저자들은 하지 자색반과 고환 통증으로 내원한 알코올성 간경화증 환자에서 LCV의 원인 감별하는 과정에서 HCC와 하대정맥 막성폐쇄를 진단하고 이에 대한 치료 후 LCV의 호전된 증례를 경험하였기에 문헌고찰과 함께 보고하는 바이다. Vasculitis is a heterogeneous group of diseases that destroy blood vessel walls by inflammation. Approximately half of vasculitis cases are idiopathic, but sometimes associated with genetic factors, medicines, chronic infection, autoimmune diseases, and malignancies. Although the mechanism remains unclear, vasculitis secondary to malignancy, also known as paraneoplastic vasculitis, has been reported. It is generally associated with hematologic malignancies rather than solid malignancies and commonly presents as leukocytoclastic vasculitis or polyarteritis nodosa. We experienced a case of leukocytoclastic vasculitis in a patient with hepatocellular carcinoma and membranous obstruction of the inferior vena cava. The purpose of this study is to establish the database that could be applied to the building LCA Firstly, in this paper, the methodologies of building LCA and energy based I/O model for estimating embodied energy of building materials were examined Then, the intensity of energy consumption and CO₂ emission for building materials and construction stage were estimated The intensity database for building materials calculated by the I/O table published in 2000 was compared with the database by the I/O table published in 1995 According to the result of comparison, there were a little differences between the results calculated by the new and the previous data, and two results showed nearly the same patterns So, it could be said that the industrial activity change has little effect on the energy intensity of building materials during the past 5 years For the calculation of the energy intensity in the construction stages, the payment documents of an ad and electricity consumptions during construction for the sample buildings were surveyed and the intensity per a building gross area were calculated The database presented in this paper could be applied to the building LCA process as the basic data for analysing global worming effect caused by building construction

Objective : The aim of this study was to evaluate the morphometric changes in neuroforamen in grade I isthmic spondylolisthesis by anterior lumbar interbody fusion [ALIF]. Methods : Fourteen patients with grade I isthmic spondylolisthesis who underwent single level ALIF with percutaneous pedicle screw fixation were enrolled. All patients underwent standing lateral radiography and magnetic resonance imaging [MRI] before surgery and at 1 week after surgery. For quantitative analysis, the foraminal height, width, epidural foraminal height, epidural foraminal width, and epidural foraminal area were evaluated at the mid-portion of 28 foramens using T2-weighted sagittal MRI. For qualitative analysis, degree of neural compression in mid-portion of 28 foramens was classified into 4 grades using T2-weighted sagittal MRI. Clinical outcomes were assessed using Visual Analogue Sale [VAS] scores for leg pain and Oswestry disability index before surgery and at 1 year after surgery. Results : The affected levels were L4-5 in 10 cases and L5-S1 in 4. The mean foraminal height was increased [p<0.001], and the mean foraminal width was decreased [p=0.014] significantly after surgery. The mean epidural foraminal height [p<0.001], epidural foraminal width [p<0.001], and epidural foraminal area [p<0.001] showed a significant increase after surgery. The mean grade for neural compression was decreased significantly after surgery [p<0.001]. VAS scores for leg pain [p=0.001] and Oswestry disability index [p=0.001] was decreased significantly at one year after surgery. Conclusion : Foraminal stenosis in grade I isthmic spondylolisthesis may effectively decompressed by ALIF with percutaneous pedicle screw fixation.
Acute Spinal Epidural Hematoma Following Unilateral Laminectomy for Bilateral Decompression

We present a case of acute spinal epidural hematoma[EDH] following unilateral laminectomy for bilateral decompression[ULBD]. A 45-year-old male presented with severe multi-level spinal stenosis underwent ULBD on the left side at the L2-3 and L3-4 level. Five hours after operation, paraparesis developed along with severe bilateral buttock pain. The CT scan showed an acute spinal EDH at the L2-3 level. The acute spinal EDH was successfully decompressed after emergency decompressive surgery with performing an additional laminectomy on the contralateral side at the L2-3 level. Although ULBD is an effective minimally invasive surgical technique for treating spinal stenosis, the possibility of acute spinal EDH should be kept in mind, as happened in our case.
Effect of Various Crucibles for High Quality AIN Crystal Growth on SiC Seed by PVT Method

<P>Aluminum nitride (AlN) bulk crystals, approximately 50.8mm in diameter and up to 5mm thickness, were grown by a physical vapor transport (PVT) method in a tantalum crucible. To investigate the effect of crucible materials, various crucible materials, a graphite and TaC-coated graphite and tantalum crucible were used for the AlN growth. XRD pattern of AlN crystal grown on SiC seed in the Ta-crucible exhibited only (00<I>l</I>) peaks, indicating that AlN single crystal was successfully grown on SiC seed. The interface structure between AlN and SiC crystals was observed by a high resolution TEM.</P>

<P>To unveil the organotypic role and vulnerability of lymphatic vessels, we generated a lymphatic vessel endothelial hyaluronan receptor-1 (LYVE-1)-Cre/iDTR double-transgenic mouse and ablated LYVE-1–expressing lymphatic vessels in adult mice in a diphtheria toxin (DT)–inducible manner based on selective expression of LYVE-1 in most lymphatic vessels. Strikingly, lymphatic vessels in the small intestine and lymph nodes were rapidly ablated, but lymphatic vessels in the other organs were relatively intact at 24 hours after DT administration. Unexpectedly, LYVE-1-Cre/iDTR mice died of sepsis without visible edema at 24 and 60 hours after DT administration. The cause of death appeared to be related to acute failure of immune surveillance systems in the small intestine and draining lymph nodes. Of note, acute loss of lymphatic lacteals in intestinal villi appeared to trigger distortion of blood capillaries and the whole architecture of the villi, whereas acute loss of lymphatic vessels in lymph nodes caused dysfunction of lymph drainage and abnormal distribution of dendritic cells and macrophages. Thus, intact lymphatic vessels are required for structural and functional maintenance of surrounding tissues in an organotypic manner, at least in the intestine and lymph nodes.</P>

A 45-year-old man presented with lower back pain and pain in the right leg of 3years duration. A plain radiographic examination revealed grade I isthmic spondylolisthesis, with instability at L4-5. Computed tomography and magnetic resonance imaging demonstrated bilateral foraminal stenosis, with soft foraminal disc herniation on the right side at the L4-5 level. He underwent anterior lumbar interbody fusion[ALIF] with percutaneous posterior fixation[PF] at the L4-5 level. Without removing the posterior bony structures, removal of foraminal disc herniation and reduction of spondylolisthesis were successfully performed using ALIF with percutaneous PF. When there is no hard disc herniation or lateral recess stenosis, ALIF with percutaneous PF can be one of the treatment options for isthmic spondylolisthesis, even in the presence of foraminal disc herniation, as in our case.

<P>Novel hole blocking materials (HBMs) based on dibenzo[<I>b</I>,<I>d</I>]furan and dibenzo[<I>b</I>,<I>d</I>]thiophene molecular dimers have been rationally designed and synthesized for high-efficiency and long-lived blue phosphorescent organic light-emitting diodes (PhOLEDs). Thermal, optical, and electrochemical analyses show that [2,2′-bidibenzo[<I>b</I>,<I>d</I>]furan]-6,6′-diylbis(diphenylphosphine oxide) (DBF-d-PO), [2,2′-bidibenzo[<I>b</I>,<I>d</I>]thiophene]-6,6′-diylbis(diphenylphosphine oxide) (DBT-d-PO), 6,6′-di(pyridine-3-yl)-2,2′-bidibenzo[<I>b</I>,<I>d</I>]furan (DBF-d-Py), and 6,6′-di(pyridine-3-yl)-2,2′-bidibenzo[<I>b</I>,<I>d</I>]thiophene (DBT-d-Py) possess high thermal stability, deep highest occupied molecular orbital energy levels (−6.61 to −6.95 eV), and high triplet energy (<I>E</I>T) (2.68-2.95 eV). Blue PhOLEDs with DBF-d-PO, DBT-d-PO, DBF-d-Py, and DBT-d-Py exhibit low turn-on and operating voltages, excellent external quantum efficiency, and high current and power efficiencies. A blue PhOLED with DBF-d-Py shows the best efficiency with a maximum external quantum efficiency of 24.3%, a maximum current efficiency of 44.3 cd A<SUP>−1</SUP>, and a maximum power efficiency of 46.4 lm W<SUP>−1</SUP>. In addition, it exhibits an outstanding external quantum efficiency of 22.4% at a practical luminance of 1000 cd m<SUP>−2</SUP> and a very high maximum luminance of 88 953 cd m<SUP>−2</SUP> at 12 V. Furthermore, blue PhOLEDs with DBF-d-Py and DBT-d-Py exhibit highly improved lifetimes compared with the conventional HBM, BmPyPB, because of the efficient hole blocking by the deep HOMO energy level and the high thermal stability stabilizing hole blocking layer morphology.</P>
Anterior Lumbar Interbody Fusion with Pedicle Screw Fixation for Elderly Isthmic Spondylolisthesis

Objective : The surgical outcome of anterior lumbar interbody fusion[ALlF] with pedicle screw fixation for elderly isthmic spondylolisthesis was analyzed. Methods : Consecutive nineteen elderly patients [aged 65 years or more] with isthmic spondylolisthesis [Grade I or II] who underwent single level ALIF with pedicle screw fixation in 2002 were analyzed. Using clinical chart and mailed questionnaires, preoperative and postoperative Visual Analogue Scale[VAS] of back and leg pain and postopertive Macnab criteria were evaluated. Results : The mean age at the time of operation was 68.4 years [range 65 to 78 years]. Twelve patients underwent ALIF with percutaneous pedicle screw fixation. Seven patients underwent ALIF followed by posterior decompression and pedicle screw fixation. The postoperative complication rate was 10.5% [wound dehiscence in 1 patient and incisional hernia in 1 patient]. There was no postoperative major morbidity or mortality. At a mean follow-up duration of 30.7 months [range 25 to 35 months], 93.3% [14/15] of the patients showed excellent or good outcomes in terms of Macnab criteria. The mean VAS scores of back pain and leg pain significantly decreased after surgery. Conclusion : ALIF with pedicle screw fixation yielded favorable results for elderly isthmic spondylolisthesis in selected cases.
