        Type 304 스테인리스 강판의 상온 및 저온 피로특성에 미치는 Ni 함량과 가공유기 마르텐사이트 변태의 영향

        Effects of Ni content(8.3∼12 wt.%) and deformation-induced martensitic transformation on the bending and zero to tension fatigue behavior of Type 304 stainless steel used for the membrane of LNG storage tank at room and cryogenic temperature were investigated. By lowering the Ni content, the bending fatigue life at room temperature was decreased at the high level of total strain amplitude. It would be attributed to the crack initiation at γ/α' interface and the low ductility with the increase of α' contents. However, at the low level of total strain amplitude, lowering the Ni content increased the bending fatigue life. It was due to the suppression of crack propagation by the α'-martensite transformation around crack tip and the increase of tensile strength. At cryogenic temperature, lowering the Ni content showed beneficial effect on the bending fatigue life due to the increase of slip deformation resistance. In the zero to tension fatigue test, lowering the Ni content showed better fatigue characteristics regardless of the temperature variation. In this case, the high tensile strength with the α'-martensite transformation mostly affects material's fatigue life.

        Type 304 스테인리스 강판의 Ni 함량 변화에 따른 굽힘 피로수명 예측에 관한 연구

        The prediction of bending fatigue life was carried out for Type 304 stainless steel sheets containing various nickel contents from 8.3 up to 12 wt.%. After the slopes of the elastic strain (Δε_(e)/2) and the plastic strain (Δε_(p)/2) lines were obtained by the tensile test and bending fatigue test data at room and cryogenic temperatures, the prediction equations for the bending fatigue life were made by using the method of universal slope. At the ranges above 10³ Cycles, the predicted equations showed higher bending fatigue life same as measured one with lowering the nickel contents in the range of 8.3-12 wt. %. But, at the ranges below 10³ Cycles, the predictions of low-cycle bending fatigue life varying the nickel contents were different from the measured ones. A limitation was encountered for the prediction of the bending fatigue life with using the tensile data. In order to increase the reliability of the prediction equations at the ranges below 10³ Cycles, it is necessary to take into account the behavior of the α'-martensite transformation during fatigue affecting the generation and propagation of fatigue cracks.         Systematic Review of Hominis Placenta Pharmacopuncture in English and Korean Literature

        Background: Hominis placenta (HP) is used in Korean medicine to tonify qi and blood, and enrich yin and tonify yang. HP has been reported to have therapeutic effects. Methods: A survey of international and Korean electronic databases was conducted using the search terms "hominis placenta pharmacopuncture" and "hominis placenta extract". The search was limited to material published up to May 31, 2017. Results: A total of 83 studies were included in this systematic review: 50 were clinical studies, 25 were basic studies, and 8 were other types of study. Among clinical studies, the most frequently treated disease groups were musculoskeletal diseases and nervous system diseases. In vitro studies were conducted mainly on anti-inflammatory, analgesic, and anti-cell necrosis models. Most of the in vivo studies were performed in rheumatoid arthritis or diabetic complications models. Conclusion: HP pharmacopuncture has effects in the treatment of various diseases. Further large-scale randomized controlled trials are needed to improve the level of evidence for HP pharmacopuncture. It would be helpful if future in vitro and in vivo studies could identify the mechanism of action of HP pharmacopuncture.

        Eligibility and Preference of New Oral Anticoagulants in Patients With Atrial Fibrillation : Comparison Between Patients With Versus Without Stroke

        <P>Background and Purpose-Recent randomized clinical trials (RCTs) have evaluated the benefit of new oral anticoagulants in reducing the risk of vascular events and bleeding complications in patients with atrial fibrillation (AF). However, abundant and strict enrollment criteria may limit the validity and applicability of results of RCTs to clinical practice. We estimated the eligibility for participation in RCTs of an unselected group of patients with AF. In addition, we compared features favoring new oral anticoagulant use between patients with versus without stroke. Randomized Evaluation of Long-Term Anticoagulation Therapy Methods-We applied enrollment criteria of 4 RCTs (RE-LY, ROCKET-AF, ARISTOTLE, and ENGAGE-AF-TIMI 48) to 695 patients with AF taking warfarin, prospectively and consecutively collected at a university medical center; 500 patients with and 195 patients without stroke. Time in therapeutic range and bleeding risk scheme (anticoagulation and risk factors in atrial fibrillation) were also measured. Results-The proportions of patients fulfilling the trial enrollment criteria varied, ranging from 39% to 72.8%, depending on the differences in indications/contraindications among studies and presence/absence of stroke. The main reasons for ineligibility for RCTs were hemorrhagic risk (anticoagulation and risk factors in atrial fibrillation [ATRIA] score) (10.8%-40.5%) and planned cardioversion (5.1%-7.7%) for nonstroke patients, and a low creatinine clearance (5.6%-9.2%) and higher risk of bleeding (15.2%-20.8%) for patients with stroke. When compared with nonstroke patients, patients with stroke showed a lower time in therapeutic range (54.4 +/- 42.8% versus 65.4 +/- 34.9%, especially with severe disability) and a high hemorrhagic risk (ATRIA score) (3.06 +/- 2.30 versus 2.18 +/- 2.16) (P<0.05 in both cases). Conclusions-Patients enrolled in RCTs are partly representative of patients with AF in clinical practice. When time in therapeutic range and bleeding tendency with warfarin use were considered, the use of new oral anticoagulants was preferred in patients with stroke than in nonstroke patients, but they were more likely to be excluded in RCTs.</P>

      • 상지 동맥 폐쇄를 동반한 흉곽출구증후군 수술 치험 : a case report

        Thoracic outlet syndrome refers to the compression of the subclavian vessels and brachial plexus at the superior aperture of the thorax. It was previously designated according to the presumable etiologies such as scalenus anticus, costoclavicular, hyperabduction, cervical rib, and first thoracic rib syndromes. We experienced a case of thoracic outlet syndrome(cervical rib) combined with arterial obstruction of upper extremity. A 47-year-old male patient had suffered from pain, tingling sensation, and coldness of right hand. Preoperative brachial and CT angiography showed extrinsic compression of proximal subclavian artery with luminal thrombosis. Thromboendarterectomy and division of pectoralis minor tendon were done(1st operation), Transaxillary resection of first rib, cervical rib and scalenus anticus, thoracoscopic thoracic sympathicotomy and thromboendarterctomy were done(2nd operation). Thromboembolectomy was done(3rd operation). After the final operation good arterial pulsation of upper extremity was shown and maintained.

      • 외상에 의한 횡격막 손상의 임상적 고찰

        Objective : According to the changes in the environmental factors, traumatic diaphragmatic injury is increased, especially due to traffic accident. Many reports described the early diagnostic methods and treatment modality. Method : We analyzed the traumatic diaphragmatic injury patients from the opening of our hospital to December 2002. Results : There were 7 patients (Male: Female=4:3). Sites of injury were 4 in left diaphragm and 3 in the right. 5 patients showed diaphragmatic rupture and 2 patient did diaphragmatic eventration. Emergency operation was done in 4 patient. One patients died of postoperative complications (Acute renal failure with sepsis). Conclusion : We concluded that if there is suspicion of diaphragmatic injury after a trauma, careful study and examination is essential and interdepartmental collaboration is very important to accomplish the good results.

        에스트라디올 패취의 피부투과 특성 및 난소제거 토끼에서의 비교 효력시험

        Transdermal patch formulations of estradiol to treat post-menopausal symptoms and prevent osteoporosis in women were developed and evaluated for the permeation characteristics through the excised hairless mouse abdorminal skin and the uterotropic effect on the ovariectomized rabbits. The design of patch formulations was optimized by varying several formulation parameters, such as type of enhancers, amount of enhancers, amount of drug loading and coating thickness. Compared to a commercially available transdermal product, several patch formulations showed the similar skin permeation profiles (following zero-order kinetics), but their skin permeation rates were lasted for the longer period (a week). In one-week uterotropic efficacy test in the ovariectimized rabbits, the selected patch formulations showed the positive effect in atrophy of the urogenital epithelium. The mean values of uterus weight in rabbits after application of patches containing estradiol were much higher than those in control group (containing no drug).

      • Presence of Malignant Mesothelial Cells in the Sputum

        <P>Malignant pleural mesothelioma and peripheral adenocarcinoma, of the lung, also known as pseudomesotheliomatous adenocarcinoma, have similar clinical and radiological characteristics and even similar microscopic findings, and this makes it difficult to differentiate them. Malignant pleural mesothelioma rarely invades the bronchial lamina or bronchioloalveolar spaces, and tumor cells are not usually found in the sputum. Therefore, the appearance of tumor cells in sputum more likely supports the diagnosis of peripheral lung cancer. We report a rare case in which malignant pleural mesothelioma cells were found in the sputum. For the differential diagnosis of a mass involving both the pleura and lung, physicians should consider that malignant mesothelial cells can be found in the sputum, although this is very rare.</P>

        Effect of Lipopolysaccharide (LPS) on Mouse Model of Steroid-Induced Avascular Necrosis in the Femoral Head (ANFH)

        Avascular necrosis of the femoral head (ANFH) is commonly observed in patients treated with excessive glucocorticoid (GC). Single administration of lipopolysaccharide (LPS) has shown to induce immune stimulatory factors. However, the effect of repeated administration of LPS on GC-induced ANFH has not been studied. Thus, the purpose of this study was (i) to examine the cytokine profile induced by repeated LPS administrations and (ii) to test the effect of repeated LPS treatments on GC-induced ANFH. A mouse necrosis model of ANFH was designed by chronic GC administration with co-treatment of LPS. Mice body weights in the LPS/prednisolone (PDN) co-treated group were lower than that of the untreated control group, but spleen weights were greater than the control group. The levels of IL-6, TNFα, and IL-33 in the liver and spleen of the LPS/PDN group were lower than the untreated control group, whereas TNFα level in the femoral head of the LPS/PDN group increased. Collectively, the effect of repeated LPS on the pathogenesis of GC-induced ANFH was associated with the TNFα level in the femoral head, but the pathogenesis did not correspond to cytokine levels in immune tissues.
