        Pruritic Urticarial Papules and Plaques of Pregnancy Managed with Ayurveda: A Case Report

        Pruritic urticarial papules and plaque of pregnancy presents with difficulty in its management owing to the limited intervention options available during pregnancy and their sub optimal responses. In pregnancy, also often the mother remains reluctant in getting exposed to biological medicines for her concern to the growing fetus. In such conditions, Ayurveda, the traditional health care system from India is often looked as a safe alternative to offer symptom resolution and safe continuation of pregnancy without the discomforts of urticaria. This case report is an illustration in this regard.

        Being healthy: Feeling healthy

        We have various understandings regarding health. Various medical systems being practiced in India other than conventional medicine i.e. Ayurveda, Yoga & Naturopathy, Unani, Siddha and Homoeopathy have defined health in their own way. From physical level up to spiritual level health has so many aspects which affect our overall personality. Some says that one who is established in self is healthy but some other says that in a perfect state of health physical homeostasis and mental equanimity occur in a balanced and healthy harmony. Another concept believes that health is normal and harmonious vibration of the elements and forces composing the human entity on the physical, mental, moral and spiritual planes of living in accordance with the constructive principles in Nature. At one place it is stated that good health means existence in harmony with evolution. The authors have reviewed various concepts and definitions of health and trying to conclude that health is a feeling which may be experienced only if you are really healthy. So, health is more and more balancing of all elements of your life in harmony with the universe.

        Effect of Strategic Feed Supplementation during Gestation on Intake, Blood-biochemical Profile and Reproductive Performance of Goats

        Effect of strategically supplemented pregnancy allowance was ascertained during different phases of gestation on the reproductive performance of does. Gravid does (18) were allotted to 3 dietary treatments HH, HL and LH in a completely randomized block design. All does were provided wheat straw ad libitum and supplemented with concentrate mixture at the rate of $20g/kgW^{0.75}$ from 0-60 days post-mating. Subsequently, HH group was given concentrate at the rate of $40g/kgW^{0.75}$ from 61 d post-mating to term. HL group was offered concentrate from 61-90 d post-mating and 121d post mating to term at the rate of $20g/kgW^{0.75}$ and from 91-120 d post-mating at the rate of $40g/kgW^{0.75}$. LH group was provided with concentrate from 61 to 120 d post-mating and 121 to term at the rate of 20 g and $40g/kgW^{0.75}$, respectively. Mean total dry matter and concentrate intake ($g/kgW^{0.75}$) was significantly higher on HH in comparison to comparable intake of HL and LH treatments, however, wheat straw followed the reverse trend. Haematological and biochemical parameters except serum glucose, total serum protein and A:G ratio did not differ significantly among dietary treatments. The gross gain in weight, products of pregnancy at 21 weeks of gestation, pregnant does average daily gain, birth weight of kids and survivability of kids were significantly lower in treatment HL in comparison to comparable values for HH and LH treatments. The net gain in gravid does was significantly higher in treatment HH than the comparable values obtained in HL and LH treatments. The last month of pregnancy was found to be nutritionally most sensitive period of gestation for native goats.

        Management of chronic dry cough through Ayurveda: Illustrating ayurvedic treatment principles through practice

        A case of chronic dry cough which was tried to be treated through so many medications in a period of two months, responded very well to a simple ayurvedic remedy composing of a common formulation chosen as per the ayurvedic principles of pathogenesis and management. This case may be an eye opener to the concurrent ayurvedic clinical practice and invites ayurvedic practitioners to stick to their own principles of disease management for obtaining better responses.

      • Impact of different LEED versions for green building certification and energy efficiency rating system: A Multifamily Midrise case study

        <P>Various versions of the Leadership in Energy and Environmental Design (LEED.) have been introduced with the addition of more stringent sustainability parameters and credit scoring schemes over the past decade. Such changes in LEED versions strongly affect the energy performance and LEED scores of the target building in the LEED certification process. Therefore, to validate and improve the current LEED version, it is crucial to investigate and compare the impact of different LEED versions on the building energy performance and scoring scheme. However, researches comparing the sustainability metrics for mid-rise multi-family buildings are rare. Therefore, this paper investigates the potential changes in the energy performance resulted from applying different LEED versions (i.e., LEED v3 and v4) for the Energy and Atmosphere (EA) category. Towards this end, a case study was carried out with energy modeling and simulation using TRACE 700 to compare the changes in the energy performance of four analysis scenarios applied to an existing mid-rise multi-family building located in Ohio. Results showed notable changes in LEED points when different versions of LEED using different ASHRAE Standards (i.e., ASHRAE Standards 90.1-2007 and 90.1-2010) are applied for the building energy analysis. In particular, mid-rise multi-family buildings could benefit from LEED v4 in terms of LEED credits as the prerequisite for the minimum energy performance improvement in EA category became significantly lenient compared to LEED v3. On the contrary, when the percentage energy performance improvement is over 34%, mid-rise multi-family buildings would benefit from LEED v3 as it becomes difficult to gain more points for similar energy performance improvement in LEED v4 compared to LEED v3. Various stakeholders including USGBC and government can benefit from using the key findings of this study for improving the LEED certification and national energy standards.</P>

        Dosimetric comparison of IMRT versus 3DCRT for post-mastectomy chest wall irradiation

        Purpose: To compare the dose distribution of three-dimensional conformal radiation therapy (3DCRT) with intensity-modulated radiation therapy (IMRT) for post-mastectomy radiotherapy (PMRT) to left chest wall. Materials and Methods: One hundred and seven patients were randomised for PMRT in 3DCRT group (n = 64) and IMRT group (n = 43). All patients received 50 Gy in 25 fractions. Planning target volume (PTV) parameters-$D_{near-max}$ ($D_2$), $D_{near-min}$ ($D_{98}$), $D_{mean}$, $V_{95}$, and $V_{107}$-homogeneity index (HI), and conformity index (CI) were compared. The mean doses of lung and heart, percentage volume of ipsilateral lung receiving 5 Gy ($V_5$), 20 Gy ($V_{20}$), and 55 Gy ($V_{55}$) and that of heart receiving 5 Gy ($V_5$), 25 Gy ($V_{25}$), and 45 Gy ($V_{45}$) were extracted from dose-volume histograms and compared. Results: PTV parameters were comparable between the two groups. CI was significantly improved with IMRT (1.127 vs. 1.254, p < 0.001) but HI was similar (0.094 vs. 0.096, p = 0.83) compared to 3DCRT. IMRT in comparison to 3DCRT significantly reduced the high-dose volumes of lung ($V_{20}$, 22.09% vs. 30.16%; $V_{55}$, 5.16% vs. 10.27%; p < 0.001) and heart ($V_{25}$, 4.59% vs. 9.19%; $V_{45}$, 1.85% vs. 7.09%; p < 0.001); mean dose of lung and heart (11.39 vs. 14.22 Gy and 4.57 vs. 8.96 Gy, respectively; p < 0.001) but not the low-dose volume ($V_5$ lung, 61.48% vs. 51.05%; $V_5$ heart, 31.02% vs. 23.27%; p < 0.001). Conclusions: For left sided breast cancer, IMRT significantly improves the conformity of plan and reduce the mean dose and high-dose volumes of ipsilateral lung and heart compared to 3DCRT, but 3DCRT is superior in terms of low-dose volume.

        ON FINSLER SPACE OF RECURRENT CURVATURE TENSORS

        The Riemannian space of recurrent curvature was defined and studied by Ruse [8] and Walker [10]. In 1963, $M{\acute{o}}or$ [4] generalised this idea for Finsler spaces and defined and studied Finsler spaces of recurrent curvature. These spaces for various curvature tensors have subsequently been studied by Mishra and Pande [1], Sen [9] and Misra [3] etc. The purpose of the present paper is to study Finsler space based on the recurrency of the curvature tensors derived from non-linear connections.

        Effects of shirodhara in generalized anxiety disorder

        Anxiety is a common clinical presentation. Primary anxiety poses a significant problem in its management. Many among the current treatment options of anxiety are habit forming causing significant withdrawal symptoms. There are dose dependent responses often associated with drug adversities. Day time sedation is an undesired effect of many drugs used for anxiety management limiting its usage. Ayurveda recommends a bio- physical procedure to manage anxiety. Shirodhara, a dripping procedure is utilized as a front line therapy for anxiety in Ayurveda. Seeing the limitations of conventional biomedical management of anxiety requiring an improvement upon the existing protocols of managements, and also seeing the use of shirodhara for anxiety management in ayurvedic clinics, a pragmatic study to evaluate the effect of shirodhara in generalized anxiety disorder was done. This was a pragmatic study consisting of shirodhara with ksheer bala oil as an intervention upon generalized anxiety disorder patients. The study was conducted upon 13 patients and observations were done for six weeks. The responses observed through the changes in Hamilton Anxiety Scale were evaluated using paired t test to observe the pre-post significance.
