      • S-509 : Anaerobic bacteremia: Clinical characteristics and outcome

        Background: Anaerobes are a relatively uncommon but important cause of bloodstream infection. However, their epidemiology has not been well defined. We investigate the clinical characteristics and comorbidity factors of anaerobic bacteremia. Method: We conducted this study at the Gachon University Gil Medical Center, a 1,200-bed tertiary care medical hospital in Korea. Data were collected from January 2008 through December 2012. Patients with one or more blood cultures growing anaerobic bacteria were included. Each patient was included only once at the time of detection of the first bloodstream infection. Results: During the 5-years, in total, 157 patients with blood cultures positive for anaerobes were enrolled. The median age was 62 years (IQR, 43-76) with 29 patients aged less than 18 years (29/157, 18.5%). Male (97/157, 61.8%) was more affected than female. Bacteroides spp. (78/157, 49.7%) and Clostridium spp. (44/157, 28.0%) were frequently isolated, among which Bacteroides fragilis (46/157, 19.3%) and Clostrodium perfringens (22/157,14.0%) were most common. The predominant underlying diseases were malignancy (43/157, 27.4%) and diabetes mellitus (20/157, 12.7%). The portal of entry was alimentary tract (39/157, 24.8%), respiratory tract (23/157, 14.6%) and urogenital tract (21/157, 13.4%). In hospital mortality rate of patients with anaerobic bacteremia was 17.2% (27/157). Conclusions: Bacteroides fragilis and Clostridium perfringens were the most common anaerobic species cultured from the blood as expected. Patients with anaerobic bacteremia showed a significant in-hospital mortality rate as well. These results suggest that the further investigations on risk factors of mortality of patients with anaerobic bacteremia would be necessary.

        Kaposi's sarcoma is a lympho-angioproliferative disease that primarily affects the skin and subcutaneous tissues. Most importantly, Kaposi’s sarcoma occurs in the immunocompromised, especially AIDS patients and organ transplant recipients being treated with immunosuppressants. The occurrence of classic Kaposi’s sarcoma after the diagnosis of any primary solid tumor has rarely been reported. We experienced a 70-year-old man, who had received surgery and chemoradiotherapy for the treatment of primary gallbladder cancer, that developed multiple tender deep purple papules on his trunk, back and feet which were proven to be pathologic symptoms of Kaposi’s sarcoma.
