        Absence and Resolution of Fragmented QRS Predict Reversible Myocardial Ischemia With Higher Probability of ST Segment Resolution in Patients With ST Segment Elevation Myocardial Infarction

        Background and Objectives: Fragmented QRS complexes (fQRS) are associated with increased morbidity and mortality. The causative relationship between fQRS and cardiac fibrosis has been shown, but whether the presence and the number of fQRS on admission of elec-trocardiogram (ECG) predicts ST segment resolution in patients undergoing primary percutaneous coronary intervention (p-PCI) has not been investigated until now. Subjects and Methods: This study included one hundred and eighty-four consecutive patients with ST elevation myocardial infarction (STEMI) who underwent p-PCI. The presence or absence of fQRS on pre and post-PCI ECG and their relation with myocardial infarction and reperfusion parameters were investigated. Results: Patients with fQRS on admission of ECG or newly developed fQRS after p-PCI had increased inflammatory markers, higher cardiac enzyme levels, increased pain to balloon time, prolonged QRS time, more extended coronary involvement and more frequent Q waves on ECG in comparison to patients with absence or resolved fQRS. The presence and higher number of fQRS on admission or post-PCI ECGs were significantly related with low percent of ST resolution and myocardial reperfusion parameters. The area under the receiver operating characteristics curve values for the presence and number of fQRS to detect Thrombolysis in Myocardial Infarction Blush Grade 0 and 1,were 0.682 and 0.703. Conclusion: In our study, fQRS was significantly related to infarction and myocardial reperfusion parameters before and after p-PCI. Successful myocardial reperfusion by p-PCI caused the reduction in number of fQRS and QRS time with higher ST resolution. fQRS may be useful in identifying the patients at higher cardiac risk with increased ischemic jeopardized or infarcted myocardium, and persistent or newly developed fQRS may predict low percent of ST segment resolution in patients undergoing p-PCI.         Human Microtubule-Associated-Protein Tau Regulates the Number of Protofilaments in Microtubules: A Synchrotron X-Ray Scattering Study

        Microtubules (MTs), a major component of the eukaryotic cytoskeleton, are 25 nm protein nanotubes with walls comprised of assembled protofilaments built from αβ heterodimeric tubulin. In neural cells, different isoforms of the microtubule-associated-protein (MAP) tau regulate tubulin assembly and MT stability. Using synchrotron small angle x-ray scattering (SAXS), we have examined the effects of all six naturally occurring central nervous system tau isoforms on the assembly structure of taxol-stabilized MTs. Most notably, we found that tau regulates the distribution of protofilament numbers in MTs as reflected in the observed increase in the average radius <R<SUP>MT</SUP>> of MTs with increasing Φ, the tau/tubulin-dimer molar ratio. Within experimental scatter, the change in <R<SUP>MT</SUP>> seems to be isoform independent. Significantly, <R<SUP>MT</SUP>> was observed to rapidly increase for 0 < Φ < 0.2 and saturate for Φ between 0.2-0.5. Thus, a local shape distortion of the tubulin dimer on tau binding, at coverages much less than a monolayer, is spread collectively over many dimers on the scale of protofilaments. This implies that tau regulates the shape of protofilaments and thus the spontaneous curvature C<SUB>o</SUB><SUP>MT</SUP> of MTs leading to changes in the curvature C<SUP>MT</SUP> (=1/R<SUP>MT</SUP>). An important biological implication of these findings is a possible allosteric role for tau where the tau-induced shape changes of the MT surface may effect the MT binding activity of other MAPs present in neurons. Furthermore, the results, which provide insight into the regulation of the elastic properties of MTs by tau, may also impact biomaterials applications requiring radial size-controlled nanotubes.

      • Paclitaxel suppresses Tau-mediated microtubule bundling in a concentration-dependent manner

        <P><B>Abstract</B></P> <P><B>Background</B></P> <P>Microtubules (MTs) are protein nanotubes comprised of straight protofilaments (PFs), head to tail assemblies of αβ-tubulin heterodimers. Previously, it was shown that Tau, a microtubule-associated protein (MAP) localized to neuronal axons, regulates the average number of PFs in microtubules with increasing inner radius <<I> R</I> <SUB>in</SUB> <SUP>MT</SUP> > observed for increasing Tau/tubulin-dimer molar ratio Φ<SUB>Tau</SUB> at paclitaxel/tubulin-dimer molar ratio Λ<SUB>Ptxl</SUB> =1/1.</P> <P><B>Methods</B></P> <P>We report a synchrotron SAXS and TEM study of the phase behavior of microtubules as a function of varying concentrations of paclitaxel (1/32≤Λ<SUB>Ptxl</SUB> ≤1/4) and Tau (human isoform 3RS, 0≤Φ<SUB>3RS</SUB> ≤1/2) at room temperature.</P> <P><B>Results</B></P> <P>Tau and paclitaxel have opposing regulatory effects on microtubule bundling architectures and microtubule diameter. Surprisingly and in contrast to previous results at Λ<SUB>Ptxl</SUB> =1/1 where microtubule bundles are absent, in the lower paclitaxel concentration regime (Λ<SUB>Ptxl</SUB> ≤1/4), we observe both microtubule doublets and triplets with increasing Tau. Furthermore, increasing paclitaxel concentration (up to Λ<SUB>Ptxl</SUB> =1/1) slightly decreased the average microtubule diameter (by ~1 PF) while increasing Tau concentration (up to Φ<SUB>3RS</SUB> =1/2) significantly increased the diameter (by ~2–3 PFs).</P> <P><B>Conclusions</B></P> <P>The suppression of Tau-mediated microtubule bundling with increasing paclitaxel is consistent with paclitaxel seeding more, but shorter, microtubules by rapidly exhausting tubulin available for polymerization. Microtubule bundles require the aggregate Tau-Tau attractions along the microtubule length to overcome individual microtubule thermal energies disrupting bundles.</P> <P><B>General significance</B></P> <P>Investigating MAP-mediated interactions between microtubules (as it relates to <I>in vivo</I> behavior) requires the elimination or minimization of paclitaxel.</P> <P><B>Highlights</B></P> <P> <UL> <LI> Increasing paclitaxel suppresses Tau-mediated microtubule bundling. </LI> <LI> A length-dependent mechanism for Tau-mediated microtubule bundling is proposed. </LI> <LI> Understanding MAP/microtubule behavior requires elimination of paclitaxel use. </LI> </UL> </P>

        Pathologic Evaluation of Routine Nasopharynx Punch Biopsy in the Adult Population: Is It Really Necessary?

        Objectives. To retrospectively evaluate the patients who underwent nasopharyngeal biopsy with imaging and biopsy results, who have or don’t have symptoms for nasopharyngeal pathology and to determine the ratio of the nasopharyngeal cancer cases and other pathologic conditions. Methods. In this retrospective study, 983 patients who underwent endoscopic nasopharyngeal biopsy for symptomatic nasopharyngeal lesions were included. All pathological results, benign or malign was recorded and classified due to the patients’ presenting symptoms such as symptomatic for nasopharyngeal pathology or asymptomatic. Computed tomography (CT) or magnetic resonance imaging (MRI) reports were also recorded separately as group A for malignancy or group B for not malignancy. Results. Forty-five (4.6%) of 983 biopsies were malignant. In this group, there is no statistically significant difference between symptomatic and asymptomatic group. For malignant pathologies, the sensitivity of MRI was found 88.2% and CT was 61.5%. Conclusion. For early diagnosis of nasopharyngeal cancer, all patients admitted to Ear, Nose and Throat (ENT) referral clinics should be examined endoscopically irrespective of their complaints and suspicious cases should be investigated by imaging especially by MRI. If MRI report clearly indicates Thornwaldt cyst or reactive lymphoid hyperplasia and this result is compatible with endoscopic findings, biopsy may not be necessary. Apart from these cases, all suspected lesions should be biopsied.

        Anti-Christian Ideas and National Ideology: Inoue Enryo and Inoue Tetsujiro`s Mobilization of Sectarian History in Meiji Japan

        Christian and anti-Christian ideas played a major role in the formation of modern Japanese national ideology. This article focuses on the construction of a sectarian history of the Tokugawa state as one part of the anti-Christian ideological writing of late nineteenth early twentieth century Japan. Academic and semi-academic writing on history and philosophy at this time was intimately connected with the major political debates which accompanied the introduction of the Imperial Constitution and the Imperial Rescript on Education. This article argues that these debates in many ways determined how these markers of state ideology would be interpreted in the future. Focusing particularly on the works of Inoue Tetsujiro and Inoue Enryo, this article shows how centrally historical discourses of sectarianism were deployed in the debates of the Meiji period, and how the historical, political and philosophical writings of figures like Enryo and Tetsujiro were integrated-both with each other, and with the pre-Meiji historical past.

        韓牛에 있어서 anaplama marginale의 抗原性에 관한 硏究

        A splenectomized 5 month old Korean calf was inoculated with cryopreserved Anaplasma marginale infected blood. At peak parasitemia(40.1%) blood was collected, infected erythrocytes washed, lysed and A. marginale initial bodies isolated by differential centrifugation. Antigenic profile of isolated A. marginale organism was analized by SDS-PAGE and Western blotting technique. Coomassian blue stained SDS-PAGE gels revealed at least ten specific protein bands of approximately 28Kd-30Kd, 38Kd, 42Kd, 58Kd, 66Kd, 72Kd and 198Kd molecular weight. Only few protein bands recognized with polypeptides in the noninfected bovine erythrocyte antigen used as control. A. marginale protein recognized by specific anti-A. marginale antibodies demonstrated 8 polypeptide at 18KD, 28-31Kd, 38Kd, 55-58Kd, 97Kd, and 198Kd molecular weight range. Negative reactivities were observed with normal bovine serum with either A. marginale or erythrocytic antigens, respectively. Predominant protein were found at 18Kd, 38Kd and 97Kd molecular weight. On going studies will analyze the relative importance of those antigens.
