        Effect of number of uses and sterilization on the instrumented area and resistance of reciprocating instruments

        Objectives: This study evaluated the effect of repeated uses and autoclaving in the instrumented area, fracture resistance, and time of instrumentation of thermally treated nickel-titanium reciprocating systems. Materials and Methods: Two hundred simulated canals were instrumented using Reciproc Blue and WaveOne Gold. Each file was used up to 10 times or until fracture. The instrumented area was measured in pre- and post-operative images, using ImageJ software. Kaplan-Meier survival analysis evaluated the number of uses of instruments before fracture. Instrumented area and time of instrumentation were analyzed by Mann-Whitney U test and Kruskal-Wallis. Correlations among the number of uses and instrumented area were measured. The level of statistical significance was set at p < 0.05. Results: Reciproc Blue presented a higher estimated number of uses in comparison with WaveOne Gold (p = 0.026), but autoclaving did not affect the resistance to fracture of instruments (p > 0.05). The instrumented area was different among the evaluated groups (p = 0.039), and the instrumented area along the uses of both tested instruments was reduced. With the time of instrumentation, there was also a significant difference among the evaluated groups; the groups without sterilization cycles were faster, in comparison to those submitted to autoclaving (p = 0.010). Conclusions: Reciproc Blue was more resistant than WaveOne Gold, suffering later fracture. Additionally, the sterilization cycles did not influence the estimated number of uses of thermally treated reciprocating instruments, but the instrumented area of root canals was reduced along with the repeated uses of both instruments.

        Analysis of clinical and radiological outcomes of long tibial stemmed total knee arthroplasty in knee osteoarthritis complicated by tibial stress fracture

        Objective: Knee arthritis associated with tibial stress fractures represents an uncommon and difficult clinical scenario to treat. The use of long, fluted tibial extension rods has been vital in the management of such cases owing to immediate fracture stability and single-stage surgery without the need to open the fracture site. This study investigates clinical and radiological outcomes following total knee arthroplasty using a tibial extension stem in cases of knee osteoarthritis with tibial stress fracture. Methods: From February 2015 to December 2020, 17 patients who had total knee arthroplasty implanted with a long stemmed tibial component were included in the study. Patient data were analyzed for knee range of motion, deformities, Knee Society score, knee function score, and time to fracture union in the pre- and postoperative periods. Results: The mean follow-up duration was 22.7 ± 11.68 months (range 12-60 months), and mean time to fracture healing was 10.23 ± 2.81 weeks (range 8-20 weeks). The preoperative mean fixed flexion deformity improved from 8.53 ± 3.43° to a mean of 0.29°, and knee flexion improved from 79.4 ± 13.90° to 125.29 ± 8.74° on postoperative assessment. The Knee Society score improved from a mean preoperative score of 18.94 ± 5.55 (range 8-28) to 89.41 ± 7.5 (range 74-102, p value < 0.001). Similarly, the knee function score improved significantly from a mean preoperative score of 15.5 ± 4.48 (range 8-26) to a mean of 85 ± 6.09 (range 72-94, p value < 0.001). Conclusion: Total knee arthroplasty using long tibial extenders has been an effective and safe surgical option for patients with advanced osteoarthritis with tibial stress fractures.

        Synthesis, Photophysical,Electrochemical, Tumor-Imaging,and Phototherapeutic Properties of Purpurinimide-<i>N</i>-substituted Cyanine Dyes Joined with Variable Lengths of Linkers

        <P>Purpurinimide methyl esters, bearing variable lengthsof N-substitutions,were conjugated individually to a cyanine dye with a carboxylic acidfunctionality. The results obtained from <I>in vitro</I> and <I>in vivo</I> studies showed a significant impactof the linkers joining the phototherapeutic and fluorescence imagingmoieties. The photosensitizer–fluorophore conjugate with aPEG linker showed the highest uptake in the liver, whereas the conjugatelinked with two carbon units showed excellent tumor-imaging and PDTefficacy at 24 h postinjection. Whole body imaging and biodistributionstudies at variable time points portrayed enhanced fluorescent uptakeof the conjugates in the tumor compared to that in the skin. Interestingly,the conjugate with the shortest linker and the one joining with twocarbon units showed faster clearance from normal organs, e.g., theliver, kidney, spleen, and lung, compared to that in tumors. Bothimaging and PDT efficacy of the conjugates were performed in BALB/cmice bearing Colon26 tumors. Compared to the others, the short linkerconjugate showed poor tumor fluorescent properties and as a corollarydoes not exhibit the dual functionality of the photosensitizer–fluorophoreconjugate. For this reason, it was not evaluated for <I>in vivo</I> PDT efficacy. However, in Colon26 tumor cells (<I>in vitro</I>), the short linker was highly effective. Among the conjugates withvariable linkers, the rate of energy transfer from the purpurinimidemoiety to the cyanine moiety increased with deceasing linker length,as examined by femtosecond laser flash photolysis measurements.         Choroidal Thickness in Multiple Sclerosis: An Optical Coherence Tomography Study

        Background and Purpose To identify changes in the choroidal thickness (CT) in multiple sclerosis (MS) patients with and without optic neuritis (ON) using enhanced-depth-imaging optical coherence tomography (EDI-OCT). Methods This cross-sectional study included 96 eyes with MS and 28 eyes of healthy controls. All participants underwent an ophthalmologic examination and EDI-OCT scanning (Spectralis, Heidelberg Engineering, Germany) to assess the CT and the retinal nerve fiber layer (RNFL) thickness. MS patients were divided into two groups: 1) with and 2) without a history of ON. The CT was evaluated in the fovea and at six horizontal and six vertical points at 500, 1,000, and 1,500 µm from the fovea. Paired t-tests were used to compare the groups, and p-value<0.05 was considered as significant. Results At all 13 measurements points, the CT was thicker in MS patients than in the healthy controls and was thinner in eyes with ON than in the contralateral eyes, but these differences were not statistically significant. However, the CT was always larger in all points in eyes with a history of ON than in the control eyes. The RNFL was significantly thinner (p<0.05) in both MS and ON eyes than in the control eyes. Conclusions The CT did not differ between MS and control eyes, but it was significantly larger in patients with a history of ON, in whom the RNFL was thinner. Further studies are necessary to establish the possible role of the choroid in MS

        Economic valuation for sustainable mangrove ecosystems management in Bohol and Palawan, Philippines

        The study used the total economic value as a framework for estimating the values of the different uses of mangroves in Palawan and Bohol, Philippines. Market surveys were conducted to estimate the revenues from the major mangrove products collected in the two study sites. Contingent valuation method and travel cost method questionnaires were used in the survey to determine the biodiversity and recreational values (in Philippine pesos or PHP). Results of the market survey showed that the highest direct use value per year was estimated for Banacon site in Bohol with PHP 33.368 million per year as compared to Kamuning site in Palawan (PHP 25.521 million). The highest value of benefit for Banacon came from crab catch with PHP 13.23 million. Mollusk catch gave higher benefits to Kamuning, amounting to PHP 17.65 million. In terms of the values of benefits from shrimp and fish catch, the values for Banacon were higher than those in Kamuning site (PHP 8.06 million and PHP 6.38 million, respectively for Banacon, and PHP 4.32 million and PHP 0.16 million, respectively, for Kamuning). Nipa thatch was the source of the third highest value of benefit for Kamuning amounting to PHP 2.42 million. The result of the contingent valuation analysis done (combining the data sets from Palawan and Bohol) using a least-squares method (i.e., bid function approach) showed a mean willingness-to-pay amount of PHP 44/month per individual. The different factors affecting the willingness to pay (25% level) include education, income and information (new). In terms of the recreation/ecotourism value of mangroves, the estimated values in Banacon, Bohol, and Kamuning, Palawan, were PHP 83,079 and PHP 2769, respectively.

        <P><B>Abstract</B></P><P>This article summarizes the recent activity of the International Stem Cell Banking Initiative (ISCBI) held at the California Institute for Regenerative Medicine (CIRM) in California (June 26, 2016) and the Korean National Institutes for Health in Korea (October 19–20, 2016). Through the workshops, ISCBI is endeavoring to support a new paradigm for human medicine using pluripotent stem cells (hPSC) for cell therapies. Priority considerations for ISCBI include ensuring the safety and efficacy of a final cell therapy product and quality assured source materials, such as stem cells and primary donor cells. To these ends, ISCBI aims to promote global harmonization on quality and safety control of stem cells for research and the development of starting materials for cell therapies, with regular workshops involving hPSC banking centers, biologists, and regulatory bodies. Here, we provide a brief overview of two such recent activities, with summaries of key issues raised.
