<P>The purpose of this study was to evaluate the impact of radioactive iodine therapy (RIT) on vocal function during the early follow-up period after total thyroidectomy (TT) using perceptive and objective measurements, questionnaires regarding subjective symptoms, and data on vocal function in a prospectively enrolled and serially followed thyroid cancer cohort. <B>Methods:</B> Of 212 patients who underwent TT and were screened between January and December 2010 at our hospital, 160 were included in the final analysis. Patients with the following histories were excluded: lateral neck dissection, organic vocal fold disease, external radiotherapy, and voice evaluation during thyroxine withdrawal. Patients were stratified into 3 groups: TT, TT with low-dose RIT (1.1–2.2 GBq), and TT with high-dose RIT (≥3.7 GBq). Voice evaluations were performed before surgery and at 1, 6, and 12 mo after TT. <B>Results:</B> Vocal characteristics were altered after TT, including changes on the grade, roughness, and strain scale; increased amplitude perturbation; decreased fundamental frequency; narrowed pitch range; and global disturbances in subjective functional parameters on the voice handicap index. However, the degree of vocal changes among the 3 groups did not significantly differ within the 1-y postoperative follow-up period. According to the results of subgroup analyses of patients who demonstrated good voice outcomes after TT, there were no significant functional differences among the 3 groups. <B>Conclusion:</B> RIT at any dose does not affect vocal function within 1 y of TT.</P>

Since most thyroid cancers are slowly growing and are not fatal, prevention of surgical complications is as important as removing the lesion. Although permanent vocal cord paralysis, a significant complication of thyroid surgery, is reported to occur in about 1-2% of cases, the actual incidence is expected to be higher for various reasons. When nerve damage occurs despite the surgeon's utmost care, appropriate measures should be taken immediately. However, even with the most aggressive approach, nerve repair (neurorraphy) rarely restores normal vocal cord movement; it is known only to maintain tension in the vocal cords to prevent atrophy. Therefore, preventing nerve damage is considered much more important. To achieve this goal, a thorough preoperative examination and understanding of the anatomical variations of the nerves around the thyroid, as well as precise and careful capsular dissection, are necessary. In this paper, the authors discuss methods for preventing and treating nerve damage during thyroid surgery.
Nasal Tip Surgery Using a Modified Septal Extension Graft by Means of Extended Marginal Incision

BACKGROUND:: The columellar strut is considered one of the standard procedures in nasal tip surgery, as it provides a structural foundation for the nasal tip. Although effective for most Caucasians, the columellar strut often results in a suboptimal outcome and does not provide an adequate foundation in Asian tip surgery. For an optimal outcome, the authors propose a different surgical paradigm for Asian tip surgery. METHODS:: Using an extended marginal incision, a wide-field endonasal dissection is performed and the distal nasal framework including the alar cartilages and the caudal septum is exposed widely. A septal extension graft is made from a preoperatively designed paper template, with markings for anchoring the alar cartilage and overlapping with the caudal septum. The extension graft is fixed to the caudal septum and the alar cartilages are sutured to the extension graft at the precise position using markings transferred to the extension graft. RESULTS:: This procedure was applied in 217 Asian rhinoplasties, with all patients having substantial improvement in their nasal tip shape. The advantages of this technique are numerous and include stable tip support and versatility in nasal tip modification. The same basic techniques and sequence can be applied to correct a wide range of tip deformities found in Asian noses. CONCLUSIONS:: The authors’ surgical paradigm combining the modified septal extension graft with wide-field endonasal dissections provides a unique opportunity to gain greater control and a more predictable outcome in Asian tip surgery. Stable septal support is crucial to the success of this technique.

Purpose: We devised omental free-shaped flap reinforcement on anastomosis and dissected area (OFFROAD) following reconstruction after gastrectomy. This study aimed to evaluate its safety and early clinical outcomes. Methods: One hundred fifty-six patients who underwent totally laparoscopic distal gastrectomy with delta anastomosis from July 2016 to April 2018 were divided into the OFFROAD group (80 patients) and non-OFFROAD group (76 patients). Differences in short-term operative outcomes and surgical complications were compared between the groups. All patients’ inflammatory marker levels were measured to monitor flap necrotic change and inflammatory reactions. The clinical features of both groups in terms of anastomotic leakage were transcribed. Results: The mean operation time of OFFROAD was five minutes shorter than non OFFROAD. Pain score in postoperative day1 and the serum WBC count on POD 1 was significantly lower in OG than in NOG. There were no statistical differences in short-term outcomes and surgical complications between two groups. Anastomotic leakage occurred in three patients in each group and there was no statistical difference in incidence. However, clinical features were notable when anastomotic leakage occurs. Unlike all three patients of non-OFFROAD group manifested every features of peritonitis, each patient of OFFROAD group just manifested only one of the three. Conclusion: This study showed the safety and feasibility of OFFROAD procedure. It might mitigate septic complications when there is an anastomotic leakage. Additional large-scale study is needed to assess the versatile usefulness of OFFROAD aside from its role as a physical barrier.

Despite intensive research efforts in recent decades, cancer remains a leading cause of death worldwide. The chalcone family is a promising group of phytochemicals for therapeutic use against cancer development. Naturally-occurring chalcones, as well as synthetic chalcone analogues, have shown many beneficial biological properties, including anti-inflammatory, antioxidant, and anti-cancer activities. In this report, trans-chalcone (TChal) was found to increase cell death in breast cancer cells, assessed using high content screening. Subsequently, using antibody array analysis, TChal was found to increase heme oxygenase-1 (HO-1) expression in TChal-treated breast cancer cells. Blocking of HO-1 by siRNA in breast cancer cells diminished the effect of TChal on cell growth inhibition. TChal-fed mice also showed less tumor growth compared to vehicle-fed mice. Overall, we found that TChal increases HO-1 expression in breast cancer cells, thereby enhancing anti-tumorigenesis. Our results suggest that HO-1 expression could be a potential new target of TChal for anti-tumorigenesis in breast cancer.
Quercetin induces dual specificity phosphatase 5 via serum response factor

The phytochemical quercetin has gained attention for its antiinflammatoryand anti-tumorigenic properties in various typesof cancer. Tumorigenesis involves the aberrant regulation ofkinase/phosphatase, highlighting the importance of maintaininghomeostasis. Dual Specificity Phosphatase (DUSP) plays acrucial role in controlling the phosphorylation of ERK. The currentstudy aimed to clone the DUSP5 promoter, and investigateits transcriptional activity in the presence of quercetin. The results revealed that quercetin-induced DUSP5 expressionis associated with the serum response factor (SRF) binding sitelocated in the DUSP5 promoter. The deletion of this site abolishedthe luciferase activity induced by quercetin, indicating itsvital role in quercetin-induced DUSP5 expression. SRF proteinis a transcription factor that potentially contributes to quercetin-induced DUSP5 expression at the transcriptional level. Additionally, quercetin enhanced SRF binding activity withoutchanging its expression. These findings provide evidence ofhow quercetin affects anti-cancer activity in colorectal tumorigenesisby inducing SRF transcription factor activity, therebyincreasing DUSP5 expression at the transcriptional level. Thisstudy highlights the importance of investigating the molecularmechanisms underlying the anti-cancer properties of quercetin,and suggests its potential use in cancer therapy.
