      • Quantitative Analyses of Esophageal Cancer Research in Pakistan

        Background: Health-care research is a neglected discipline in Pakistan and research related to esophageal cancer (ranks 9th in Pakistani males and 5th in females) is no exception in this regard. Particularly, there are no data available to delineate the overall status of esophageal cancer epidemiological studies in Pakistan. This study describes the first ever effort to make a systematic quantification, in an attempt to provide a road-map to all stakeholders for designing appropriate epidemiological, diagnostic and therapeutic strategies. Materials and Methods: International (PubMed, ISI Web of Knowledge) and local (PakMedinet) scientific databases as well as Google search engine were searched using specified keywords to extract relevant publication. Well-defined inclusion criteria were implemented to select publications for final analyses. All data were recorded by at least 3 authors and consensus data were entered into and analyzed for descriptive statistics (such as frequencies, percentages and annual growth rates) using Microsoft Excel and SPSS software. Results: A total of 79 publications fulfilled the inclusion criteria including 20 publications for which full texts were not available. Of the 79 publications, 59 (74.6%) were original/research publications, 5 (6.3%) were case reports, 4 (5.1%) were research communications, 2 (2.5%) were review articles, 1 was (1.2%) correspondence and 8 (10.1%) were undefined categories. Only 13 (<20%) cities of Pakistan contributed towards the 79 publications. On average, only 1.9 relevant publications/year were published from 1976 (year of first publication) to the present. Alarmingly, a decline in the annual growth at -4.1% was recorded in the last six years. Conclusions: Esophageal cancer research is largely unfathomed in Pakistan. Urgent/dramatic steps are required by all concerned to address this common (and under reported) cancer of Pakistan.

        A Meta-analysis: Shape of Age Effects and Second Language Grammar Acquisition

        This study reports findings of a meta-analysis on the effects of the age of first exposure, known as the age of onset (AO), on the acquisition of morphosyntax in a second language (L2). Several studies report restrictive effects of AO on L2 grammar acquisition (Abrahamsson, 2012; DeKeyser et al., 2010; Qureshi, 2016); however, there is a disagreement about the way sensitivity to second language acquisition declines (Birdsong, 2009; Long, 2013). Researchers disagree about the nature of the decline and argue whether the decrease that occurs in learners’ ultimate proficiency in the second language is gradual and continuous or gradual but followed by a plateau, without further decline. To explore the nature of AO, 29 samples from 14 studies were analyzed. The results revealed a large effect size1 (d = 1.13, SD = .06) for the impact of AO on ultimate proficiency in L2 morphosyntax. However, for the disaggregated data (i.e., when earlier & late learners are separated), a linear decline was observed for the 6 to 16 age-group (21 samples), while a flattening of the regression line was detected for the 17 to 24 age-group (8 samples). More importantly, the findings revealed two different shapes of decline based on participants’ first language and the target L2. Overall, the findings support a bounded critical period for second language acquisition and indicate the typological distance between the two languages as a major influencing factor.

        Grammaticality Judgment Task: Reliability and Scope

        Although extensively used, concerns have been expressed about the reliability and generalizability of grammaticality judgment tasks (GJTs; Alanazi, 2015). It has been argued that learners might guess grammaticality based on feel (Bialystock, 1979) and that the GJT-based results ignore grammatical complexity (Ellis, 1991). While several studies have attempted to validate the results of a GJT against other tasks, the tasks used in these studies required language production (e.g., Leow, 1996; Mandell, 1999), which is different from error identification and correction. The current study used an editing task (ET) to validate learners’ performance on a GJT. An editing task, like a GJT, requires identification of grammatical inaccuracies. Besides, it situates errors in a meaningful context and offers opportunities for multiple corrections of the same error. Overall, 311 participants took part in the study. A paired sample t-test showed a significant difference (t = 24.10, p = .00, d = 1.91) between the two tasks. Fifteen percent of the data (45 participants) was further inspected to have a better understanding of learners’ error correction-pattern on the editing task. The results revealed that about 20% of the errors identified were wrongly corrected, which exposes limitations of the GJTs that only require judging the well-formedness of a construction. Moreover, performance on the editing task revealed a greater level of diversity in learners’ responses. On average, eight of the total twelve grammatical features were corrected in 5.87 (SD = 2.47) different ways. The variety of responses on the ET reveals the dynamic nature of learners’ interlanguage that allows for multiple ways for correction. The ET, as compared to the GJT, provides an ecologically more valid way to assess L2 learners’ grammatical proficiency.

      • Molecular Characterization of FLT3 Mutations in Acute Leukemia Patients

        Fms-like tyrosine kinase 3 (FLT3) performs a vital role in the pathogenesis of hematopoietic malignancies. Therefore in recent times, the focus of several studies was on use of FLT3 as a prognostic marker. The present study investigated the molecular characterization and incidence of FLT3 mutations in acute leukemia patients in Pakistan. A total of 55 patients were studied, of which 25 were suffering from acute lymphoblastic leukemia (ALL) and 30 were suffering from acute myeloid leukemia (AML). The polymerase chain reaction demonstrated FLT3/ITD mutations in 1 (4%) of 25 ALL patients, a male with the L2 subtype. In AML cases the rate was 4 (13.3%) of 30, three males and one female. The AML-M4 subtype was found in three and the AML M2 subtype in the other. In the AML cases, a statistically significant (p=0.009) relationship was found between WBC (109/L) and FLT3/ITD positivity. However, no significant relationship was found with other clinical parameters (p>0.05). In acute myeloid leukemia (AML) $FLT3/ITD^+$ mutation was more prevalent in elderly patients 31-40 age groups, 21-30 and 51-60 age groups respectively. In acute lymphoblastic leukemia (ALL) statistically no significant relationship was found between clinical features and FLT3/ITD positivity (p>0.05). However, in acute lymphoblastic leukemia (ALL) $FLT3/ITD^+$ mutation was more commonly found in age groups of 21-30.
