        Evidence-based Medicine versus the Conventional Approach to Journal Club Sessions: Which One Is More Successful in Teaching Critical Appraisal Skills?

        This study aimed to compare evidence-based medicine (EBM) vs. conventional approachesto journal club sessions in teaching critical appraisal skills in reading papersby emergency medicine residents. This double cut off discontinuation regression quasi-experimental study was conducted among emergency medicine residents. EBM vs. the conventional approach were applied to teach critical appraisal skills for half of theresidents as an experimental group and another half as a control group respectively. Both groups participated in one hour monthly journal club sessions for six months. Before and after the study, all participants were examined by two tests: the Fresno Test(FT) [to evaluate their knowledge about EBM] and the Critical Appraisal Skills Test(CAST) [to evaluate their competency with critical appraisal skills]. The allocation ofthe participants into the experimental or control groups was according to their CASTscores before the study. 50 emergency medicine residents participated. After the study,the scores of both groups in the FT and CAST significantly improved (p<0.01), andthe promotion of scores of the FT and CAST in the experimental group were more thanthat of the conventional group (p<0.0001). The current study indicated that an evidence-based medicine approach in journal club sessions was comparatively more advantageouscompared to the conventional approach in teaching critical appraisal skillsfor reading papers among the residents of emergency medicine.

        Rotorcraft Flight Regime Recognition in Complex Maneuvers Using Adaptive Extended Kalman Filter

        The retirement time of helicopter components can be extended using the health and usage monitoring system (HUMS) and revisiting the flight spectrum with enough precision through fatigue damage analysis. The recognition of flight regimes for air vehicles with complex maneuvers is a major problem in determining the experienced spectrum. The relationship between flight variables and regimes is complex and coupled during a mission profile with multiple maneuvers that needs more efficient algorithms. In this paper, a logical framework is developed to identify the complex maneuvers based on the qualitative and descriptive interpretation of each maneuver by the pilot. The regime recognition criteria are implemented in the algorithm using an adaptive extended Kalman filter with both flight instruments and control movements as the measurements. The proposed algorithm classifies the flight regimes into the standard fatigue spectrum while needing no large amount of training data and not being sensitive to pilot behavior and mission variations. The proposed regime recognition algorithm is evaluated through extensive simulation of a comprehensive dynamic model of helicopter flight. The results show the improved accuracy of the proposed method in comparison with that of previous ones, while the proposed method is easier to embed in HUMS software without exhaustive training.

        Reliability evaluation of water distribution network considering mechanical characteristics using informational entropy

        Many studies have been carried out to investigate the important factors in calculating the realistic entropy amount of water distribution networks, but none of them have considered both mechanical and hydraulic characteristics of the networks. Also, the entropy difference in various networks has not been calculated exactly. Therefore, this study suggested a modified entropy function to calculate the informational entropy of water distribution networks so that the order of demand nodes and entropy difference among various networks could be calculated by taking into account both mechanical and hydraulic characteristics of the network. This modification was performed through defining a coefficient in the entropy function as the amount of outflow at each node to all dissipated power in the network. Hence, a more realistic method for calculating entropy was presented by considering both mechanical and hydraulic characteristics of network while keeping simplicity. The efficiency of the suggested method was evaluated by calculating the entropy of some sample water networks using the modified function.

        Bistoon town located in the western part of Iran is a cultural heritage tourism destination characterized by its unique historical site which counts as one of the most famous sites in Near Eastern archeology. It is Combination of five different engineering's of irrigation, stone; restoration, bridge construction and industry all have created a two-thousand-year masterpiece. Bistoon was inscribed into UNESCO's World Heritage in 2006 and attracts thousands of national and international tourists in a year. This research examines the perceptions of the residents toward heritage tourism. The study techniques include questionnaire survey and interviews. Evidence of conflict is seen among residents towards tourism development in the area.

      • Colorectal Cancer Mortality in Shiraz, Iran

        Background: Mortality among Iranian patients with colorectal cancer has not been fully examined and the factors associated with their survival are still controversial. This study aimed to determine the mortality rate and its related factors among the patients with colorectal cancer in southwestern regions in Iran. Materials and Methods: This prospective cohort study was conducted on 220 patients with colorectal cancer referred to Fahighi Hospital, Shiraz, Iran from 2009 to 2014. Data were collected from the patients' medical records and were analyzed using Cox regression analysis. Results: Over a median follow-up of 29.3 months, 56 out of the 220 patients (25.5%) died, 32 (14.5%) aged below 40 years, and 45.5% were female. Based on the results of multiple Cox regression analysis, family history of gastrointestinal cancer, stage III, former smoking, type of lesion (fungative and polypoid), and opium use were associated with a greater risk of colorectal cancer mortality (all P<0.05). Conclusions: This cohort study found that the mortality rate of colorectal cancer in Iran is lower than that in European countries. In addition, behavioral and clinical factors were significantly associated with the survival rate. Addressing the related factors would help healthcare providers and physicians provide the best care and improve the survival rate.

        A microlagal-based carbonaceous sensor for enzymatic determination of glucose in blood serum

        In the present work, Chlorella vulgaris, a well-known microalga was used as a carbonic precursor, tosynthesis the water-soluble carbon dots (CDs) via hydrothermal method. To improve the properties,acidic carbon dots (ACDs) were also synthesized via ultrasoncation of microalgal biomass after acidichydrolysis (using diluted acidic solution). The aqueous solution of ACDs was used as a sensor for glucosedetermination based on glucose oxidase enzyme (GOx) reaction and fluorescence quenching of ACDs inthe presence of Fe3+ ions under the optimized conditions. The linear range of glucose in the blood serumwas measured from a range of 5 mM to 500 mM with a limit of detection (LOD) 2.84 mM. Additionally, thehydrogen peroxide, as the conventional reactive oxygen species (ROS) was determined based on theFenton reaction. The linear range of H2O2 was obtained from 10 mM to 200 mM with LOD of 975 nM. Inall of these applications, fluorescence quenching of ACDs was followed according to photo-inducedelectron transfer (PET) mechanism in the presence of generated Fe3+ ions by Fenton reaction.

        Modelling of I-Shaped Beam-to-Tubular Column Connection Subjected to Post-Fire Conditions

        This paper presents numerical results of structural post- fire bahaviour of I-shaped beam-to-chord joints in offshore platformstopside. Considering the high risk of fire events in offshore oil/gas platforms, this study focuses on the structural behaviour ofthese connections at post- fire condition. A highly detailed three-dimensional (3-D) finite element (FE) model of this connectionhas been created using the ABAQUS software. Steel members and connection components are considered to behave nonlinearly. The results of finite element and experimental tests conducted on I shape beam-to-tubular column connections in furnace fireconditions are compared, and the obtained failure modes and moment-rotation-temperature characteristics are in good agreementwith those associated with experimental tests. The validated model has been used to conduct numerical parametric studies togenerate theoretical data to help develop detailed understanding of the joint behaviour in post-fire condition.

        In recent years, cloud computing is highly embraced and more organizations consider at least some type of cloud strategy and apply theming their business process. Since failure is probable in cloud data centers and access to cloud resources available is fundamental, evaluation and application of different fault-tolerance methods is inevitable. On the other hand, the increasing growth of cloud storage users motivated us to study fault-tolerance techniques, and their strengths and weaknesses. In this paper, after introducing the concept off ault-tolerance in the context of cloud computing, the fault-tolerant techniques are presented, and after introduction of some measures, a comparative analysis is provided.

        An efficient reliability estimation method for CNTFET-based logic circuits

        Carbon nanotube field-effect transistors (CNTFETs) have been widely studied as a promising technology to be included in post-complementary metal-oxide-semiconductor integrated circuits. Despite significant advantages in terms of delay and power dissipation, the fabrication process for CNTFETs is plagued by fault occurrences. Therefore, developing a fast and accurate method for estimating the reliability of CNTFET-based digital circuits was the main goal of this study. In the proposed method, effects related to faults that occur in a gate's transistors are first represented as a probability transfer matrix. Next, the target circuit's graph is traversed in topological order and the reliabilities of the circuit's gates are computed. The accuracy of this method (less than 3% reliability estimation error) was verified through various simulations on the ISCAS 85 benchmark circuits. The proposed method outperforms previous methods in terms of both accuracy and computational complexity.

        Tidal Asymmetry in a Tidal Creek with Mixed Mainly Semidiurnal Tide, Bushehr Port, Persian Gulf

        This study investigated the tidal asymmetry imposed by both the interaction of principal tides and the higher harmonics generated by distortions within a tidal creek network with mixed mainly semidiurnal tide in the Bushehr Port, Persian Gulf. Since velocity and water-level imposed by principal triad tides K1-O1-M2 are in quadrature, duration asymmetries during a tidal period in this short, shallow inverse estuary should be manifest as skewed velocities. The principal tides produce periodic asymmetries including a strong ebb-dominance and a weak flood-dominance condition during spring and neap tides respectively. The higher harmonics induced by nonlinearities engender a flood-dominance condition where the convergence effects are higher than frictional effects, and an ebbdominance condition where intertidal storage are extended. Since the triad K1-O1-M2 driven asymmetry is not overcome by higher harmonics close to the mouth, the periodic asymmetry dominates within the creek in which higher harmonics reinforce the weak flood-dominance (strong ebb-dominance) condition in the convergent channel (divergent area). Also, the maximum flood and the maximum ebb from all harmonic constituents occurred close to high water slack time during both spring and neap tides in this short creek. Since occational wetting of intertidal areas happened close to the high water (HW) time during spring tide, the water level flooded slowly close to the HW time of the spring tide.
