Simultaneous reconstruction of the forearm extensor compartment tendon, soft tissue, and skin

Malignant peripheral nerve sheath tumor (MPNST) is a very rare type of sarcoma, with an incidence of 0.001%. MPNST has a 5-year survival rate near 80%, so successful reconstruction techniques are important to ensure the patient’s quality of life. Sarcoma of the forearm is known for its poor prognosis, which leads to wider excision, making reconstruction even more challenging due to the unique anatomical structure and delicate function of the forearm. A 44-year-old male presented with a large mass that had two aspects, measuring 9×6 cm and 7×5 cm, on the dorsal aspect of the right forearm. The extensor compartment muscles (EDM, EDC, EIP, EPB, EPL, ECRB, ECRL, APL) and invaded radius were resected with the mass. Tendon transfer of the entire extensor compartment with skin defect coverage using a 24×8 cm anterolateral thigh (ALT) perforator free flap was performed. The patient was discharged after 18 days without wound complications, and has not complained of discomfort during supination, pronation, or wrist extension/flexion through 3 years of follow-up. To our knowledge, this is the first report of successful reconstruction of the entire forearm extensor compartment with ALT free flap coverage after resection of MPNST.

Zinc and Tin composite nanostructures were fabricated as gas sensors on the interdigitated ITO (Indium Tin Oxide) electrodes. Zinc oxide (ZnO) and Tin dioxide (SnO₂) nanostructures were grown at 900℃ in the Vapor-Liquid-Solid(VLS) growth mechanism in a furnace. FE-SEM images in Figure 1 illustrated the composite nanostructures of ZnO-SnO₂, called “Nano-vine”. The normal metal catalysts such as Au and Cu were replaced with the Tin in the ITO electrodes. The Nano-vine was partially more sensitive in the detection sensitivity of noxious gases such as NO₂, CO and NH3 than that produced by nanostructures using Zn or Sn single material powders.

Background: This study summarizes a survey conducted to improve and expand the external quality assessment (EQA) scheme for transfusion in Korea. Methods: The EQA items provided by the College of American Pathologists, Royal College of Pathologists of Australasia, National External Quality Assessment Service, and German program were explored. A commercially operated EQA program was also included in the survey. Results: Items not included in the Korean EQA programs included extended antigen typing, electronic cross-matching, eluate survey, and fetal red cell detection. Granulocyte- and platelet-related testing was also not covered by the Korean program. There were issues regarding establishing the criteria for acceptable ranges and manufacturing of EQA materials. Conclusions: We successfully conducted a survey for the improvement and expansion of EQA for transfusion medicine. A pilot project for extended antigen typing is expected to be implemented in 2021, and ongoing expansion will continue in the following years.

BACKGROUND:: The purpose of this study was to evaluate the results of long-term follow-up in patients with relatively severe unilateral craniofacial microsomia after mandibular distraction. METHODS:: The sample consisted of 26 patients with an average age of 6.08 years at the time of distraction. All patients had nonsyndromic unilateral craniofacial microsomia (Pruzansky-Kaban types IIA, IIB, and III). Follow-up for all patients continued until the completion of growth. The 26 clinical records and posteroanterior cephalograms of the patients, taken before distraction (time 0) and approximately 1, 4, and 11 years (time 1, time 2, and time 3, respectively) after distraction, were used. Four items (i.e., supraorbital tilting angle and occlusal tilting angle to the horizontal reference line, and maxillary and mandibular tilting angles to the vertical reference line) were analyzed at each of the four time intervals. RESULTS:: The overall mean distraction amount was 23 mm vertically and 21 mm horizontally. The average cephalometric follow-up was 131.7 months. In this study, the mandibular horizontal and vertical changes after distraction showed a gradual return of the asymmetry with growth in all patients. There were no statistically significant differences in any of the analyzed angles between the preoperative and final records, except for the supraorbital tilting angle. CONCLUSIONS:: The longitudinal results achieved by distraction osteogenesis were unstable and generally led to relapse, although they were very good in the early postoperative period. Therefore, further efforts are suggested to find a solution that prevents relapse. CLINICAL QUESTION/LEVEL OF EVIDENCE:: Therapeutic, IV.

Background: Tongue reconstruction is challenging with the unique function and anatomy. Goals for reconstruction differ depending on the extent of reconstruction. Thin and pliable flaps are useful for tongue tip reconstruction, for appearance and mobility. This study reports lateral arm free flap (LAFF) as a safe and optimal option for hemi-tongue reconstruction, especially for tongue tip after hemiglossectomy. Methods: Thirteen LAFFs were performed for hemi-tongue reconstruction after hemiglossectomy from 1995 to 2018. Of the 13 patients, seven were male and six were female, age varying from 24 to 64 years. Results: All flaps healed uneventfully without complications. Donor sites were closed primarily. The recipient vessels for microvascular anastomosis were mainly superior thyroidal artery, external jugular vein. All patients returned to normal diet, with no complaints regarding reconstructed tongue and donor site. Conclusion: The LAFF is hairless, thin (especially with lateral epicondyle approach), and potentially sensate. They are advantageous features for tongue tip and hemi-tongue reconstruction. Donor site sacrifices the inessential posterior radial collateral artery, and the scar is hidden under short sleeve shirts. We believe that LAFF can be considered as the first choice flap for hemitongue reconstruction, over radial forearm free flaps.
