        울산 청년니트 지원사업의 효과에 관한 사례연구 : 참여자의 전환 경험을 중심으로

        이 논문은 울산의 한 기관이 2019년부터 진행해온 청년니트 지원사업의 효과를 참여자의 전환 경험의 관점에서 파악하고자 한다. 이러한 분석을 통 해 청년니트 지원사업이 중점을 두어야 할 요소를 찾고, 이를 바탕으로 울산의 청년 니트 지원을 위한 정책 과제를 찾는 것을 목적으로 한다. 본 연구는 이 사업의 프로그램 내용과 사례자들의 기록을 검토하고, 이 사업을 실행한 사례관리자들과 이 사업의 지원대상인 니트 청년(대표 사례자 및 제2 사례 자 그룹)들과 인터뷰를 시행하였다. 사례자, 사례관리자, 비교그룹 등 3자에 대한 인터뷰와 그 분석 방법은 연구의 신뢰성과 타당성을 높이기 위해 삼각 검증(triangulation)을 통한 교차 검증이라고 할 수 있다. 인터뷰 내용을 분석 하기 위해 인터뷰 자료를 1차와 2차에 걸쳐 코딩을 하는 방법을 사용하였다. 분석 결과, 이 프로그램 참여자들 중 1명을 제외한 3명이 명시적으로 전환을 경험했으며, 그 계기로 상담자와의 상담과 사례관리자의 지지를 들었다. 이 런 사실은 사례관리자와 비교그룹과의 인터뷰에서도 확인되었다. 이 연구에 서 도출된 시사점은 다음과 같다. 첫째, 니트지원사업의 성공에는 사례관리 자의 역량과 열정이 중요하다. 둘째, 광역 및 기초지자체의 니트 사업에 대 한 정책 시행이 필요하다. 셋째, 니트 지원사업은 그 사업기간이 1년 이상의 장기여야 한다. This paper aims to understand the effectiveness of the youth NEET support project, which has been carried out by an institution in Ulsan since 2019. Through the analysis on the clients’ turning experience, it tries to find the key elements of youth NEET support project, and based on this, to find policy agendas for supporting youth NEET in Ulsan. This study reviewed the program contents of this project and the records of the clients, and conducted interviews with the clients(representative and second case groups) and case managers who executed this project. Interviews with three parties and the comparison and analysis of their records can be said to be cross-validation through triangulation to increase the reliability and validity of the study. In order to analyze the contents of the interview, the method of coding the interview data over the first and second rounds was used. We find three people among four experienced explicit turning, and find the causes of turning to be counseling with case managers and support from them. This fact was also confirmed in interviews with case managers and comparison groups. The implications derived from this study are as follows. First, the competence and passion of the case manager are important for the success of the NEET support project. Second, it is necessary to implement policies for NEET projects in metropolitan and basic local governments. Third, the NEET support project continues for long duration over one year.

        Two-year clinical outcomes after discontinuation of long-term golimumab therapy in Korean patients with rheumatoid arthritis

        Background/Aims: The aim of this study was to investigate long-term post-discontinuation outcomes in patients with rheumatoid arthritis (RA) who had been treated with tumor necrosis factor-α inhibitors (TNF-αi) which was then discontinued. Methods: Sixty Korean patients with RA who participated in a 5-year GO-BEFORE and GO-FORWARD extension trials were included in this retrospective study. Golimumab was deliberately discontinued after the extension study (baseline). Patients were then followed by their rheumatologists. We reviewed their medical records for 2 years (max 28 months) following golimumab discontinuation. Patients were divided into a maintained benefit (MB) group and a loss-of-benefit (LB) group based on treatment pattern after golimumab discontinuation. The LB group included patients whose conventional disease-modifying antirheumatic drug(s) were stepped-up or added/switched (SC) and those who restarted biologic therapy (RB). Results: The mean age of patients at baseline was 56.5 years and 55 (91.7%) were females. At the end of follow-up, 23 (38.3%) patients remained in the MB group. In the LB group, 75.7% and 24.3% were assigned into SC and RB subgroups, respectively. Fifty percent of patients lost MB after 23.3 months. Demographics and clinical variables at baseline were comparable between MB and LB groups except for age, C-reactive protein level, and corticosteroid use. Restarting biologic therapy was associated with swollen joint count (adjusted hazard ratio [HR], 1.90; 95% confidence interval [CI], 1.01 to 3.55) and disease duration (adjusted HR, 1.12; 95% CI, 1.02 to 1.23) at baseline. Conclusions: Treatment strategies after discontinuing TNF-αi are needed to better maintain disease control and quality of life of patients with RA.

        Reliability and Validity of the Korean Version of the Gout Impact Scale

        Background: The Gout Impact Scale (GIS), part of the Gout Assessment Questionnaire 2.0, measures gout-specific health-related quality of life (HRQOL). This study aimed to translate the GIS into Korean and validate the Korean version (K-GIS) using generic HRQOL measures. Methods: The GIS was translated into Korean and back-translated into English. We asked patients aged 18 years or older who met the 2015 gout classification criteria to fill out the questionnaires (from January 2022 to June 2022); the K-GIS (5 scales [0–100 scores each]), along with the Korean version of Health Assessment Questionnaire (HAQ) and EuroQol-5 dimension (EQ-5D). We investigated the internal consistency, construct validity, and discriminative validity for gout characteristics of K-GIS. The K-GIS form was administrated to patients 4 weeks later to assess the test-retest reliability using the intraclass correlation coefficient (ICC). Results: One hundred patients completed the questionnaire. The mean ± standard deviation age of the patients was 53.0 ± 15.1 years, and 99.0% of the patients were men. All scales had high degree of internal consistency (Cronbach’s α = 0.59 to 0.96) and test-retest reliability (n = 18, ICC = 0.83 to 0.94, all P < 0.001), except for unmet gout treatment needs. Weak-tomoderate correlations were observed between the K-GIS scales and HAQ or EQ-5D (r = 0.21 to 0.46). The K-GIS scores were significantly higher in the presence of bone erosion, absence of urate-lowering therapy, serum urate levels > 6 mg/dL, frequent gout flares in the past year, and fewer comorbidities. In contrast, neither the HAQ nor the EQ-5D could discern these subsets of patients. Conclusion: The K-GIS is a reliable and valid HRQOL measure for patients with gout. Higher K-GIS scores were associated with clinical characteristics leading to unfavorable outcomes, which were not demonstrated by the HAQ and EQ-5D.

        <P>Indole-3-acetic acid (IAA) is the most common plant hormone of the auxin class and is known to have many effects including cell proliferation enhancement and antioxidant property. However, no study has revealed its defensive effects against oxidative toxicity in human dental pulp stem cells (hDPSCs). In this study, we investigated the effects of IAA on hydrogen peroxide- (H<SUB>2</SUB>O<SUB>2</SUB>-) induced oxidative toxicity in hDPSCs. H<SUB>2</SUB>O<SUB>2</SUB>-induced cytotoxicity was attenuated after IAA treatment. Cell cycle analysis using FACS showed that the damaged cell cycle and increased number of apoptotic cells by H<SUB>2</SUB>O<SUB>2</SUB> treatment were recovered after the treatment of IAA. The H<SUB>2</SUB>O<SUB>2</SUB>-mediated increased expression of the proapoptotic genes, <I>BAX</I> and <I>p53</I>, was attenuated by IAA treatment, while IAA treatment increased antiapoptotic genes, <I>BCL-2</I> and <I>ATF5</I> expression. The increases of cleaved caspase-3 and ROS by H<SUB>2</SUB>O<SUB>2</SUB> were also decreased after treatment of IAA. To further investigate the mechanism of IAA, Nrf2-related antioxidant pathway was examined and the results showed that the level of Nrf2 and HO-1 expressions, stimulated by H<SUB>2</SUB>O<SUB>2,</SUB> decreased after treatment of IAA. Moreover, IAA treatment protected hDPSCs against H<SUB>2</SUB>O<SUB>2</SUB>-induced oxidative stress via increased expression of Nrf2 and HO-1, mediated by the AKT pathway.</P>

      • <i>Enterococcus faecium</i> L-15 Cell-Free Extract Improves the Chondrogenic Differentiation of Human Dental Pulp Stem Cells

        <P>Hyaline cartilage is a tissue of very low regenerative capacity because of its histology and limited nutrient supply. Cell-based therapies have been spotlighted in the regeneration of damaged cartilage. Dental pulp stem cells (DPSCs) are multipotent and are easily accessible for therapeutic purposes. In human gastrointestinal tracts, <I>Enterococcus faecium</I> is a naturally occurring commensal species of lactic acid bacteria. In this work, the human DPSCs were differentiated into chondrocytes using a chondrogenic differentiation medium with or without L-15 extract. We observed that chondrogenic differentiation improved in an <I>E. faecium</I> L-15 extract (L-15)-treated DPSC group via evaluation of chondrogenic-marker mRNA expression levels. In particular, we found that L-15 treatment promoted early-stage DPSC differentiation. Cells treated with L-15 were inhibited at later stages and were less likely to transform into hypertrophic chondrocytes. In L-15-treated groups, the total amount of cartilage extracellular matrix increased during the differentiation process. These results suggest that L-15 promotes chondrogenic differentiation, and that L-15 may be used for cartilage repair or cartilage health supplements. To our knowledge, this is the first report demonstrating the beneficial effect of L-15 treatment on chondrogenic differentiation.</P>

      • Understanding of 2015 Revised P.E. Curriculum Development Process -1 - Focusing on the Objective and Contents of P.E. Curriculum -

        Purpose: This study was done to develop a new draft of P.E. curriculum in accordance with the basic directions of 2015 revised curriculum based on creativity-convergence type human resources. To reflect the internal needs to improve P.E. curriculum, the emphasis was on finding ways to redraft the P.E. curriculum in order to foster the core competencies required at the nation level. Method: The development of new draft was done in two stages unlike that of previous process. The nature of P.E., objective, information system, physical activities selection, criteria, examples and accomplishment standards were developed in the first stage. The teaching.learning method, and its significance, evaluation and assessment were developed in the second stage. The second stage was focused on the teaching-learning method and its assessment, however, parts of the first stage was also modified and supplemented. Result: The modified and supplement parts of the first stage were as the following; First, the nature of P.E. were clarified and compressed. The objectives of P.E. were stated as a whole in the first stage, while the objectives of elementary schools and middle schools were further divided separately and briefly stated with an integrated objective to improve the clarity of the goal statement in the proposed section. Second, key concepts and generalized contents were partly modified in its defining elements and activities. In addition, ‘functional’ items developed in the first stage were reconstructed in terms that can encompass the contents of standard of accomplishment more implicitly. Terms to describe in detail were used to promote the cultivation of core competence rather than using terms related to the execution of the achievement standard as a parent concept covering various standard of accomplishment. Third, amount of the standard of accomplishment were reduced since some of the standard of accomplishment were duplicates of detailed achievement standard and some of them were difficult to understand. The improvement was made to reduce the content of standard of accomplishment as well as the number of its lower categories in the second stage. Fourth, examples of physical activities were ordered by classes to avoid any duplication. Since the selection criteria refer to the relevant teaching and learning methods stated, the ``selection criteria and examples of physical activity`` was changed to the ``examples of physical activity`` and suggested physical activity examples were re-selected and refined to suit each classes. Conclusion: In the first stage, the elementary and middle schools curriculums were improved to nurture creative-convergent talents required by the future society while desirable human ideal was reflected for the core competencies and skill-based society. More improvements were searched while the continuity of current curriculum were maintained. In addition, character strengthening education and nurturing creative and well rounded talents were reflected. The strengthening of safety education in particular was debated in terms of how it could be done through the P.E. curriculum which was in great needs of the nation and society.
