그룹에서 baseline TG는 대조군 보다 더 높게 나타났으며(p<0.05), 자율신경계 활성은 낮게 나타났다. 항산화효소 유전자 발현은 모든 그룹에서 시기간 유의한 차이가 나타났다. 결론적으로 비만어린이에서 산화적 손상과 항산화효소 활성은 정상어린이와 유사하게 나타났다. 그러나 정상어린이에 비해 비만어린이의 TG는 더 높게 나타났고 자율신경계 활성은 더 낮게 나타났다. 따라서 비만어린이에서 TG의 증가와 자율신경계 활성의 감소는 비만환자의 출발점이 어린시기에 시작될 수 있음을 시사한다. 또한 규칙적인 유산소성 운동은 어린시기에서 항산화효소 발현을 수정하여 줄 수 있다고 사료된다. The purpose of this study was to examine the effect of 12 wk of aerobic exercise on lipid profiles, antioxidant enzyme activities, oxidative products, and autonomic nervous activity (ANA) in children with obesity. We studied 16 children with obesity and 19 age-matched normal weight controls over a period of 12 wk, during which time moderate intense running exercise was performed. Measurements included peak oxygen uptake, body composition, blood lipid profiles, ox-LDL, 8-OHdG, SOD, GPx activities, total mRNA, and ANA. There were no differences in body weight between periods in the OW group, but body weight increased after 12 wk in OR and CO groups. There were no differences in WHR between periods in the OR and CO groups, but the WHR values decreased after 12 weeks in the OW group. In the obese group, the baseline TG was higher than in the controls (p<0.05), while the ANA level was lower. There were differences in antioxidant enzyme gene expressions between periods in all groups. In conclusion, oxidative damage and antioxidant enzyme activities in obese children were found to be similar to those of normal weight children. However, TG was higher and ANA was lower in obese children than in normal weight children. These results indicated that increased TG and decreased ANA levels begins in childhood in obese patients. Also, regular aerobic exercise may modify the antioxidant enzyme gene expression in early life.
