        Prolonged Delirium Secondary to Hypoxic-ischemic Encephalopathy Following Cardiac Arrest

        Hypoxic-ischemic brain injury encompasses a complex constellation of pathophysiological and cellular brain injury induced by hypoxia, ischemia, cytotoxicity, or combinations of these mechanisms and can result in poor outcomes including significant changes in personality and cognitive impairments in memory, cognition, and attention. We report a case of a male patient with normal premorbid functioning who developed prolonged delirium following hypoxic-ischemic brain insults subsequent to cardiac arrest. The case highlights the importance of adopting a multidisciplinary treatment approach involving the coordinated care of medical and nursing teams to optimise management of patients suffering from such a debilitating organic brain syndrome.

        Psychosis, Treatment Emergent Extrapyramidal Events, and Subsequent Onset of Huntington’s Disease: A Case Report and Review of the Literature

        Huntington’s disease (HD) is an autosomal dominant neurodegenerative disease characterized by a triad of progressive motor dysfunction, cognitive decline and psychiatric disturbances. The hallmark of HD is the distinctive choreiform movement disorder that typically has a subtle, insidious onset in the fourth to fifth decade of life and gradually worsens over 10 to 20 years until death. Notably, two-thirds of HD patients present with chorea and one third with mental changes. The prevalence of psychiatric symptoms is significantly higher than in the general population, and is estimated to be around 66-73%. Here, we report a unique case of subsequent onset of HD in a patient previously treated for schizophrenia and complicated by the extrapyramidal side effects to antipsychotics.

        Experimental investigation and optimization of pulse electrodeposition parameters for Ni-TiO2 coating on Inconel 617 using TOPSIS technique

        The quality of nano coating and its strength are purely influenced by deposition parameters. Pulse electrodeposition is apromising technique for material coating. Inconel 617 is a widely used alloy in high temperature and aerospace applications. Considering wide range of its applications, study of micro hardness and surface roughness of Inconel 617 is essential. Thisresearch deals with optimization of coating parameters in pulse electrodeposition of Ni-TiO2 on Inconel 617. Both, single andmulti-objective optimization techniques are selected, with input design of L9 orthogonal array. Pulse frequency, duty cycle andcurrent density are the input parameters. Micro hardness and surface roughness were determined from Coated Inconel 617. TOPSIS technique is adopted to improve micro hardness and surface roughness. Based on the experimental results, Microhardness of 474 Hv and surface roughness of 0.412 μm are achieved with remarkable improvement in closeness coefficientvalue from 0.4475 to 0.913. SEM analysis is carried out to ensure the surface quality. Moreover EDAX and XRD test to identifythe presence of coating material.

        Response surface methodology optimization of pulse electrodeposited Ni- nano SiC coatings on Al 7075 substrate

        In this research work, Ni- nano SiC coating was pulse electrodeposited on Al 7075 substrate in a watt’s bath. A 3-level, 3-factorcentral composite design (CCD) of experiments were carried out to assess the influence of pulse plating parameters, i.e.,Current Density (A/cm2), Duty Cycle (%), Frequency (Hz) on the microhardness of Ni-SiC coating. A regression model isdeveloped to predict the microhardness of the developed coating using design expert software and optimal parameters forachieving maximum microhardness were found out. The morphology of the nano SiC coating was investigated using FESEMcoupled with EDAX, and X-ray diffractometer. Corrosion studies were carried out using Potentiometric polarization andElectrochemical Impedance tests, and the corrosion results show that corrosion potential for the aluminium alloy is -0.92 Vand for the Ni- nano SiC coating it increases to -0.65 V. Tafel and Nyquist plots reveal the improved corrosion characteristicsof the pulse plated Ni-SiC coating. Significant improvement in the microhardness of the coating is achieved with a maximumhardness of 329.6 HV. Results show that about 3 time’s enhancement in the microhardness obtained with the Ni-SiC nanocomposite coating, when compared to the Al7075 substrate.

        Humoral response to viral vector COVID-19 vaccine in hemodialysis patients

        Background: The coronavirus disease 2019 (COVID-19) vaccine is not readily available in many countries where dosing interval is spaced more than ideal. Patients with chronic kidney disease, especially those on maintenance hemodialysis, have a tendency for a reduced immune response. This study was undertaken to demonstrate the distinct humoral immune response to the viral vector COVID-19 vaccine in patients with kidney failure receiving maintenance hemodialysis. Methods: The study was carried out with two cohorts: 1) patients receiving maintenance hemodialysis and 2) healthcare workers from the same dialysis center as controls, each group with 72 subjects. Participants received a dose of Covishield ChAdOx1 nCoV-19 coronavirus vaccine. The humoral immunological response was determined using electrochemiluminescence immunoassay which quantitatively measures antibodies to the severe acute respiratory syndrome coronavirus 2 spike protein receptor-binding domain. Results: All study subjects in the control group developed a humoral response (antibody titer of ≥0.8 U/mL), while only 64 of 72 in the dialysis group (88.9%) were responders. Age (ρ = –0.234, p = 0.04) and sodium level (ρ = 0.237, p = 0.04) correlated with low antibody titer in bivariate analysis. In multivariate analysis, only age (odds ratio, 1.10; 95% confidence interval, 1.01–1.22; p = 0.045) was associated with nonresponders. Conclusion: Our study demonstrated a weak antibody response of hemodialysis patients to the viral vector COVID-19 vaccine. Older age was associated with nonresponders. Evaluation of both humoral and cellular immunity after the second vaccine dose and serial antibody titers can help determine the need for booster shots.
